this section describes my approach to the patient with diffuse, nonscarring hair loss.<br />

Pertinent questions are asked in logical order to arrive correctly at a diagnosis to initiate<br />

appropriate treatment (Box 1.1). Scarring causes of hair loss are beyond the realm of this<br />

1. Is the hair loss due to external or internal causation?<br />

The initial step in determining the type of hair loss experienced by the patient is to<br />

determine whether the loss is due to internal causes that are medically treatable or<br />

external causes that are cosmetically treatable. This can easily be determined by<br />

examining the hair bulb under the microscope. The hair bulb is the remnants of the<br />

1. Is the hair loss due to external or internal causation? Examine shed hair bulb under<br />

microscope for hair bulb morphology.<br />

2. What is the magnitude of the hair loss? Gently perform 10 hair clump pulls<br />

randomly throughout scalp.<br />

3. Is hair breakage occurring? Have patient collect 4 consecutive days of hair loss and<br />

place each day’s loss in a separate plastic bag, noting if the hair was shampooed on that<br />

day on the outside of the bag.<br />

4. Is there a treatable medical cause? Evaluate past medical history and review of<br />

systems.
