5. Donation from the Commack Soccer League of new uniforms for all four Commack

Debt Service Money Market Account with JP Morgan Chase and move funds from TD
Bank to JP Morgan Chase.

H. The administration recommends that the Board of Education approve the Revision to
Financial Information Forms for smartboard installations and authorize the President to
sign same.
I. The administration recommends that the Board of Education adopt Board Policy #3300 –
Fund Balance.
J. The Board of Education is requested to acknowledge receipt of the Independent Auditor’s
Report for the year ended June 30, 2011.
A. The Board of Education is requested to acknowledge receipt of the statement for
professional services rendered on non-retainer matters in the amount of $22,362.89
for the period ending September 31, 2011 submitted by Lamb & Barnosky, LLP.
B. The administration recommends that the Board of Education approve the lease agreement
between Commack Union Free School District and Commack youth League for the rental
of the area on Mandracchia-Sawmill Intermediate School Grounds for the period of
September 1, 2011 through September 1, 2020 and authorize the President of the Board
to sign same.

A. The Board of Education is requested to acknowledge receipt of the Internal Claims
Auditor’s Monthly Update (see attached).
B. The Board of Education is requested to acknowledge receipt of the CSE/CPSE
recommendations approved on the following dates: May 2, 4, 9, 11, 18, 23, 25; June 1,
6, 7, 8, 15, 20, 2011; August 11; and September 14, 2011.
C. The Board of Education is requested to acknowledge receipt of the Treasurer’s
Reports and CD Rates.
D. The Board of Education is requested to adopt the following resolution:
RESOLVED, that the Board of Education of the Commack Union Free School
District shall hold a special meeting on Thursday, November 3, 2011 at 8:00
p.m. at Hubbs Administration Center for the purpose of adjourning immediately
into Executive Session; and on Thursday, November 17, 2011 at 6:30 p.m. at
Hubbs Administration Center, Clay Pitts Road, E. Northport, NY for the purpose
of immediately adjourning into Executive Session; at 8:00 p.m. the Board shall
reconvene in public session for the purpose of holding its Regular Public
Meeting.
