It isvery im¬portant to maintainthe health of the baby teeth alsoknown as primary teeth. Neglectedcav¬ities can and frequentlylead to problems whichaffect developing adult teeth,also known as per¬manentteeth.Primary teeth or baby teeth areimportant for:(1) proper chewing and eating,(2) providing space for theperma¬nent teeth and guidingthem into the correct position,and(3) permitting normaldevelop¬ment of the jaw bonesand mus¬cles. Primary teethalso affect the development ofspeech and add to an attractiveappearance.While the front 4 teeth last until6-7 years of age, the back teeth(cuspids and molars) aren’t replaceduntil age 10-13. Cavitiesleft untreated will almost alwayscause pain and/or infection priorto the teeth falling out naturally.This can lead to disturbancesin your child’s sleep, activity,eating, drinking, and school attendance.If left untreated longenough, in¬fections can permanentlydamage the adult teethand spread to other parts of thebody.The Center for Disease Controland Prevention reports thatcavi¬ties are perhaps the mostpreva¬lent of infectious diseasesin our nation’s children.More than 40% of children havecavities by the time they reachKindergarten. The bacteria inour mouth breaks down sugarin the food we eat causing thetooth to break down, and overtime could eventu¬ally lead toa cav¬ity. Cavi¬ties can start atanytime once there is a tooth inthe mouth. The first tooth willappear in your child’s moutharound 6 months of age and allthe baby teeth, 20 teeth total,will erupt into the mouth around2 – 2 ½ years of age.This is why having your child’sfirst dental check up at 12months of age is very important.At your child’s first dentalvisit, the dentist should discusswith you proper oral hygiene,diet, proper use of fluoride andany oral habits your child mayhave. The firstfew visits to the doctor’s office,took up in filling the necessaryforms for organ donations. Thethought being you are saving alife and who cares what happensonce you are dead mademe go through the filling up ofthe donor forms.At the time the National BoneMarrow Registry was not heardand so no testing or forms werefilled up. We were quite happyto hear her, so daddy asked whoJaden was to see how alert shewas. When she didn’t respond,daddy asked if Jaden was hersister. Maya spoke right up andsaid, “No, he is my brother. Heis naughty sometimes and youand mommy put him in timeout.” We were so relieved toknow our little Maya was stillalert!We were frightened and hadno idea what was happeningto our little girl, while the doctorsworked furiously to find ananswer. Luckily, everyone onMaya’s medical team contributedto making the diagnosis injust a few hours! Unfortunately,it was discovered that Maya hasHLH (hemophagocytic lymphohistiocytosis),a rare blooddisease involving the destructionof one’s normal blood cellsby the white blood cells. HLHis so rare that there is no currentdata on which to base survivalodds. While chemotherapy isgiven to control the disease, theonly known cure is a bone marrowtransplant. Maya has beenreceiving chemotherapy sinceSeptember 14, , 2009, the dayher diagnosis was made, andnow she is desperately seekinga matched bone marrow donorfor a cure.Maya was in the hospital for 10weeks, 6 of which were spentin the ICU. Even though Mayahas had several setbacks, shehas managed to bounce backeach time. She is our resilientlittle warrior. The hospitalstay continued throughNovember, but Maya madeenough progress that she wasable to come home temporarilyfor Thanksgiving! If so, you are notalone. There are millions ofpeople who go through similarexperiences, from time to time,at school, church, shoppingmall, crowded places, and evenin the sanctity of their ownhome.For some, these symptoms maybe so severe that they may notbe able to carry on their dailyactivities. This is an example ofa panic attack.Then, there are others whohave a general anxiety ornervousness for years, and gethooked on prescription pills,alcohol, drugs, or cigarettes tosuppress their symptoms. Theyalso experience nervousness,palpitation, sweating, and somedizziness, but, not to the extentexperienced by people withpanic attacks. Chronic anxietymay indeed be an external expressionof a mild depression.Panic attacks and anxiety disordersmaybe related to a familyhistory, a past traumatic experience,a bad relationship, stressat work place, financial crisis,and others. It may also representan emotional problem,where an