NOTICE OF SALE THROUGH MARKET DETERMINED PRICING
The NATIONAL FOOD AUTHORITY(NFA) Regional Office No. XII, Tacurong City
invites all interested NFA Licensed Grains
businessmen/wholesalers/retailers/traders, ricemilers and legitimate farmer
organizations (FO’s) to participate in the SMDP of 31,005 bags more or less
local rice stocks aged one (1) year and above and to submit sealed offers for
the SMDP of the following volume of Local rice stocks presently stored at NFA
Warehouses in Region XII as follows:

The sale of more or less 31,005 bags Local Rice Stocks at the Indicative Price
of Twenty-Two Pesos & 50/100 (Php22.50) per kg shall be on AS IS, WHERE
IS, NO SELECTION AND NO SEGREGATION BASIS.

The bidding forms including the other provisions stipulated in the terms and
conditions shall be available at the Operations Section, Regional Office No. 12,
Tacurong City c/o Mr. Proceso M. Polo, Jr. and shall be issued only to offerors
who have submitted the SMDP requirements and upon payment of a nonrefundable
bidding fee of Php500.00 for NFA Accredited Retailers and NFA
Accredited Farmer Organization under the Institutionalized Procurement
Program (IPP) category and Php1,000.00 for Open to all NFA Licensed Grains
Businessmen as Wholesalers/Traders/Ricemillers and Legitimate Farmer
Organizations category.
All interested and qualified bidders can purchase SMDP documents upon
submission to SMDP Secretariat of the following pre-qualification documents for
identification of the bidder’s category where applicable:
1. Letter of Intent to Participate in the SMDP signed by the licensee or
Grains businessmen, and for FO.’s , the Chairman or President.
2. Certified photocopy of the Certificate of Grains Business License, license
plate, sticker and official receipt of payment of license fees valid as of
December 2010.
3. Certified photocopy of the valid City/ Municipal License/ Business Permit if
engaged in the grains business.

4. Photocopy of the Master’s Passbook valid as of December 2010 as
certified by the NFA Provincial Manager.
5. Certificate of Membership to be issued by the PFAB Chairman or the PFAC
Chairman in the Province where the Farmer Organization is located as
designated by the PFAB or Certificate of Membership to be issued by the
National NAFC Chairman or Provincial NAFC as designated by NFAC
National Chairman or President of the Irrigator’s Association.
6. Certified photocopy of the Certificate of Registration issued by the
Regional/Provincial Office of the Cooperative Development Authority
(CDA) which ever is applicable.
7. Certified photocopy of the Retailer’s Passbook issued by the NFA as
certified by the NFA Provincial Manager.
8. Certified photocopy of the SEC Registration Certificate, for Corporations.
9. Certified photocopy of the DTI Registration Certificate, for Sole
Proprietorship and Partnership.
10. Special Power of Attorney (SPA) for the representative, if sole
proprietorship/partnership business and/or Board Resolution or
Secretary’s Certificate, if corporation/cooperative indicating that they are
authorized to purchase of SMDP documents, attend the pre-SMDP
conference and the conduct of the SMDP proper (submission and opening
of bids) and all other activities related thereto, such as, to sign
documents on the withdrawal of stocks in the NFA warehouses like
Warehouse Stock issue (WSI), receive the stocks for and in behalf of the
bidder. The authorized representative shall be limited to the immediate
members of the family for sole proprietorship/ partnership business and
an officer or official of the company/corporation, if corporation, and a
Board of Director in the case Farmer Organization (F.O.).
11. No single person to be covered with SPA shall be allowed to
represent more than one entity.

NFA Region XII encourages all interested offerors to conduct ocular inspection
of the subject stocks prior to the pre-SMDP schedule. For further information,

THE NATIONAL FOOD AUTHORITY RESERVES THE RIGHT TO REJECT ANY
OR ALL BIDS/ OFFERS, TO WAIVE ANY DEFECT AND INFORMALITIES FOUND
THEREIN, AND TO ACCEPT ONLY SUCH BIDS/OFFERS THAT ARE
ADVANTAGEOUS TO THE AGENCY. THE DECISION OF THE COMMITTEE AS
APPROVED BY THE REGIONAL MANAGER SHALL BE DEEMED FINAL.
