This has been an exciting and
enlightening year for me. My
advice to you is to get involved.
Marguerite “Cissy” Picou, MD
We have had an exciting
year so far. Several
of your academy officers
and Sonora went
to the Southeast Forum
and learned all kinds of
information about
other states and problems
that they are having.
The one thing that
I learned is that I am
glad that I live in Louisiana.
It is incredible
to hear how hard it is
for other states to get
malpractice insurance
and with no cap getting
sued for incredibly high
settlements. We also
learned that other
states are having CME
on cruise ships and
things of this nature
and it will be something
that we look into
for the future.
In September, we had a
meeting at Beaufort
Plantation and started
getting ready for the
Winter Conference.
Please join us for some
fun on the slopes. The
operations committee
met and approved the
budget and gave
Sonora a well deserving
raise. The legislative
committee met and had
several areas of concern.
One was lowering
the malpractice cap to
250,000 dollars. I really
don't believe that will
happen, but it would
not hurt for you to
write your legislators
and tell them what a
great idea this would
be.
In October, several of
us went to San Diego.
The most exciting thing
that happened there
was that one of our
own, Dr. Michael Fleming
was elected President-elect
of the American
Academy of Family
Physicians and will be
the President next year.
We congratulate Mike
on a job well done. It
was fun and exciting to
be part of that.
Things are going well
with Sonora as our executive
vice president.
My compliments to her
and her staff. If you
have any needs, just let
her know.
Dr. Zoorob is in charge
of research for the
Academy, so if any of
you have interest in
that, please let him
know.
This has been an exciting
and enlightening
year for me. My advice
to you is to get involved.
Meningococcal Meningitis:
How Can We Help Prevent It
Every winter and spring we hear of
an epidemic of meningitis at one of
our colleges. Last year it was at Tulane.
We may not have one this year
in Louisiana, but an epidemic will
occur somewhere on a US campus.
That is why I have chosen to address
this topic. Family physicians see and
deal with adolescents more than any
other primary care specialist, and I
am sure you get questions about this
disease from parents and college students.
Meningococcal disease is a lifethreatening
infection, which is spread
from person to person through direct
exchange of respiratory secretions
via kissing and other close contact.
Common symptoms include: fever,
headache, neck stiffness, lethargy,
vomiting, and rash.
In the US, 50% of cases occur in patients
15 and older. In the 1990s,
outbreaks increased on colleges campuses
with most patients in the 15- to
24-year-old age range. Risk factors
for college students include freshman
status, living in a dormitory, being
Caucasian, smoking, and frequenting
bars. The American Academy of
Pediatrics issued recommendations
for healthcare providers and universities
to educate freshman students
about the increased risk of meningitis
and the benefits of vaccination.
Our academy recommends vaccination
to patients with compliment deficiencies
and asplenia. It also recommends
vaccinating travelers to
epidemic countries. Freshman students
in college need to be counseled,
especially if they live in dormitories,
so they can make informed
decisions about vaccination. Although
the AAFP recommends that
colleges take the lead in providing
such education, meningitis is still a
burden on family physicians. We
must be able to respond and address
patient and parent concerns.
The US vaccination and prevention
strategies for meningitis are as follows:
1. Antimicrobial prophylaxis for
close contacts with patients.
2. Routine vaccination of higher
risk populations, such as military
recruits.
3. Mass vaccination in response to
community outbreaks defined as
two to three cases within three
months or an attack rate of 10
per 100,000 individuals.
4. Information for freshman students
and parents about increased
risk and availability of
the vaccine.
I hope I have given you a brief summary
of meningococcal disease and
methods of prevention and vaccination.
Roger Zoorob, MD, MPH
\It is worth mentioning that new
sereotype combination conjugated
vaccines are in the pipeline and have
already started to being administered
in Europe. They will be used in the
US in the future.