individual dreadsfacing a situation and getsabscessed and overwhelmed byall things that can go wrong.That person may exhaust allthe energy in the negativethoughts, and thus allowing notime for the mind to respondnormally to a simple challenge.As a result, a person may feelvery nervous, weak, dizzy,and experience all other panicsymptoms just thinking of thedreaded outcome, which, inmany cases, may never happen.Most people with panic disorderor anxiety live re-creatingthe unpleasant past, and dreadingthe future. They never havetime to live in the present.Some panic or anxiety symptomsmaybe simply relatedto normal fight responsewhere your body may releaseadrenaline that increases theheart rate, respirations, andcauses flushing. Occasionally,flight response may be moreevident, where the heart ratemay slow down, you may feelweak in your legs, and mayfeel like you are about to passout. These symptoms are amanifestation of slowing of theheart rate and pooling of theblood in the lower extremities.There may be strong emotionalcomponents that can repeatedlytrigger these hormonal responsesto even simple situations.Some people with depressionmay have an imbalancein a brain neurotransmittercalledthe serotonin,which acts as a linkbetween nerve cellsand helps them totransmit data andcommunicate witheach other. Replacingserotoninhas been founduseful in controllingdepression andanxiety symptoms.What can you doabout these panicattacks and generalizedanxiety?The key is tounderstand what isprecipitating your attacks. Youneed to make a note of thosesituations that provoke yourattacks and how your mind andbody responds to those situations.Maintain a notebook andlist all the symptoms that youfeel before, during, and afterand anticipated attack. Usually,the symptoms begin long beforeyou face a stressful situation.From the time, you decidethat you have to face a stressfulsituation that may bring onyour panic or anxiety attack,list all the abnormal things youfeel in terms of heart rate, respirations,sweating, dizziness,fuzziness, spaced out feeling,hot sensation, or anything else.You may feel these symptomsseveral times before the dreadedincidence, but, do not worry.The symptoms may evenintensify, as you get closer tothe situation. But, once youdare and face the situation, youmay notice that your anxietylevel may indeed be a littleless, as your mind engages inthe actual act of dealing withthe situation, which leaves verylittle room for your negativethoughts. When the situationcomes to past and none of thedreaded outcomes happen, youare relieved. You feel like youcould have walked through thesituation, after all, without anysweat. The key to a successfultreatment of panic and anxietyattacks is to feel good, enthusiastic,and optimistic before youface a given situation so thatyou can put your best effort, focusall your energy on dealingwith the situation, and comeout feeling better, satisfied, andaccelerated; and not feelingdrained or exhausted.Panic attacks and anxiety aremulti-factorial, meaning; theymay be related to hormonal,physical, emotional, personal,or chemical imbalances in thebrain. Hence, a multi-prongapproach is essential in dealingwith these situations, in orderto gain substantial benefits. Itmay take a long time to gainconfidence and overcome fearand anxiety.Since, most of the physicalmanifestations of panic attacksare related to adrenalineor vagal excess, they can becontrolled by training our mindto respond to a give stress byreleasing lesser amounts ofthese agents by retraining ourthoughts, and the perception ofour challenges. Exercise alsohelps us to reduce our bodyresponse by releasing lesseramounts of adrenaline for agiven level of stress. We alsoneed to minimize our focuson these hormonal surges andrecall past positive aspects. Astime goes on your body willlearn not to respond to everysimple thought with a burst ofadrenaline. It is like developingimmunity to minimally negativethoughts that never reallyharm anyone in the long run.Meditation is an excellent wayto suppress your body’s responseto hormonal surges andengage your mind positively.As I mentioned, the momentyour mind perceives an ideaof facing a dreaded situation,it will shift the negativethoughts into overdrive. If youforce your mind to focus onmeditation and chant the word,“Om,” or something else thatsoothes your mind, now literallyyou have put your negativethoughts on hold.That will reduce yourbody’s adrenalinesurges. As