It is important to ensure that additional resources are made available where needs are greatest. This suggests someredistribution <strong>of</strong> resources <strong>and</strong> <strong>in</strong>creased effort is directed towards districts that have accommodated large numbers<strong>of</strong> IDPs from Dili.At a time when the country is fragile, but health services are be<strong>in</strong>g exp<strong>and</strong>ed with new personnel <strong>and</strong> an <strong>in</strong>flow <strong>of</strong>resources, consideration <strong>of</strong> the role <strong>of</strong> health activities <strong>in</strong> nation-build<strong>in</strong>g deserves attention. This is especially sogiven that the health sector successfully avoided be<strong>in</strong>g drawn <strong>in</strong>to the conflict, <strong>and</strong> health pr<strong>of</strong>essionals, almostacross the board, managed to reta<strong>in</strong> their identity as neutral pr<strong>of</strong>essionals. The <strong>Health</strong> <strong>and</strong> Peacebuild<strong>in</strong>g Filter [31]may assist <strong>in</strong> plann<strong>in</strong>g <strong>and</strong> design<strong>in</strong>g future health-related <strong>in</strong>terventions.While the experiences which unfolded <strong>in</strong> <strong>Timor</strong> <strong>Leste</strong> are unlikely to be replicated <strong>in</strong> other sett<strong>in</strong>gs, they raise anumber <strong>of</strong> issues which deserve consideration by other countries. These <strong>in</strong>clude ensur<strong>in</strong>g that whatever disasterplann<strong>in</strong>g has already taken place can be activated at short notice, ensur<strong>in</strong>g a designated leader has been identified<strong>in</strong> the health sector, <strong>and</strong> rapidly <strong>in</strong>vok<strong>in</strong>g an agreed coord<strong>in</strong>ation structure to enable communication with all relevantactors <strong>in</strong>clud<strong>in</strong>g the NGO sector. Disaster plans should consider political violence <strong>and</strong> <strong>in</strong>stability as an importantscenario around which to develop response strategies. The <strong>Timor</strong>ese experience highlights the importance <strong>of</strong> solid<strong>and</strong> supportive work<strong>in</strong>g relationships between the UN, <strong>in</strong>ternational NGO community <strong>and</strong> the government. Goodlevels <strong>of</strong> communication <strong>and</strong> trust assist coord<strong>in</strong>ation <strong>and</strong> more effective programm<strong>in</strong>g. Equity <strong>in</strong> response requiresattention. Listen<strong>in</strong>g to community members, service providers, <strong>and</strong> decision-makers is <strong>in</strong>valuable if gaps are to beidentified <strong>and</strong> better responses <strong>in</strong>stituted.Recommendations:M<strong>in</strong>istry <strong>of</strong> <strong>Health</strong>:1. The M<strong>in</strong>istry <strong>of</strong> <strong>Health</strong> should enhance its ability to respond to future disasters <strong>and</strong> emergencies by:a. Appo<strong>in</strong>t<strong>in</strong>g a very senior named MoH leader to coord<strong>in</strong>ate <strong>and</strong> plan all health-related disaster prevention,mitigation <strong>and</strong> response activities <strong>and</strong> to drive cont<strong>in</strong>gency plann<strong>in</strong>g;b. Ensur<strong>in</strong>g that the directors <strong>of</strong> key health <strong>in</strong>stitutions, such as the National Hospital, SAMES, ambulanceservices, <strong>and</strong> district health teams, are tra<strong>in</strong>ed <strong>in</strong>, <strong>and</strong> take responsibility for, develop<strong>in</strong>g <strong>in</strong>stitutional <strong>and</strong>sectoral emergency plans;c. Develop plans with security structures to make secure public <strong>and</strong> emergency access to key sites, such asMoH HQ, National Hospital, SAMES, <strong>and</strong> Ambulance HQ.d. Prioritise alternatives for IDPs to avoid camps be<strong>in</strong>g based at SAMES <strong>and</strong> National Hospital.e. Strengthen the capacity <strong>of</strong> regional hospitals <strong>and</strong> Level 4 CHCs to deal with emergencies.f. Consider establish<strong>in</strong>g secondary sites for key functions (i.e. tra<strong>in</strong><strong>in</strong>g, drugs distribution) to ensure cont<strong>in</strong>u<strong>in</strong>gsupport for core MoH activities should Dili be <strong>in</strong>accessible.g. Ma<strong>in</strong>ta<strong>in</strong> a national database <strong>of</strong> health facilities, health pr<strong>of</strong>essionals, agencies <strong>and</strong> their capacity to assist <strong>in</strong>situations <strong>of</strong> heightened need.2. Re<strong>in</strong>force current attention to human resources <strong>in</strong> the health sector <strong>and</strong> strengthen their capacity to effectivelyrespond to emergencies, crises <strong>and</strong> <strong>in</strong>stability:a. Establish a roll<strong>in</strong>g program <strong>of</strong> tra<strong>in</strong><strong>in</strong>g around disaster management <strong>and</strong> mitigation <strong>and</strong> m<strong>in</strong>imum st<strong>and</strong>ards<strong>of</strong> care <strong>in</strong> emergency situations.b. Re<strong>in</strong>force commitment <strong>of</strong> pr<strong>of</strong>essionals to provid<strong>in</strong>g impartial <strong>and</strong> equitable services through service contracts<strong>and</strong> tra<strong>in</strong><strong>in</strong>gc. Clarify policies govern<strong>in</strong>g the reallocation <strong>and</strong> deployment <strong>of</strong> health workers who have been displaced todistricts/locations/camps <strong>and</strong> unable to report to work at their usual location. Further, guidel<strong>in</strong>es should bedeveloped to clarifywhen <strong>and</strong> how these health workers mayreturn to take up their employment.d. Initiate a mechanism to determ<strong>in</strong>e how the MoH <strong>and</strong> Cuban Medical Brigade can better work together <strong>and</strong> toaddress concerns relat<strong>in</strong>g to community responsiveness <strong>and</strong> cultural sensitivity, l<strong>in</strong>es <strong>of</strong> responsibility, tra<strong>in</strong><strong>in</strong>g<strong>and</strong> preparation, cl<strong>in</strong>ical policies <strong>and</strong> guidel<strong>in</strong>es, <strong>and</strong> <strong>in</strong>formation systems.52 TIMOR-LESTE HEALTH SECTOR RESILIENCE AND PERFORMANCE IN A TIME OF INSTABILITY