F R O M T H E E V P
With the Annual Assembly and all of
the post-meeting activities behind us, I
would like to take this opportunity to
thank the Board of Directors and YOU,
the members of the Louisiana Academy
of Family Physicians, for giving
me the opportunity to serve you as the
EVP. It has been a pleasure serving you
in the various capacities I’ve served in
for over 6 years. I’ve had the opportunity
to work in every capacity within
the LAFP office.
I am very appreciative of the support
the Board of Directors, and Executive
Committee members, in particular,
have given me in this time of transition.
Their vast experience and expertise has
been very beneficial to the smooth operation
of the headquarters office and
to the success of our 2002 CME meetings.
For that, I’m very grateful.
I humbly accept this immense responsibility
of serving as the administrative
leader of this great organization. I am
also very appreciative to you the members
of the LAFP. After all, if it were
not for you there would not be a Louisiana
AFP.
My immediate goals for the organization
are to maintain it’s fiscal health
and, most importantly serve you, our
membership.
I, as well as the physician leadership of
the organization, am very concerned
about the needs of the membership. It
is our goal to provide services that
would be beneficial to every member.
However, as with any organization, our
ability to meet your needs is dependant
upon meeting our financial obligations.
Thus, the LAFP Board is working on
ways to better connect the leadership to
the membership and its needs.
The LAFP, as many other AAFP Chapters,
is struggling with declining attendance
at our CME programs. This decline
in attendance has a direct bearing
on the amount of financial support we
are able to recruit from our supporters,
thus possibly affecting our overall financial
stability. Pharmaceutical companies
are reluctant to support meetings
with less than 100 physicians in attendance.
We realize that you have many competing
factors in making a choice of how
to spend your CME dollars. Which is
why we will make an effort to offer a
variety of CME activities on relevant
topics.
This is why we need your help to ensure
the stability of your organization.
So, You may ask, “How can I help”
Well, here’s how…
1. Send me your comments & suggestions
on how to improve our
CME Offerings (which I'll forward
to the Education Committee).
2. Encourage pharmaceutical representatives
to support our CME
meetings. The more support we
get, the more we can offer, in
terms of high-quality CME with
nationally renowned speakers, and
exciting fun-filled social activities,
etc.
3. Make plans to attend at least one
LAFP meeting every three years.
Increased physician attendance,
would encourage pharmaceutical
companies to support meetings
that they perceive as valuable to
“their docs”.
4. Send us your suggestions of speakers,
locations, and other useful information.
5. Let us know how we can be of assistance
to you.
6. Let us know how you can be of
assistance to us (maybe you’re a
speaker with an expertise in a particular
area, etc.).
Plans are well underway for our 2003
CME meetings. Won’t you make plans
to join us in Breckenridge, March 1-4 th ,
for some “fun on the slopes.” The Education
Committee has worked diligently
on topic selection and program
design. So this meeting promises to be
one you won’t want to miss! I am anxiously
awaiting your registrations. You
should have received a meeting brochure
by this time. If not, please let me
know.
Also make plans to join us for our 56th
Annual Assembly in Lafayette, July
17-20th. There you’ll be given the opportunity
to earn at least 20 hours of
CME credit, as well as the opportunity
Sonora S. Norman
to participate in the LAFP’s policymaking
process during our General Assembly.
I would also ask that you take a moment
to visit our improved website at
www.lafp.org and give me your comments
and/or suggestions of information
you would like to see on the site.
The website currently has information
on our upcoming meetings (Winter
Conference Brochure & Registration
Form), as well as other interesting information.
So take time to check it out,
if you haven’t already done so. I would
also suggest that you bookmark the
page, as we will be updating & posting
new information on a regular basis.
In closing, please feel free to contact
me at 1-800-375-5237 or at academy@lafp.org,
if you have any questions
or concerns you would like myself
or the board of directors to address.
Thank you in advance for your continued
support! I look forward to working
with each of you!