you focuson the meditation,slow your breathing.Taking deep breathsin and out will helpto reduce your hyperventilation.Whenyou are not hyperventilating,yourdizziness becomesless obvious. Thesemay sound too goodto be true. And, youare probably right! Ittakes a lot of practiceand hence maintaininga diary is importantto chart your course.Knowledge is power. It providesconfidence and insight.Hence, reading a couple ofbooks on these topics mightshed some light on how expertshave approached these problemsin a systematic mannerthat can greatly benefit you.Imagery is a useful way toredirect your mind away fromnegative thoughts. I use thistechnique whenever I am doingstress tests or heart catheterizationson patients. I tell mypatients to imagine that theyare lying on a beach in Hawaii,watching a beautiful sunset,while sipping on their favoritedrink, and enjoying the fireplace.You can create your ownimagery or recall a pleasant ora funny incidence in your life,and slowly, reply the act, onestepat a time, and enjoy everymoment. By all means, takeyour time.You also need to get sufficientsleep. If you cannot sleep, becauseyour mind is in overdrivewith negative thoughts, tryreading a book, drinking warmmilk (no caffeine), or taking ahot shower.People with mixed anxiety anddepression may have a chemicalimbalance in the brain thatmay respond to serotonin releasingdrugs. There are naturalsubstances that can work onpeople with mild depression.One such agent is St. John’sWort. The active ingredient inthe wart (Hypericum)increases serotonin in the brainand helps in controlling thesymptoms of mild depressionand anxiety. As of<strong>Feb</strong>ruary <strong>2010</strong>, The EB-3 categoryis backlogged to 2002(2001 for Indian nationals) andEB-2 is stuck at 2005 for Indiaand China nationals.Here are some tips (and resources)for those waiting fortheir priority dates to becomecurrent:“Free” I-765 and I-131 renewals.If you paid $1010 (or $930for a child under 14 filing separately,or $600 together with theparent) for your adjustment ofstatus filing fee, there is no feefor extensions on your I-765,and I-131. In order to ensureyou are not charged this fee,include a copy of your I-485receipt notice showing thatyou paid the fee, also provingthe case was filed on or after7/30/2007. Where a divorce isgranted after the foreign spouseobtains a green card (now calledan immigrant spouse), it does notchange/ invalidate a grantedgreen card where the immigrantspouse has been married to theUSC for three years or moreand has received an unconditionalgreen card. However, theforeign spouse may have to wait5 years to apply for naturalization.b. He willonly be admitted to that date.• If he does not plan totravel abroad again prior to theexpiration of this Form I-94,he and his employer must filea new H-1 petition and requestfor extension of stay right awayin order to maintain legal status.• If he does plan to travelabroad again prior to the <strong>Feb</strong>ruary,<strong>2010</strong>expiration of thisForm I-94, he can seek reentryduring the limited period oftime that the Consular visa remainsvalid, being sure to presentthe correct Approval Noticeupon his return.• If he does travelabroad again prior to the expirationof this Form I-94, but willnot return until after the Consularvisa has expired, he mustmake arrangements to apply fora new entry visa at an AmericanConsulate abroad before returning.Example 2: The H-1 applicanthas a Consular-issued visavalid until July, <strong>2010</strong>. Becausehe changed jobs recently, hepresents a new Form I-797Approval Notice from his newemployer valid until December,2011. He should be admitteduntil December 2011. Ifhis Form I-94 is mistakenlyissued with an expiration dateof July, <strong>2010</strong>, he must have hisemployer apply for an extensionof stay by filing a new H-1Petition prior to July, <strong>2010</strong>,unless, as above, he will betravelling abroad again beforethe Form I-94 expires. If heremains in the U.S. beyondJuly, <strong>2010</strong> in reliance on theForm I-797, he will be in theU.S. illegally.It matters not whether the mistakeis made by the applicant(by presenting the wrong approvalnotice) or the CustomsOfficer (in failing to grant theproper period of time). Ineither case, the applicant isbound by the date on the FormI-94 issued at the time of lastarrival-and must take correctiveaction in order to maintainlegal status.