people with MD relat<strong>in</strong>g to the participants’ psychosocial wellbe<strong>in</strong>g was the<br />
importance <strong>of</strong> ‘understand<strong>in</strong>g’ the condition. A focus group study <strong>in</strong> Sweden 91<br />
demonstrated that people with MD wanted more <strong>in</strong>formation about many<br />
aspects <strong>of</strong> their condition and its consequences, <strong>in</strong>clud<strong>in</strong>g how they might<br />
prepare for a future with severe visual impairment.<br />
The majority <strong>of</strong> MDSQ respondents (1247 (90%)) had been told that ‘noth<strong>in</strong>g<br />
can be done to help with your MD’. Of those 757 (61%) said they felt<br />
depressed or anxious on hear<strong>in</strong>g this news and 54 (4.3%) said it led them to<br />
feel suicidal. Similar experiences at diagnosis were reported by MD patients <strong>in</strong><br />
Australia 78 .<br />
The Royal College <strong>of</strong> Ophthalmologists’ guidel<strong>in</strong>es for the management <strong>of</strong><br />
MD 92 stress the importance <strong>of</strong> early provision <strong>of</strong> <strong>in</strong>formation and support.<br />
Future research is needed to assess the extent to which the<br />
recommendations have been effective as far as the patients are concerned.<br />
5. Extent <strong>of</strong> impairment<br />
5.1 Bilateral and unilateral <strong>in</strong>volvement<br />
Williams et al 30 compared people with one or both eyes affected by MD <strong>in</strong> the<br />
US and found that those who were legally bl<strong>in</strong>d <strong>in</strong> only one eye reported<br />
greater emotional distress than those with bilateral bl<strong>in</strong>dness. Anecdotal<br />
evidence <strong>of</strong> greater distress among people with some vision <strong>in</strong> one eye<br />
compared with those with no vision was reported <strong>in</strong> much earlier work 93 . In<br />
both articles, fear <strong>of</strong> los<strong>in</strong>g vision <strong>in</strong> the second eye was suggested as the<br />