A Letter to the Board of Directors and Members and
Staff of the Louisiana Academy of Family Physicians
Dear colleagues,
Dr. Robert Jarvik, the inventor of the Jarvik-7 artificial heart, once
said, “Leaders are visionaries with a poorly developed sense of fear
and no concept of the odds against them.” The history of leadership
of the LAPF is extraordinary. We are the progeny of visionaries like
Rafael Sanchez, Gerry Gehringer, Deke Sorkow, Jerry Keller and
others who have led our state organization and become leaders on a
national scale. None could have succeeded without the support of
the members of the Boards of Directors from around the state who
represent our members, and the members themselves who have
given encouragement to those leaders.
I remember my first AAFP Congress of Delegates, where I watched
as Gerry Gehringer was elected as President-Elect of the AAFP. I
wanted to one day be like him and the others I saw that day. It is
humbling to follow such leaders, the mentors that steeled me with the passion to lead our organization.
Last month Sally and I were recipients of your outpouring of support. The Louisiana hospitality room in San Diego featured a
restaurant theme, “Café Lagniappe”, serving up a Shreveport-developed specialty, crawfish and grits, to all comers to the Congress
of Delegates. Our delegation welcomed over 700 to taste the spirit of Louisiana, and to sample our hospitality. LAFP
President Cissy Picou, along with her faithful companion (is this the Cajun royalty thing, President-consort!) Brian, and a remarkable
contingent of LAFP members and staff dressed in chef’s jackets and toques to proclaim that we in Louisiana offered
“Something Special in 2002” for our Academy. Even Jerry Keller donned the toque, telling AAFP Delegate Ed Martin that he
wouldn’t have done it for anyone else!
To all of you, those who came and enjoyed beautiful San Diego and those who stayed home and did what we do best – care for
our patients – Sally, my family and I thank you from the bottom of our hearts.
With gratitude,
Dr. Fleming, accompanied by his wife . Sally , gives his
acceptance speech after being elected AAFP’s President-
Elect in San Diego, CA.
The Louisiana Academy of Family Physicians Foundation (LAFP-Foundation) is a largely membership-driven organization.
Therefore, we rely heavily on direct support from our membership, indirect support through the American Academy
of Family Physicians, and gifts from major stakeholders in our work such as hospitals and pharmaceutical companies.
Our donors have endorsed the LAFP-Foundation mission to provide quality, low-cost healthcare to all of Louisiana’s citizens.
The LAFP-Foundation supplements direct gifts with fundraising activities, including the annual silent auction. Here
again, our biggest supporters are our core family physicians’ community.
We are grateful for the support shown by the individuals listed below, who gave generously to the LAFP-Foundation in
Dear LAFP Member
“Even if you're on the right track, you'll get run over if you just sit there.” _ Arthur Godfrey
In 1987, the Louisiana Academy of Family Physicians Foundation was established to promote and support innovation
and improvement of the specialty of Family Practice. Through our combined efforts, our 2002 Foundation accomplishments
include:
• Funding for LSU-New Orleans, LSU-Shreveport and Tulane FMIG programs
• Funding for student attendance at the National Conference of Students and Residents
• Funding for student and resident attendance at the Louisiana Academy of Family Physicians Scientific Assembly
• Payment of dues for Louisiana's first year medical students
• Presentation of the Gerald R. Gehringer Family Practice Award to a Senior at LSU School of Medicine in
New Orleans.
While we are on the right track, we can not let the momentum stop. We must continue our quest to be the foremost
champion of Family Medicine in Louisiana.
Help our Foundation stay on track. Remember, it is member support that enables the Foundation to continue its mission
to attract, encourage and educate students in the field of family medicine.
Sincerely,
Michael J. Marcello, M.D.
LSUHSC-S—Alexandria
Family Practice Residency
Robin Dale, MD, PGY-II
What an exciting time for us here in
Alexandria! We are in the midst of recruiting
season once again. Even more
exciting are the newest members of our
faculty, Dr. Tuncay Ertan and Dr. Brian
Jobe.
