Instructions for the trainee• Your answers to the <strong>assessment</strong> tasksneed to show the assessor that youhave a full underst<strong>and</strong>ing of the topic.The assessor may require you todiscuss your responses.• This <strong>assessment</strong> is “open book”, whichmeans that you can use anyinformation you wish when you arecompleting this <strong>assessment</strong>. Thisinformation may include learningmaterials, books, the internet <strong>and</strong> yourorganisation’s policies <strong>and</strong> procedures.• Your answer for Task One is a practicaldemonstration. Other termsthat may be used are c<strong>and</strong>idate, student oremployee.To <strong>report</strong> is to pass on information about aperson's condition to the appropriateauthority, as specified in the organisation’spolicies <strong>and</strong> procedures.Service plan is a general term thatdescribes the individual or group plan forservice delivery. Task One – <strong>Observe</strong>, <strong>report</strong>, <strong>and</strong> <strong>document</strong> changes in condition in a person receiving care orsupport in a health or disability setting (demonstration) Outcomes 1, 2 <strong>and</strong> 3For this task you must demonstrate that you have:1 <strong>Observe</strong>d two changes in the condition of either one person or two different people.These changes can be physical, behavioural, environmental, or emotional.Your observation must relate to:• Conditions <strong>document</strong>ed in the person’s service plan.• Observations requested in the progress <strong>report</strong>s.• Changes in condition recorded in the progress <strong>report</strong>s.• Any changes in condition not previously <strong>report</strong>ed.2 Reported these two changes:• Following your organisation’s policies <strong>and</strong> procedures.• Following legislative requirements <strong>and</strong> the Code of Rights.3 Documented these two changes:• Describing, in your notes, the impact of these changes on the achievement of the person’s independence goals.• Making sure that your change notes are accurate <strong>and</strong> effective. They must be legible, factual, written in permanent ink, be dated,give the time, be signed, describe condition changes, describe the response to care, <strong>and</strong> follow legislative requirements.• Following your organisation’s policies <strong>and</strong> procedures.You will be observed demonstrating this task <strong>and</strong> your observer will complete the observation checklist. You might be observed bythe assessor or by a verifier, such as your manager, supervisor or team leader who can verify that they have observed you carrying outthis type of work in your day-to-day work.
