Instructions for the trainee• Your responses to the assessment tasksneed to show the assessor that you havea full understanding of the topic. Theassessor may require you to discuss yourresponses.• This assessment is “open book”, whichmeans that you can use any informationyou wish when you are completing thisassessment. This information may includelearning materials, books, the internetand your organisation’s policies andprocedures.• Your answers can be written or spoken.You will be asked to carry out apractical task.−−When you write your answer, usepen in the space provided. Initial anyalterations you make. Please write yourname on any additional pages andattach them to the assessment.−−When you answer verbally, the assessorwill make full notes of your answer inthe space provided in this assessment ormay use a digital voice recorder.− − When you are asked to carry out apractical task, your assessor or verifierwill observe you and complete achecklist. The assessor/verifier will requireyou to perform the task on more thanone occasion, over a two-week period.• For any documents you supply, remove allidentifying details of the people receivingservices or support.• When referring to theories and/orpublished documents, please acknowledgethe published source.