Dr. Ertan, pulmonologist, joined our
staff in October. He originally hails from
Turkey where he completed medical
school. He completed his residency and
fellowship in pulmonology at Tulane. He
was Assistant Chief of Medicine and Director
of Medical Education for the VA
Hospital and Huey P. Long Medical Center
from 1969-1971. He had a thriving
private practice at Rapides Regional
Medical Center from 1971-1996, where
he began the respiratory care unit. In
1996, he semi-retired to Colorado where
he started another cardiopulmonary unit at
Routt Hospital in Steamboat Springs. To
our good fortune, he could not stay away
from his love of medicine and teaching.
As such, he agreed to join our faculty and
has already boosted our knowledge of
inpatient medicine logarithmically. The
residents are enjoying rounds treme n-
dously and learning new skills each day.
Dr. Ertan is an avid tennis and golf
player. He is also a former Olympian as
he played table tennis in the 1952 Olympic
Games as the national champion for
Turkey.
Dr. Jobe is a board certified Family
Practitioner. He finished his undergraduate
degree at LSU in Baton Rouge with a
degree in biochemistry in 1989. His completed
his M.D. at LSU-Shreveport in
1994 and he finished his family practice
residency at UMC in Lafayette in 1997.
He had a private practice in Eunice for 2
years and then worked in several Emergency
Rooms until 2001. He also has had
experience in Occupational Medicine during
the last year before joining our staff.
His great variety and depth of experience
will be greatly utilized. He has a beautiful
wife and 3 handsome boys ages 5 through
8. His spare time is spent reading and
with his family. We are delighted to have
these 2 new staff.
We are very pleased to have third
year medical students from Tulane doing
clerkships with us. We hope to have many
more. Many fourth year medical students
have scheduled rotations with us to experience
our program firsthand as we are
able to accommodate most requests for
clerkships. The students continue to be
especially impressed with the volume of
procedures that we perform. Both the staff
and residents are eager to teach and love
to do so. Expanding the knowledge base
here is easy given our vast array of pathology.
Over the last few weeks, our inpatient
team has cared for a patient with
an atrial myxoma and another with
Miller-Fisher syndrome.
Our LSUFP flag football team thus
far has a winning season. Our bowling
team is in sixth place and out of running
for the title, but they are having fun no
matter what. Our last journal club, held at
the home of Drs. Chris and Amy Griffin,
was a fantastic discussion of losartan versus
atenolol therapy in isolated systolic
hypertension in the elderly. After our serious
discussion, a ping-pong tournament
ensued with all involved having a great
time. I am very happy to say that I beat
Dr. Ertan but he has revenge planned.
Dr. Laura Fairbanks and Dr. Amy
Griffin are heading our Tar Wars program
with tremendous success. Each resident
has given a presentation to local fifth
grade classes in an effort to prevent tobacco
use. Over 20 schools throughout
the central Louisiana area have heard this
message from our program this year. This
anti-tobacco message is conveyed clearly
and concisely in a way that fifth graders
can understand. We look forward to continuing
the Tar Wars in years to come.
BATON ROUGE GENERAL
FAMILY MEDICINE RESIDENCY
Joshua Summers, PGY III
The streak continues. Congratulations
to our most recent graduates on their
success with the Family Practice Board
Examination. Since the birth of this program,
we have maintained a 100% pass
rate for first time takers on the boards. We
thank our faculty and community preceptors
on their hard work in preparing us for
future success.
We are prepared to enter another successful
recruiting season. Our recruiting
committee is hard at work reviewing
many exceptional applicants. We will begin
our interview process in November,
and conclude at the end of January with
an unprecedented “second look,” where
we will highlight our ability to host the
ALSO course.
