<strong>ACT</strong>: <strong>Requesting</strong> <strong>Accommodations</strong><br />

<strong>Please</strong> <strong>see</strong> <strong>the</strong> <strong>information</strong> below regarding <strong>the</strong> request for accommodations on <strong>the</strong> 2011-2012<br />

<strong>ACT</strong>:<br />

Review and print out <strong>the</strong> 6 page PDF, available at http://www.act.org/aap/pdf/ext-time.pdf<br />

Basic Information:<br />

● The bottom of page 1 provides <strong>information</strong> on <strong>the</strong> criteria for requesting extended<br />

time, “Eligibility Requirements”: (i.e. Professionally Diagnosed Disability: testing or reconfirmation<br />

of testing within <strong>the</strong> last 3 years and a current IEP, 504 or Accommodation<br />

Plan).<br />

● The last page of <strong>the</strong> document provides a chart which differentiates what constitutes<br />

extended time testing versus special testing.<br />

Filling out <strong>the</strong> application:<br />

● Fill out side 1 (student and parent must sign and date <strong>the</strong> bottom) and <strong>the</strong> top of side 2.<br />

● If student needs accommodations in addition to extended time, review <strong>the</strong> options in<br />

section D.<br />

● Bring <strong>the</strong> <strong>ACT</strong> application, extended time application and check for <strong>ACT</strong> to Mrs.<br />

Christen in Student Services (2nd floor). Mrs. Christen fills out G, H, I and will<br />

compile <strong>the</strong> student’s psychoeducational testing and 504 plans, Student Success Plans<br />

and mail everything to <strong>ACT</strong>.<br />

● <strong>ACT</strong> applications and mailing envelopes can be picked up in <strong>the</strong> counseling office. The<br />

extended time application can be printed from <strong>the</strong> above link.<br />

● Remember to include <strong>the</strong> Marian High School Code, which is 281-718, on <strong>the</strong> back of<br />

your <strong>ACT</strong> envelope and check <strong>the</strong> boxes indicating you have enclosed payment, chosen<br />

a test date and at least one test center code.<br />

If a student would like to test again, on a national test date, with extended time, please <strong>see</strong> page<br />

3 (right-hand column) for registration options.<br />

<strong>Please</strong> note <strong>the</strong> postmark deadlines for requesting extended time:<br />

Students must provide Mrs. Christen with <strong>the</strong>ir materials (<strong>ACT</strong> application, extended time<br />

application, check and <strong>ACT</strong> mailing envelope) at least two weeks prior to <strong>the</strong> deadline listed<br />

above to ensure ample time to ga<strong>the</strong>r <strong>information</strong>.<br />

If you are taking <strong>the</strong> February 11th, 2012 <strong>ACT</strong>, please submit your materials to Mrs. Christen no<br />

later than December 14, 2011.<br />
