10”x12” envelope addressed to the institution with no return address, and 3 unaffixed stamps for online applications<br />
or 5 unaffixed stamps for paper applications. Common Application <strong>School</strong>s and electronic submission schools<br />
do not require envelopes and postage.<br />

MTHS PROCEDURES FOR REQUESTING TRANSCRIPTS<br />
2012-2013<br />
• A transcript release form must be completed for all colleges, universities, scholarships, & NCAA.<br />
• Indicate the individual colleges/institutions in the appropriate box.<br />
• All “official” transcripts must be sent directly from the high school. At no time can a student or<br />
parent handle or mail an official transcript.<br />

TRANSCRIPTS<br />
Complete a TRANSCRIPT RELEASE FORM (reverse side) and submit to the Guidance Office Staff. Allow up to<br />
10 school days for processing.<br />
You will also need to electronically complete a transcript request in Naviance for each school.<br />

RECOMMENDATION<br />
Counselor/<strong>School</strong> Report <strong>Form</strong> should be submitted with the transcript release form. Check in Naviance to<br />
determine if you need an envelope addressed directly to the<br />
College/University with the appropriate postage attached (indicated by the postage symbol), otherwise<br />
forms will be submitted electronically.<br />
Please complete the “Recommendation Letter Questionnaire” in the ABOUT ME section of Naviance before<br />
requesting letters of recommendation from your teachers and counselor. The FERPA Waiver also must be<br />
completed. You must request teachers and your counselor via Naviance in the “Colleges I’m Applying To” section.<br />
Please allow 3 weeks for processing letters.<br />

APPLICATION (CA)<br />
If the student applied electronically, the CA SSR Counselor <strong>School</strong> <strong>Form</strong> will be completed and submitted<br />
online and an electronic transcript will be uploaded to the student’s application. The teacher(s) & your<br />
counselor that you requested on Naviance will upload the CA Teacher <strong>School</strong> <strong>Form</strong>(s). Please check on CA<br />
to see how many teacher recommendations are needed for each institution. You will need to enter your<br />
CA username and password in Naviance in the FERPA waiver for the SSR to be completed by your<br />
counselor.<br />

MID-YEAR REPORTS<br />
Know your deadlines and submit materials in time to meet those deadlines. Do not include school holidays<br />
when you calculate the 10 schools days needed to process.<br />
If your college requests Marking Period Grades, Mid-Year Reports, or <strong>School</strong> Report forms, you must<br />
download and submit the completed mid-year grades form from Naviance and return it to the Guidance<br />
Office<br />

All ACT and SAT scores reported to the high school will appear on your transcript. Some<br />
colleges will accept these scores for admission purposes. Check with colleges to see if they require test<br />
scores directly from the testing agency. If you want to remove scores from your transcript, please submit a<br />
written request with your signature and attach it to this form and return it to the school counseling office<br />
staff. **Warning: The scores will be removed PERMANENTLY.<br />
The first five (5) institutions will receive requested transcripts free of charge. For each<br />
additional transcript request, there is a $3 fee. <strong>Transcript</strong> fees are payable by check or<br />
money order to “<strong>Manheim</strong> <strong>Township</strong> <strong>School</strong> <strong>District</strong>.” Payment must be received before the transcript will be<br />
released.<br />
There is a $3 fee for each alumni transcript requested. All transcripts requested for<br />
scholarship purposes are exempted from fees. Students who are experiencing financial hardship should<br />
contact their counselor to determine eligibility for fee waiver programs.