Establish an annual award for health workers <strong>and</strong> facilities who perform admirably <strong>in</strong> times <strong>of</strong> adversity <strong>and</strong>who demonstrate a commitment to the values promoted by the M<strong>in</strong>istry <strong>of</strong> <strong>Health</strong>3. Develop enhanced policies <strong>and</strong> systems for ma<strong>in</strong>ta<strong>in</strong><strong>in</strong>g cont<strong>in</strong>uity <strong>of</strong> care for pregnant women <strong>and</strong> patients withTB <strong>and</strong> other chronic conditions: consider a patient-held record card.4. Recognise <strong>and</strong> devote more attention to mental health needs <strong>and</strong> psychosocial challenges dur<strong>in</strong>g periods <strong>of</strong><strong>in</strong>stability, <strong>and</strong> ensure that ongo<strong>in</strong>g <strong>and</strong> effective care <strong>and</strong> support is available to those with differ<strong>in</strong>g needs.5. Enhance processes <strong>of</strong> consultation <strong>and</strong> engagement with the community <strong>and</strong> civil society organizations aroundhealth issues.6. Explore the role <strong>of</strong> the health sector <strong>in</strong> nation-build<strong>in</strong>g <strong>and</strong> peacebuild<strong>in</strong>g <strong>and</strong> promote tra<strong>in</strong><strong>in</strong>g <strong>and</strong> use <strong>of</strong> relevantdesign, plann<strong>in</strong>g <strong>and</strong> monitor<strong>in</strong>g tools7. In relation to services made available to the community <strong>in</strong> emergencies:a. Periodically assess <strong>and</strong> audit the quality <strong>of</strong> care provided by health care providers <strong>in</strong> keep<strong>in</strong>g with qualityimprovement <strong>in</strong>itiativesb. Ensure access to mosquito nets <strong>and</strong> water <strong>and</strong> sanitation where coverage has been found to be <strong>in</strong>complete.d. Improve the quality <strong>of</strong> communication strategies for health related campaigns; provide additional technicalsupport to health promotione. Develop a system to rapidly identify security impacts on health services such as unsafe access routes todesignated key health facilities.National Government <strong>and</strong> MTRC:8. Use the opportunity <strong>of</strong> reflection on the crisis to:a. Include a civil <strong>in</strong>stability component to national disaster plann<strong>in</strong>gb. Evaluate <strong>and</strong> review SLS <strong>and</strong> camp organisational structures <strong>and</strong> develop guidel<strong>in</strong>es to enhance equity,effectiveness <strong>and</strong> efficiency <strong>in</strong> identify<strong>in</strong>g <strong>and</strong> meet<strong>in</strong>g community needsc. Develop the capacity, <strong>and</strong> adequately resource, District Adm<strong>in</strong>istrators <strong>and</strong> <strong>Health</strong> Managers so that they canrespond better to meet<strong>in</strong>g basic community needsd. Consider the role that health can play <strong>in</strong> simu malu, nation-build<strong>in</strong>g <strong>and</strong> peace-build<strong>in</strong>g activitiesNGOs:9. Evaluate <strong>and</strong> review crisis response capacities <strong>and</strong> build upon any lessons to plan more effectively for future<strong>in</strong>stability.a. Ensure that resource distribution is equitable <strong>and</strong> aimed to meet the needs <strong>of</strong> all those affected by an emergency<strong>and</strong> consequent displacement, across both Dili <strong>and</strong> the districts, whether <strong>in</strong> IDP camps or with<strong>in</strong> the generalcommunity;b. Facilitate tra<strong>in</strong><strong>in</strong>g <strong>of</strong> NGO development staff <strong>in</strong> disaster plann<strong>in</strong>g <strong>and</strong> management <strong>and</strong> review applicability <strong>of</strong>m<strong>in</strong>imum st<strong>and</strong>ards <strong>in</strong> humanitarian responses (SPHERE st<strong>and</strong>ards) to the <strong>Timor</strong>ese context;c. Apply peacebuild<strong>in</strong>g <strong>and</strong> conflict sensitivity pr<strong>in</strong>ciples to ongo<strong>in</strong>g projects <strong>and</strong> programs.UN agencies:10. a. Work with MoH, NGOs, <strong>and</strong> the UN family to ensure that there is agreement on how future immunizationcampaigns will operate. Agree <strong>in</strong> advance on l<strong>in</strong>es <strong>of</strong> responsibility, the roles <strong>of</strong> key players, the content <strong>of</strong>activities so that a response can be operationalised at short notice;b. Provide high-level, timely, <strong>and</strong> appropriate technical expertise to response coord<strong>in</strong>ation structures, work<strong>in</strong>ggroups <strong>and</strong> M<strong>in</strong>istries;c. Identify positive policy <strong>and</strong> programmatic changes which have occurred dur<strong>in</strong>g the emergency <strong>and</strong> consultwith stakeholders regard<strong>in</strong>g how these can be implemented more widely;d.