*Form I-94: this is the Arrival-Departure Record whichis filled out by the travelerenroute to the U.S. and thenpresented to the Customs Officerwith their passport andForm I-797 Approval Notice.Once the decision is madeto admit the applicant to theU.S., the Customs Officer willwrite the Class of Admission(e.g. H-1B or L-1A) and thedate to which admitted on thedocument and staple a portionin the passport. In deciding on theright business entity, you musttake several factors into considerationsince your ultimatedecision can have an impact onmultiple aspects of your business,including the payment oftaxes, your personal liability forthe debts of the business, ownershipsuccession, and the ease ofmanagement of the business. Inmaking this important decision,the following concerns must betaken into account:1. Your intended size andnature of the business.2. The level of controlyou wish to have in your business.3. Whether or not you willbe sole owner of the business.4. Your need for accessto cash out of the business foryourself.5. The level of personal liabilityyou wish to take on asthe owner of the business.6. The tax implicationsof the different business entities.7. Whether or not youwill need or intent to reinvestbusiness earnings into thebusiness.Sole ProprietorshipThe vast majority of smallbusinesses begin as sole proprietorships.These businessesare owned by one individual,usually the person who hasresponsibility of running thebusiness day-to-day. Sole proprietorsown all of the assetsand profits of the business,but also assume completeresponsibility for its liabilitiesand debts. Consequently, interms of the law, the businessowner is one and the samewith the actual business.Therefore, while sole proprietorshipsmay be the easiestand least expensive businessstructure in terms of creation,taxation and dissolution, soleproprietors take on unlimitedliability and are legally responsiblefor all debts againstthe business. Furthermore,sole proprietors often are at adisadvantage in raising fundsfor the business and oftentimes, are limited to using personalfunds or consumer loansto add capital to the business.CorporationOne option that intendedbusiness owners may have tolimit their personal liabilityand increase their ability toraise funds is to incorporatethe business as a corporation.A corporation is considered bylaw to be a unique, separateentity, apart from its owner(s).Consequently, a corporationcan enter into contractualagreements, can be sued orand can be taxed separate fromits owner(s). The owners of acorporation are its shareholders,and may elect a board ofdirectors to oversee the businessdecisions and/or managersto run the business day-to-day.Additionally, corporations canraise needed funds through thesale of their stock. In additionto these advantages, corporationstatus also comes withseveral disadvantages, such asthe need for increased time andmoney creation, increased stateand federal regulations andincreased overall taxation, dueto what is known as “doubletaxation.” Double taxation iswhen the corporation is firsttaxed for its profits and theshareholders are then taxedfor any dividends paid to themfrom the corporation.However, the Subchapter C taxelection enables the corporation’searnings and/or profits tobe treated as distributions andtherefore, pass through directlyto the personal tax returns ofthe shareholders. It is importantto note however, that thereare certain requirements thatmust be met for a corporationto be able to elect Chapter Cstatus and the assistance of aqualified attorney is helpful inascertaining this eligibility.Limited Liability CompanyThe LLC is another option forto-be business owners. TheLLC is a relatively new hybridbusiness structure in Texasand is designed to providethe limited liability featuresof a corporation and the taxefficiencies and operationalflexibility of a partnership orsole proprietorship. Finally,GPs (General Partnerships),LPs (Limited Partnerships)and LLPs (Limited LiabilityPartnerships) are also availableif an intended business ownerwants to avail himself/herselfto the advantages and disadvantagesof the partnershipstructure.In summary, deciding on theform of business structure thatbest suits your business needsshould be given careful consideration.It is recommended thatyou consult a qualified attorneyin order to help you select thebusiness entity that is rightfor you, as the decision willinevitably have long-term andwide-ranging implications.