We also use this time to recruit LSU
undergraduates and expose them to the
joys of family medicine. Twenty, highly
motivated, pre-med students spend one
evening each month with us to gain an
understanding about family practice. The
program’s highlight is “A Day in the Life
of a Physician,” where students will follow
resident physicians as they see patients
in the office. We hope to someday
be recruiting these bright students to our
program to provide care for the citizens of
Baton Rouge.
As fall comes upon us, we see an
increase in collegiate and intramural athletics
here in town. We have been afforded
a tremendous opportunity in the
sports medicine domain. Our program
Director, Derek Anderson, was recently
named one of the new LSU team physicians.
As residents, we now can see LSU
athletes at an evening clinic on campus.
This has significantly improved our sports
medicine exposure when combined with
the work we do for Southern University,
many local high schools, the Senior
Olympics, and the Louisiana State Troopers
Association.
The Family Medicine Residency
does as much as we can, to aid the community
we serve. Each month we provide
meals for the women and children of Baton
Rouge, who have fallen on hard times.
With the help of our residency coordinator,
Connie Rome, our office manager,
Loretta Pouciau, and my wife, KC, we
prepare and serve meals at the St. Vi n-
cent’s DePaul Women and Children’s
Shelter. It has been an excellent opportunity
to see people turn their lives around.
We have seen many women gain new
employment and move into their own
housing. The greatest reward is to see
those you have helped turn around and
help others get back on their feet.
We have also made great strides to
rebuild the community around us. With
the help of the Mid City Redevelopment
Alliance, we have helped to renovate
houses and paint schools, in order to improve
the lives of our patients. We have
gained a reputation in the community as
not only quality physicians, but also people
who truly care for others.
It is easy to see why a community
based training program is superior to a
university setting. It has been an excellent
opportunity to see the private patients of
our community preceptors in their offices.
Patients are very receptive to having
young doctors take an interest in their
care.
We are also more prepared for life
after residency training, as our program
runs like a group practice. The Family
Health Center is a freestanding facility
with lab and x-ray capabilities. Along
with excellent patient care, the residency
provides extensive training in practice
19
management and coding. This has certainly
decreased my anxiety, as I prepare for private
practice in the summer.
I have thoroughly enjoyed my time
training in Baton Rouge. Though initially
drawn to return to my Florida roots, the
relationships forged and great Louisiana
cooking, will likely keep me in Baton
Rouge. I suppose I’ll need to form a new
Gatorclub, with whom I can spend my Saturdays
next fall.
Lake Charles Memorial Hospital
Rick Jeansonne, PGY2
Happy Holidays from the LSU Family
Practice Residency at Lake Charles Memorial
Hospital. It has been an exciting year
for everyone. The new interns are quickly
coming out of their shell and have truly
been remarkable and aggressive in taking
care of patients and getting plenty of procedures
under their belts. Our third year residents
have their futures determined. Chris
Achee, MD and James McNally, MD will
be opening a joint practice in Jennings, LA.
Dan Coffee, MD will be moving back to
his home state of New York and doing ER
work. Stacey Mayeaux, MD has signed
with Doctors Hospital of Opelousas to join
fellow alum Dr. Laura Braham. And finally,
Carolyn Hutchinson, MD has tentative
plans to move back to her hometown
in Monroe, LA.
The Second year class is excited about
all the moonlighting opportunities here in
SW Louisiana and we have been working
hard to get all of our numbers and papers in
order to venture out into this new part of
our lives.
Again, we are also proud parents; Michael
Jeansonne, Elizabeth Achee, Evan
(Daughter of alum Darren Duet), and Alyson
Baquet round out the new additions to
our family, all of which were born in the
past 2 months. Expecting a baby at the
New Year is Michelle McNally, wife of
“Chief” James McNally.
