Dear <strong>Dental</strong> <strong>Hygiene</strong> Candidate:An application to practice dental hygiene in the State of Wyoming and a copy of the Wyoming <strong>Dental</strong> Practice Actand the Board’s Rules and Regulations can be downloaded from the Board’s website for use when seekinglicensure in Wyoming.While this letter attempts to summarize for you the information you must provide, you are responsible forreferring to Chapter 3 of the Board’s Rules and Regulations for more specific information and details settingforth Wyoming’s licensure requirements. That is, Chapter 3, Requirements for <strong>Licensure</strong>, will help you todetermine under which category of licensure you qualify. Also, please refer to page 9 of the application regardingthe items that must be included before your application is considered complete.REQUIREMENTS FOR INITIAL LICENSURE AS A NEW GRADUATE:1) You have graduated from a CODA accredited U.S. or Canadian <strong>Dental</strong> <strong>Hygiene</strong> School within the past twelve(12) months of application submission and you have never been licensed to practice as a dental hygienist in thisor any other state;2) You have successfully passed the National Board <strong>Dental</strong> <strong>Hygiene</strong> Examination and you have successfullypassed a regional or clinical board examination from one of the following testing agencies: WREB, CRDTS,NERB, CITA, or SRTA;3) You shall submit to a background investigation if requested and a license shall not be issued until theinvestigation is complete;4) Application/examination fee of $150.00 (non-refundable) in the form of a cashier’s check or money order.REQUIREMENTS FOR LICENSURE BY “CREDENTIALS” AND “ALTERNATIVE” LICENSURE:Wyoming has contracted with <strong>Professional</strong> Background Information Service (PBIS) to perform a completebackground check on all applicants for licensure by credentials and “alternative licensure” candidates. A fee of$400 is charged by PBIS for this service. The applicant, under either of these 2 categories must apply and pay thefee directly to PBIS. PBIS will obtain original source documents for the Wyoming Board. For a PBIS applicationplease contact:<strong>Professional</strong> Background Information Service (PBIS)23460 N. 19 th Ave., Suite 225Phoenix, AZ 85027Ph: (602) 861-5867 Fax: (602) 861-9656Web Site: www.pbisonline.comIt may take up to 90 days for PBIS to complete the verification process, so please apply early. Also, please beadvised that your application is deemed “incomplete” until such time as PBIS has submitted its report directly tothe Board. Please contact PBIS directly on the status of your documentation.In addition to the documentation obtained by PBIS, credential and alternative candidates must also complete theBoard’s Application for licensure and return it along with the application/examination fee of $200.00 (nonrefundable)in the form of a cashier’s check or money order directly to the Wyoming Board.


The following applies to all (initial new graduate, credential and alternative) candidates:APPLICATION: We require originals and notary seals, and therefore, faxed applications are unacceptable. Youmust complete the Board’s Application by meeting all the requirements stated on the application. (Please answerall questions and properly supplement your application with all additional information requested on the applicationwhen submitting your application to the Board office. Failure to provide all requested information will result in adelay in processing your application.)CPR REQUIREMENT: Basic Life Support (CPR) is required for licensure and renewal. The course needs to be ahands-on course sponsored by one of the following organizaitons: American Heart Association, American RedCross or a Board approved course.WYOMING JURISPRUDENCE EXAMINATION: When your application is deemed complete by the Board staffand the Application Review Committee, you will be notified, in writing, that you are eligible to take the WyomingBoard jurisprudence examination, and provided with a list of the board members and their office locations. Youmay schedule a time to take the exam with a member whose office is most convenient to your location. This 25question multiple choice written exam tests your knowledge of the Wyoming <strong>Dental</strong> Practice Act and Board Rulesand Regulations.If we may be of further service to you, please let us know.Debra L.
