Dear Students and Parents, <br />
The <strong>Bailey</strong> <strong>Health</strong> <strong>Center</strong> welcomes you to Lafayette College. We are committed to providing <br />
<strong>student</strong>s with <strong>the</strong> best possible medical care. Our services include comprehensive primary care, <br />
treatment of acute injuries and urgent medical problems, gynecologic care, and psychiatric <br />
care. Our staff includes nurses, mid-‐level providers, and physicians who are board certified in <br />
family practice, emergency medicine, psychiatry, and orthopedics. The <strong>Health</strong> <strong>Center</strong> provides <br />
medical care without utilizing health insurance. <strong>Health</strong> insurance may be used for filling <br />
prescriptions at our pharmacy, lab tests, and orthopedic devices. <br />
There are several important health forms which must be completed and returned to <strong>the</strong> <strong>Health</strong> <br />
<strong>Center</strong> by July 15 th . The <strong>Bailey</strong> <strong>Health</strong> <strong>Center</strong> utilizes an electronic health record system to keep <br />
track of vital health information on each <strong>student</strong>. You will need to access this system through <br />
an Online Student <strong>Health</strong> (OSH) portal at https://lafayette.medicatconnect.com. Enter your <br />
network ID and password to <strong>the</strong> OSH portal. All first year <strong>student</strong>s are required to submit <strong>the</strong> <br />
following forms to <strong>the</strong> <strong>Health</strong> <strong>Center</strong>: <br />
1. Immunization Record and TB Questionnaire-‐ This form should be downloaded and <br />
brought to your health care provider to verify that you have received <strong>the</strong> required <br />
immunizations. Pennsylvania State Law requires that ALL <strong>student</strong>s residing in college <br />
owned housing be vaccinated against meningitis or sign a waiver. <br />
2. Physical Exam-‐ This form should be downloaded and brought to your health care <br />
provider. The NCAA mandates Sickle Cell Testing for all varsity athletes. <br />

3. <strong>Health</strong> History-‐ Students should complete this on <strong>the</strong> OSH portal and submit it online. <br />
4. Emergency Notification and Medical Insurance Form-‐ Students and parents should <br />
complete this on <strong>the</strong> OSH portal and submit online. <br />
In anticipation of <strong>the</strong> Affordable Care Act mandate, <strong>the</strong> College requires that all <strong>student</strong>s have <br />
health insurance while attending Lafayette. You should have received a memorandum which <br />
describes <strong>the</strong> details of this important policy. Students will be automatically enrolled in our <br />
Student <strong>Health</strong> Insurance Plan serviced by University <strong>Health</strong> Plans. This enrollment will result in <br />
an automatic premium charge of $1,577 to each <strong>student</strong>’s account. The charge will only be <br />
removed <strong>from</strong> <strong>the</strong> <strong>student</strong>’s account if <strong>the</strong> <strong>student</strong> waives <strong>the</strong> enrollment and provides proof of
comparable coverage. We strongly recommend consideration of <strong>the</strong> plan even if you have <br />
o<strong>the</strong>r health insurance. Many <strong>student</strong>s discover that <strong>the</strong>re are limitations to <strong>the</strong>ir insurance <br />
coverage while attending school. To waive enrollment access <strong>the</strong> Lafayette College link at <br />
www.universityhealthplans.com . <br />
Finally, <strong>the</strong> College is expanding its effort to educate <strong>student</strong>s on several health and wellness <br />
issues that impact college <strong>student</strong>s. The College requires that all incoming <strong>student</strong>s complete <br />
an online education course called “My Student Body” which covers <strong>the</strong> most significant <br />
behavioral health issues on campus: alcohol abuse, drug abuse, and sexual violence. Students <br />
are required to complete each module and pass a short quiz. Students will be unable to register <br />
for spring classes unless <strong>the</strong>y have completed this course. To take <strong>the</strong> course, access <strong>the</strong> link to <br />
My Student Body on <strong>the</strong> <strong>Health</strong> <strong>Center</strong> web site and follow <strong>the</strong> instructions. <br />

<strong>Health</strong> <strong>Center</strong> is open Monday through Friday <strong>from</strong> 8AM until 5PM and on weekends <strong>from</strong> <br />
