Todd as Special Deputy Receiver of MIC and Tharp and Associates to providereceivership services to the Receiver. This Order also approved the Engagement Agreementof the law firm of Hennelly & Steadman, PLC to provide legal services to the Receiver.1

Tharp & Associates, <strong>Inc</strong>. has submitted an itemized statement for servicesrendered related to MIC by members of Tharp & Associates and for reimbursable costsincurred in connection with those services pursuant to the orders referenced above for thetime period of March 14, 2012 through April 2012. The statements have been approved bythe Deputy Receiver of the Arizona Department of Insurance and the total amounts billedfor this period by Tharp & Associates, <strong>Inc</strong>. under these statements is One Hundred Twenty-Eight Thousand, Eight Hundred Eleven and 91/<strong>10</strong>0 ($128,811.91) dollars.3. Hennelly & Steadman, PLC has submitted itemized statements for servicesrendered related to MIC by members of Hennelly & Steadman, PLC, and for reimbursablecosts incurred in connection with those services pursuant to the orders referenced above forthe time period of March 14, 2012 through April 2012. The statements for this period havebeen approved by the Deputy Receiver of the Arizona Department of Insurance and by theSpecial Deputy Receiver for MIC. The total amounts billed by Hennelly & Steadman, PLCfor this time frame as reflected on the statements is Eighty-Eight Thousand, Seven HundredTwenty-Four and 11/<strong>10</strong>0 ($88,724.11) dollars.4. Van Cott & Talamante, PLLC has submitted itemized statements for servicesrendered related to MIC by members of Van Cott & Talamante, PLLC, and for reimbursablecosts incurred in connection with those services pursuant to the orders referenced above forthe time period of March 14, 2012 through April 2012. The statements for this period havebeen approved by the Deputy Receiver of the Arizona Department of Insurance and by theSpecial Deputy Receiver for MIC. Order Re <strong>Petition</strong> <strong>No</strong>. 1 entered by the Court on April 13, 2012, provides inpertinent part:...The Receiver may file pay petitions without including as exhibits the itemizedstatements of services rendered to, and costs incurred or expended on behalf of,the Receivership, provided such itemized statements are first submitted to andapproved by the Deputy Receiver or Deputy Director, and either of the Co-Special Deputy Receivers (except for the Special Deputy Receiver statements),and provided further, that the pay petition includes a statement that anyonedesiring additional information concerning the services and costs to be paid underthe pay petition may obtain redacted versions of such information from theSpecial Deputy Receiver by delivering to the Special Deputy Receiver and theReceiver's counsel, Hennelly & Steadman, PLC, a written request specifying theadditional information requested at least three days prior to the date set forhearing on the pay petition. Upon request of the Court, the Receiver shall makeavailable for in camera review by the Court, the itemized statements andsupporting documentation for the services and costs to be paid under the paypetition.The billings for Tharp & Associates, <strong>Inc</strong>., Hennelly & Steadman, PLC and Van Cott &Talamante, PLLC are submitted in camera to the Court contemporaneously with the filingof this <strong>Petition</strong>.6. Any person who seeks additional information related to the billing at issue inthis <strong>Petition</strong> may obtain it by complying with the requirements set forth in the precedingparagraph.WHEREFORE the Receiver requests that the Court enter an Order authorizing theSpecial Deputy Receiver to pay forthwith from Receivership funds as follows:3