November 8-9 marked the Annual Fall
Festival Weekend in which we entertained
over 17 medical students and spouses. Friday
night was the Gumbo and Bonfire on
the Calcasieu River, with live music by
some of the residents. I also wish to add
that no wildlife were harmed in the musical
fiasco, as Tuan Pham, MD sang “Living on
a Prayer” by Bon Jovi (his trademark). Saturday
night was the tailgate party at my
house followed by a walk to the McNeese-
SW Texas Football game, with tickets
complimentary of our Sports Medicine faculty,
Dr. Bryan Barootes. Numerous faculty
were present from the departments of
Cardiology, OB-GYN, Medicine, and Surgery
to stir the excitement of our program
in the hearts of interested students. It looks
like it’ll be another great year of interviewing
and the applicants get better and more
numerous each year. We should have six
great new residents again next year!
All is quiet on the western front (of
Louisiana), and we wish everyone a Merry
Christmas and a Blessed New Year.
LSUHSC-Shreveport
Residency Program
Kim J. Hunter, Coordinator
Can you believe it is already November,
our PGY-1's are nearing the 6 month
mark. They are doing a great job, adapting
well to seeing patients and learning a lot
during their rotations. ERAS is up and running,
fast and furious. We have begun interviewing
and have scheduled several future
interviews. This is a very exciting
time, so far thirteen of our applicants are
LSU students. Debbie Henry, Academic
Coordinator did a great job hosting and
arranging our resident fair in October. It
was a great success. We had 20 Family
Medicine programs to participate from
Texas, Arkansas, Mississippi and Louisiana.
Student participation was outstanding.
We would like to welcome to our staff,
full time faculty member, Patrick B. Wood,
MD Assistant Professor of Family Medicine,
Co-Director, Primary Care Pain Management
Clinic Adjunct Assistant Professor,
in the Department of Anesthesiology.
Dr. Wood completed his residency in Family
Medicine at LSU Health Sciences Center
in Shreveport following graduation
from medical school from the same institution.
Following his residency, he undertook
a yearlong fellowship in Psychopharmacology
Research in the Department of Psychiatry.
His primary research focus is the
Fibromyalgia Syndrome. Dr. Wood serves
as coordinator for the Behavior Medicine
curriculum for the Family Medicine residency
program. We would also like to
welcome home Randy Richter, MD. Dr.
Richter has been in Germany since July
2002 serving our country as a member of
the Army Medical Unit. We look forwarding
to him returning to work November 18,
2002. Several of our faculty members have
submitted packages for promotions: J. Bart
Pope for Professor and Rush-Kolodzey,
Minao Baudoin and Jeffrey German for
Associate Professor. Congratulations to
each of them.
Congratulations goes out to Dr. Patrick
Deere, 2nd year and bride-elect Kelly, announced
their engagement and will tie the
knot December 7, 2002. Our resident, faculty
and medical students were actively
involved again in the “Tar Wars” effort.
Along with the support of AHEC they presented
this program to over 500 fourth and
fifth graders in October.
University Medical Center
Lafayette
David Reehlmann, LCSW
Hurricanes, hurricanes, hurricanes . . .
and the occasional tornado! Those should
become our bywords, some of us here in
Lafayette feel. While we experienced the
arrival of tropical storm Isidore relatively
unscathed, the arrival of Lili exactly a
week later was another matter. Thank
goodness she had calmed down from the
anticipated level IV to a level II before
making landfall. Still, her arrival was
worse than many/most expected. While the
serious damage was generally confined to
the “other” living things we share our globe
with (namely trees) the toll on them was
great. At this writing 6 weeks later, the
cleanup of debris in local neighborhoods
seems to only now be winding down. It
almost seems unusual to no longer see that
material piled car-high almost continuously
along the sides of many residential streets.
Still, we were lucky. No one was
killed. The inconvenience of the entire city
being without electricity from hours to a
week or so and the extra work of cleaning
up seem insignificant compared to what
could have been. Buildings can be repaired,
and other trees can be planted.
