Do you remember this letter? If Yes: Are you able to speak with me now? We have Māori interviewers available if you would prefer to speak with them. If No: We are conducting a survey about the health, disability and ACC system in New Zealand. We want to speak to people who have had contact with the health system sometime in the past 6 months. This is with people like your GP, A&E, a hospital, a specialist or x-rays, physios, occupational health people and so on. It also includes ACC for injury [or WINZ for health related claims]. The contact could have been for yourself, your children, or another child or adult you look after. Are you able to speak with me now? We have Māori interviewers available if you would prefer to speak with one of them. Will you be responding to this survey about your own experiences or on behalf of someone else that you look after?
