We all dream <strong>of</strong> a healthier future. And together,<br />
we can raise awareness about heart health. You<br />
can help spread the word and encourage others<br />
to understand their risk <strong>of</strong> heart disease by<br />
getting a heart health assessment.<br />
Now is a great time to participate in <strong>Host</strong> a <strong>Heart</strong>,<br />
a program to inspire change in the people you<br />
care most about.<br />
By hosting a day <strong>of</strong> heart health assessments,<br />
your family, friends and colleagues will receive<br />
individualized assessments focused on their<br />
health needs.<br />

As a host, you can invite eight guests for an<br />
exclusive heart health assessment day in our<br />
heart center. We’ll assist with scheduling each<br />
<strong>of</strong> your guests, and we’ll send them a packet<br />
<strong>of</strong> information to help them prepare for<br />
their assessment.<br />
Your guests will be scheduled to arrive at the heart<br />
center @ 30 min intervals from 8am-11:30am.<br />
Each session will last approximately 90 min. Each<br />
guest will receive an individual assessment,<br />
including a one-on-one consultation with a cardiac<br />
nurse practitioner. Your guests will also enjoy a<br />
heart-healthy breakfast at the conclusion <strong>of</strong> their<br />
assessment.<br />
Each assessment is $60 per person. We will assist<br />
your guests at registration with their payment.<br />
You may also pay for your guests, which truly<br />
makes this a gift from the heart.<br />
