Sunfl ower Centre
The Sunflower centre aims to provide a
range of services to anyone whose life has
been affected by the diagnosis of a life limiting
illness. The diagnosis of a life limiting
illness can come as an enormous shock to
patients, their families and carers.
You may feel fearful, anxious and isolated
but at the Sunflower Centre we aim to provide
a range of services that support you
and your family, enhancing your own coping
strategies. Patients and families may
self- refer or be referred by their G.P or
Healthcare professional involved in their
care. On your initial visit you will be allocated
to a nurse, who will become your
“Key Worker”.
Art Therapy
Breathlessness management
Complementary therapies
Chaplaincy/Spiritual support
Carers Support Group
Counseling
Discharge Support Group/managed by
previously discharged patients.
Diversional Therapies
Here you will have the opportunity to
discuss your concerns and find out about
what services may be beneficial to you.
Following this assessment we will work
with you to develop your own personal
care plan, referring you on to appropriate
services within the hospice.
Your Key worker will co-ordinate and implement
your plan by meeting regularly
with you to ensure that any unmet needs
are identified and addressed. This may
ultimately involve planning for your discharge.
The Sunflower centre is open 5 days per
week from 10am until 3pm.
The Sunflower Centre services include:
Family Support including advice on
finances, benefits and housing etc.
Fatigue Management
Hairdressing
Living Well programme
Lymphoedema out-patient clinic
Occupational Therapy
Physiotherapy
Tai Chi group
Open House Drop-in information session- for those seeking further information about our services
Our services are provided either in groups or on a one to one basis for which appointments are required.
All services are provided free of charge and there is free parking.
If people have diffi culty in transporting themselves to the Sunfl ower Centre then please discuss this on
your initial assessment/ visit.
Refreshments are available throughout the day including lunch. Weekly Group Programme
