The administration recommends that the Board of Education approve the “District of Location” contract with Central Islip UFSD for the 2011-12 school year and authorize the President of the Board to sign same. D. The administration recommends that the Board of Education approve the following field trips: 1. Commack High School Field Trip NYSSMA Winter Conference Rochester Conference Center Rochester, New York December 1-4, 2011 2. Mandracchia-Sawmill Intermediate School Field Trip Camp Mariah/Sharpe Reservation 436 Van Wyck Lake Road Fishkill, New York May 9-11, 2012 3. Commack High School Field Trip Varsity Basketball Trip to Washington, DC December 28-30, 2011 E. The administration recommends that the Board of Education accept the following donations: 1. Donation from K. Stanley Photography of $80.00 to benefit Commack High School Athletics. 2. Donation from The Weight Room Plus Wanda’s Work Out, Inc. of a leg extension machine to be used at Burr Intermediate School fitness room. 3. Donation to the District for the Commack High School Art Department Photography Program of 35mm cameras and equipment. 4. Donation from K. Stanley Photography of $239.00 to benefit Commack High School Athletics. 5. Donation from the Commack Soccer League of new uniforms for all four Commack Middle School Soccer Teams. 6. Donation from the Mandracchia/Sawmill Intermediate School PTA of $2,500.00 to be applied toward grade 4 field trips. 7. Donation from Commack Middle School Student Government of $2,000.00 to purchase a 46” LCD television and supplies to be used to display student work, curricular and extra-curricular activities, messages, etc. in the main lobby. F. The administration recommends that the Board of Education approve the appointment of candidates to the District’s Volunteer Program. G. The administration recommends that the Board of Education approve the opening of a new Debt Service Money Market Account with JP Morgan Chase and move funds from TD Bank to JP Morgan Chase.

The administration recommends that the Board of Education approve the Revision to Financial Information Forms for smartboard installations and authorize the President to sign same. I. The administration recommends that the Board of Education adopt Board Policy #3300 – Fund Balance. J. The Board of Education is requested to acknowledge receipt of the Independent Auditor’s Report for the year ended June 30, 2011. A. The Board of Education is requested to acknowledge receipt of the statement for professional services rendered on non-retainer matters in the amount of $22,362.89 for the period ending September 31, 2011 submitted by Lamb & Barnosky, LLP. B. The administration recommends that the Board of Education approve the lease agreement between Commack Union Free School District and Commack youth League for the rental of the area on Mandracchia-Sawmill Intermediate School Grounds for the period of September 1, 2011 through September 1, 2020 and authorize the President of the Board to sign same. XI. Board Business XII. Adjournment A. The Board of Education is requested to acknowledge receipt of the Internal Claims Auditor’s Monthly Update (see attached). B. The Board of Education is requested to acknowledge receipt of the CSE/CPSE recommendations approved on the following dates: May 2, 4, 9, 11, 18, 23, 25; June 1, 6, 7, 8, 15, 20, 2011; August 11; and September 14, 2011. C. The Board of Education is requested to acknowledge receipt of the Treasurer’s Reports and CD Rates. D. The Board of Education is requested to adopt the following resolution: RESOLVED, that the Board of Education of the Commack Union Free School District shall hold a special meeting on Thursday, November 3, 2011 at 8:00 p.m. at Hubbs Administration Center for the purpose of adjourning immediately into Executive Session; and on Thursday, November 17, 2011 at 6:30 p.m. at Hubbs Administration Center, Clay Pitts Road, E. Northport, NY for the purpose of immediately adjourning into Executive Session; at 8:00 p.m. the Board shall reconvene in public session for the purpose of holding its Regular Public Meeting. Public discussion relating to staff, students or others in which their reputation, privacy or due process rights would in some way be violated is prohibited.