During the storm, Family Practice
Center personnel were required to report
for duty. Children and/or other family
members could be brought along; although
no one actually brought their children. Personnel
stayed for 12 or more hours until the
worst had passed. Two of our residents,
Tom Price and Vanessa Thomas were on
hand at the Heymann Center to provide
care to medically fragile homebound patients
who had been brought to that facility.
Reports are they were quite busy.
It was anything but an ill wind that
enabled us to bring Alan Broussard, MD
onto the full- time faculty on 1 September
’02. Alan, a former graduate of this program,
has been practicing in this area for a
number of years. He was voted “Preceptor
of the Year” by the residents and given the
award for it at last year’s graduation ceremony.
We are excited about his being here.
Welcome, Alan!
Results were relatively recently obtained
from the ACGME advising us of the
continuation of our full accreditation. We
are proud of that and appreciate the hard
work of all in the program, hospital and
(Continued on page 21)
other locations who contributed to that
accomplishment.
This year’s American Board of Family
Practice In-Training Examination was
taken by all residents nationally on Friday,
November 1 st . As always, we are eager
to see those results.
Recruiting season appears to be well
underway. We have been attending various
residency fairs and enjoying interviews
with applicants. It would be a bit
easier if the Electronic Residency Application
System was fully functional in all
of our offices here, but we are managing,
somehow.
The Pennington Biomedical Research
Center held an obesity workshop at UMC
on September 30 th . First year FP residents
from Lafayette, Alexandria and Lake
Charles attended the half-day workshop,
as did the new Lafayette IM residents.
The workshop, “Interactive and Practical
Approaches to Managing Your Overweight
Patient,” is actually a production
of C.O.R.E (Centers for Obesity Research
and Education). C.O.R.E. encompasses
eight of the leading centers involved with
the medical management of overweight
and obese persons in the United States.
Pennington and the other nationally recognized
centers have combined forces to
disseminate knowledge about obesity
treatment to the medical community. We
appreciate and thank the Pennington people
for providing this experience to our
residents; and also thank Al Rees for his
efforts in scheduling and facilitating the
activity.
Missing M.D.s
Wanted: more geriatricians to treat the aging
By ANDREW GOLDSTEIN
Stephen Bzdok began experiencing seizures
at age 65, and doctor after doctor was unable
to discover the cause. A Las Vegas lounge
singer for more than 20 years, Bzdok nearly
became homebound. The internists and neurologists
he saw never took the time — and
didn’t have the expertise — to figure out what
was wrong. Then an especially fierce seizure
put Bzdok in a coma for four months. His
family, assuming the worst, sold most of his
possessions. One day, miraculously, Bzdok
woke up. This time he consulted a geriatrician
— a doctor with special training in care
for the elderly — who soon discovered the
problem: a heart murmur. Bzdok had a pacemaker
implanted, and has been seizure free
ever since. The fun-loving sexagenarian says
he feels as if he were 50 again. He lives on
his own, walks half a mile a day and spends
weekends on a speedboat.
If only the rest of the nation’s seniors were as
lucky. When politicians and journalists talk
about problems in elder care, they usually
point to horrific abuse in nursing homes or to
retirees who are forced to skip meals to pay
for prescription drugs. But the most serious
crisis in elder care has been largely ignored:
not enough doctors who know how
to diagnose and treat aging’s special ailments.
Out of 650,000 physicians in the U.S.
today, just 9,000 are certified in geriatric
medicine. And that number is shrinking, as
many quit the poorly paid field or retire. Only
3 out of 144 U.S. medical schools have a
full department in geriatrics (all have a
department in pediatrics), and fewer than 3%
of medical students take even one course in
geriatrics, according to the International Longevity
Center. During the next 30 years,
more than 70 million baby boomers will
reach age 65. Who will care for them
Medicine for the elderly is a different science
from medicine for the young or the middleaged.
