Submission:An application <strong>for</strong>m must be completed in order <strong>for</strong> an abstract to be considered. <strong>The</strong>application can be found on the AADSM website at http://www.aadsm.org/research<strong>awards</strong>.aspx.All application <strong>for</strong>ms must be typed. Abstracts must con<strong>for</strong>m to the Abstract Guidelineslisted above and must be in a Micros<strong>of</strong>t Word <strong>for</strong>mat. Both the application <strong>for</strong>m and theabstract must be submitted via email to presentations@aadsm.org.REMEMBER - All authors must disclose significant relationships with commercialcompanies. Send a completed Conflict <strong>of</strong> Interest <strong>for</strong>m <strong>for</strong> each author via fax (630-737-9790) or email (presentations@aadsm.org). See page 15 to review the COI policyand complete the <strong>for</strong>m available at www.aadsm.org/meeting<strong>abstracts</strong>.aspx.A research abstract will be considered <strong>for</strong> an award if the first author fulfills the followingeligibility criteria and makes this indication on the application.<strong>The</strong> deadline <strong>for</strong> all submissions is December 18, 2013.Receipt <strong>of</strong> submissions will be confirmed via email. If you do not receive an emailacknowledgement <strong>of</strong> your submission within three business days, contact Randi Prince at630-737-9761.Graduate Student Research Award Eligibility:1. Be an undergraduate, graduate, master, doctoral, post-doctoral fellow or residentdoing research in dental sleep medicine. All students who submit an abstract andare not members <strong>of</strong> the AADSM will be required to become a member (studentmembership <strong>for</strong> AADSM is free);2. Currently enrolled in an advanced educational program and conduct researchrelated to dental sleep medicine; or3. Having received their certificate or degree within 18 months prior to the abstractapplication deadline.Entries will not be accepted from individuals who have previously received this awardmore than two times. Any research study completed during a dental/medical/otherpostgraduate or residency program shall be eligible. Abstracts must not be closely relatedto another submitted abstract with the same co-authors.Clinical Research Award Eligibility:1. Be a dentist, physician, healthcare pr<strong>of</strong>essional or other pr<strong>of</strong>essional conductingresearch in dental sleep medicine;2. First author on the abstract; and3. Not a graduate student.All awardees will be invited to present an eight-minute summary <strong>of</strong> their papers at theannual meeting.
