MEMORANDUM OF AGREEMENT<br />
KNOW ALL PERSONS BY THESE PRESENTS:<br />
This agreement entered into by and between:<br />
The COLLEGE OF EDUCATION, a department within the EASTERN VISAYAS STATE UNIVERSITY,<br />
hereinafter referred to as the “FIRST PARTY”<br />
and<br />
The EASTERN VISAYAS STATE UNIVERSITY, a state university created and existing under Philippine law,<br />
with principal address at Tacloban City, Leyte, and represented by its President, DR. CATALINO B.<br />
BELTRAN, herein referred to as the “SECOND PARTY”<br />
WITNESSETH that:<br />
WHEREAS the FIRST PARTY, in its continuing mission to training teachers for the progress and social<br />
development <strong>of</strong> the country, would like to provide scholarship grants to financially challenged and<br />
academically superior students at the SECOND PARTY;<br />
WHEREAS this “Scholarship for Future Educators,” herein referred as the SCHOLARSHIP PROGRAM, is<br />
being initiated by the FIRST PARTY upon the requests <strong>of</strong> students at the SECOND PARTY;<br />
WHEREAS through the efforts and representation <strong>of</strong> the FIRST PARTY, a scholarship fund in the amount <strong>of</strong><br />
SEVENTY-FIVE THOUSAND PESOS ONLY (₱75,000; USD 1,744) covering tuition assistance to fifteen (15)<br />
students, will be raised by the FIRST PARTY by soliciting individual private donors;<br />
WHEREAS an active partnership between the FIRST PARTY and the SECOND PARTY is needed for the<br />
effective implementation <strong>of</strong> the SCHOLARSHIP PROGRAM;<br />
THEREFORE the parties have agreed on the scholarship program under the following terms and conditions:<br />
A. OBLIGATION OF THE FIRST PARTY:<br />
1. Provide a fund in the amount <strong>of</strong> SEVENTY-FIVE THOUSAND PESOS ONLY (₱75,000; USD 1,744) for<br />
the first semester <strong>of</strong> SY <strong>2012</strong>-2013. The allocation per scholar/beneficiary shall not exceed FIVE<br />
THOUSAND (₱5,000) and may only be applied to tuition costs or additional university fees. School<br />
fees, miscellaneous fees in excess <strong>of</strong> (₱5,000) shall be covered by the student/beneficiary.<br />
2. Accept applications and conduct interviews <strong>of</strong> the applicants, establishing that all applicants meet<br />
stated financial and academic requirements, and evaluate the applicants under the following<br />
criteria:<br />
a) At least three (3) members must be on the selection committee.<br />
b) The committee must use an objective selection rubric similar to the attached example, with<br />
scores from all panel members averaged.<br />
c) Under no circumstances may scholarships be “reserved” for acquaintances or relatives. The<br />
interview and supporting application is the only basis that may be used for awarding<br />
scholarships.<br />
d) Priority shall be given to fourth year students. Should additional slots be available, they may be<br />
filled by third, second, or first year students.<br />
3. Select 15 scholars and inform all scholar applicants at least two weeks before the beginning <strong>of</strong> the<br />
semester <strong>of</strong> the results. Use the preferred contact information provided by the student on the<br />
application form.
B. OBLIGATION OF THE SECOND PARTY<br />
1. Provide scholarships for up to FIFTEEN (15) students for one semester taking up any degree in the<br />
College <strong>of</strong> Education in the SECOND PARTY. Should less than 15 students be awarded scholarships,<br />
surplus from the original funding will be retained and applied to the next semester’s scholarship.<br />
2. Take charge in the advertisement <strong>of</strong> the scholarship through the Office for Scholarship and Financial<br />
Assistance.<br />
3. Furnish the FIRST PARTY grade reports <strong>of</strong> all scholarship applicants.<br />
4. Furnish the FIRST PARTY a financial report on the exact amounts credited and debited from the<br />
University Cashier’s account named “Scholarship for Future Educators” and the operational status<br />
<strong>of</strong> the Scholarship Program.<br />
C. QUALIFICATIONS AND REQUIREMENTS FOR AWARDEES<br />
Scholarship awardees must:<br />
1. Demonstrate that their parents are incapable <strong>of</strong> paying all college expenses. This may be<br />
established by furnishing a tax exemption note from the Bureau <strong>of</strong> Internal Revenue OR having<br />
certification signed by a parish priest or barangay kagawad;<br />
2. Be enrolled in a course within the College <strong>of</strong> Education;<br />
3. Have no failing grade or incomplete grades;<br />
4. Maintain a grade point better than 2.2;<br />
5. Not be a recipient <strong>of</strong> any other concurrent scholarship;<br />
6. Attend an orientation session at the beginning <strong>of</strong> the semester;<br />
7. Create a thank-you letter, to be distributed to scholarship donors.<br />
D. RECISSION/CANCELLATION OF SCHOLARSHIP:<br />
Failure on the part <strong>of</strong> the grantees to meet any <strong>of</strong> the aforementioned “QUALIFICATIONS AND<br />
REQUIREMENTS FOR AWARDEES” will be sufficient ground for terminating the assistance.<br />