Seniors face a range of age-related
ailments — dementia, Alzheimer's, arthritis —
and typically swallow more than four medications
a day. A doctor’s attention to how drugs
interact can mean the difference between
saving a life and taking one. Many geriatricians
say they devote much of their efforts to
sorting out the complications caused by their
patients' previous doctors. Each year 17% of
elderly Americans are hospitalized after experiencing
a dangerous drug interaction or
taking incorrect dosages. The American
Medical Association says nearly 7 million
seniors a year are routinely prescribed drugs
that are too risky for them.
Mary Richert, 79, of Jennings, La., had been
living on her own, taking trips as far away as
Alaska, until she began suffering from memory
and balance problems. She stopped driving
and traveling and became depressed. At
the urging of her children, she spent 18
months trying to find a doctor who could help
her. But the message from the series of internists,
urologists, neuropsychologists and
neurologists she saw was always the same:
“Mama's problems are because Mama’s getting
older.”
That wasn’t the reason. Two years ago,
Richert went to see Charles Cefalu, one of
the top geriatric specialists in the U.S. “He
was different from any doctor I had been to,”
says Richert. “He took his time.” Cefalu’s
thorough examination helped him figure out
the cause of Richert’s seeming dementia: her
brain — like that of a child born with “water
on the brain”--had stopped draining cerebral
fluid. Cefalu sent Richert to a neurosurgeon,
who placed a shunt inside her brain to act as
a drain. Her health improved dramatically.
Today she gardens, travels and — to the
consternation of her children — is driving
again.
It’s an inspirational tale, yet what saved
Richert is what keeps doctors away from
geriatrics: it takes time to diagnose elders’
problems. A physician is reimbursed by insurers
for a typical office visit of 15 or 20 minutes.
But a geriatrician needs on average at
least an hour for an initial assessment. While
a general internist can see 2,000 to 3,000
patients a year, a geriatrician sees fewer than
500 patients, making for a smaller income.
Says Joseph Ouslander, head of Emory University’s
geriatric-medicine program: “I can
go into my clinic and spend an hour with a
very complicated 90-year-old patient, identify
her 10 medical problems, sort out her dozen
medications, talk to her children. And I get
the same reimbursement as the dermatologist
who spends 30 seconds shaving off a
mole.”
A geriatrician’s income is almost entirely dependent
on the amount that Medicare pays
for each patient, in effect leaving the doctor’s
salary up to the Federal Government. And
this year Congress has cut Medicare reimbursements
5.4%. Louisiana Senator John
Breaux, who chairs the Senate Aging Committee,
is one of the few members of Congress
to call attention to the shortages in
elder care, most recently at a hearing last
February. Yet Breaux, a fiscal conservative,
told TIME he won’t consider raising Medicare
reimbursement rates. “That’s not in the cards
right now,” he says. Instead, he proposes
forgiving student loans to doctors who go into
geriatric medicine.
A handful of medical schools have begun to
boost their geriatric programs, despite the
lack of student demand. (One medical resident
said he never took a class in geriatrics
because “old people are gross.”) Cefalu
started an expansive program at Louisiana
State University that he hopes will act
as a model for other schools. But when
Greg Sachs, chief of geriatrics at the University
of Chicago, launched a geriatrics fellowship
in 1986, only a few young doctors entered
the two-year program, in fits and starts,
and interest dried up altogether in the mid-
’90s. This year Sachs has just a single fellow.
When the legions of baby boomers begin to
retire, many will need the kind of geriatricians
who have the skills to save people like Bzdok
and Richert. Jennifer Moore, 30, hopes to be
such a doctor. Inspired to go into geriatrics by
watching her grandmother take care of her
grandfather after he suffered several strokes,
Moore followed med school with a two-year
fellowship in geriatrics. But now she’s having
trouble finding a satisfying job. “How can I
provide good care in the manner in which I
was trained,” she asks, “when all I get is 10
minutes with each patient” Unwilling to compromise
her standards, Moore is looking to
teach geriatric medicine instead. Now the
question is whether she can find some students.
