from the editor...Here it is…already 2005… and the news about cancer is pretty much what weexpected – some bad, some good, and some hopeful.According to <strong>Cancer</strong> Facts and Figures 2005, a publication of the American <strong>Cancer</strong>Society (ACS), cancer, for the first time, snuck past heart disease as the leadingcause of death among Americans under age 85. (For obvious reasons, heart diseaseremains the number one disease killer for all Americans.) Yes, people are still dyingof these diseases, and yes, that’s bad news.Despite cancer’s new ranking, however, the number of deaths for both cancer andheart disease fell. In fact, death rates for two of the most common, but preventablecancers – lung and colon – showed declines. This is very good news, and expertssay that this downward trend will most likely continue.And then there’s the hopeful news, borne out by additional statistics that cancerprevention strategies are working…or can work with a little cooperation anddiligence! According to the ACS, the dramatic decline in the number of Americanswho smoke was the primary reason for the proportionate decline in death ratesfor both cancer and heart disease. The mortality rates for lung cancer dropped formen and leveled out for women. And for the first time, <strong>Cancer</strong> Facts & Figuresfeatured a section on infection-induced cancers, claiming that 17% of cancersworldwide and 7% of cancers in the United States are caused by infections, manyof which are “highly preventable.” This is a shot of hope whose impact will befelt around the world.This issue of <strong>Roswell</strong>ness offers up RPCI’s own recipe for cancer management, andits main ingredient is a pound of prevention. SO, WHATBOB?ABOUTINNOVATIVE STOP SMOKING CAMPAIGN TARGETS THE EVERYMANBy David Murray“I feel like a nerve. At least whenI’m smoking, it gives me a break.There is no break right now.… It’sjust a constant wanting to hurtsomething or hurt somebody.”So says Ethan Teicher on thefirst day of the rest of his life.Teicher is the sheet-metal workerwho served as the Everyman “Bob”in “Bob Quits,” last fall’s innovativequit-smoking campaign in WesternNew York.Anyone who views theWeb-based campaign recognizesthe modern, reality-TV feel.And anyone who’s ever quitsmoking recognizes the realismof Teicher’s pain.That authenticity is just thequality that drew <strong>Roswell</strong> <strong>Park</strong>’sDirector of Tobacco Control, Dr. K.Michael Cummings, to the “BobQuits” campaign, which he helpedbring to Western New York.“Bob Quits” was originallydesigned by the American LegacyFoundation, a national anti-smokinggroup, for a quit-smoking programin New York City.Dr. Cummings, along withmembers of the Erie-NiagaraTobacco-Free Coalition (of which<strong>Roswell</strong> <strong>Park</strong> is a member), saw thecampaign’s potential to work inWestern New York, too.So the RPCI researcher appliedfor and won a grant from the NewYork State Department of Health –and received matching funds fromthe Foundation – and spent$300,000 converting the “BobQuits” campaign to suit the WesternNew York area and creating a TV,radio, print and “shoe-leathermarketing” blitz to focus theattention of Buffalo and surroundingcitizens on the “Bob Quits” Web sitein time for the Great AmericanSmokeout in <strong>No</strong>vember.The purpose was to get smokersto the Web site, where they couldfollow their fellow nicotine addictnavigating the ups and downs of hislast seven days as a smoker and hisfirst 21 days as a nonsmoker.Through daily video clips anda daily journal, Teicher leads usthrough his agitated, shaky, intense(and occasionally humorous)journey, along the way educatingviewers on the complexity of tobaccoaddiction and the importance ofcreating a comprehensive smokingcessation plan.We watch Teicher work his plan:■ We see him preparing to quitsmoking; the 35-year-old learns fromhis doctor that he’s got the lungs of a121-year-old, and his heavy-smokingworkmate Donny laughingly declareshe’s betting against him to quit.■ We watch him smoke his lastcigarette. “I don’t usually smoke ’emdown this far,” he says. “But I guessI’m allowed.”■ We follow his struggle through thetypical ups (more energy) and downs(more tension) of quitting smoking.He often reflects on how being asmoker not only affected his health,but also his relationships. He was asmoker for the birth of all three ofhis kids, and he remembers cravinga cigarette immediately after thedelivery: “It’s a girl. I’ll be rightback.”■ And at the end, as we see Teicheremerge triumphant but notoverconfident after three smoke-freeweeks, we are touched to see that thedoubting Donny has quit, too, insupport of his friend.Cummings explains why he thinks“Bob Quits” is so effective. Firstof all, Teicher was well-chosen as“Bob”; aside from his sympatheticpersonality, he’s in the 18- to 40-year-old age group that Dr. Cummings2


and the Coalition are targeting.“We have some pretty goodtools to help people quit smoking,”Cummings says; “Bob Quits” andother campaigns are merely designedto let smokers know where theycan get that help.“Bob Quits” did its job; the Website got 250,000 hits, which, in turn,created a four- to five-fold increasein call volume for the New YorkState Smokers’ Quitline, wherecallers could speak to professionalcounselors who offered a free quitsmokingstarter kit.Dr. Cummings doesn’t try to hidehis pride in the program. He thinksthe program helped people quitsmoking in Western New York. Healso hopes it encouraged theAmerican Legacy Foundation towork with similar communitiesacross the country to customize itsclever “Bob Quits” program, and itsnext-generation effort, “MaryQuits.”It did. “We consider ourpartnership with the Erie-NiagaraTobacco Free Coalition and <strong>Roswell</strong><strong>Park</strong> <strong>Cancer</strong> <strong>Institute</strong> on ‘Bob Quits’a success,” says Amber HardyThornton, executive vice presidentfor program development for theFoundation, “and we are nowexploring how we can replicate itin key cities nationwide.”Although “Bob Quits” is windingdown after its <strong>No</strong>vember launch,Cummings has grant money linedup to appropriate “Mary Quits”for a similarly aggressive campaignin Western New York in January,2006.Will Mary be as successful asBob? Are We Doing Enough?Colorectal cancer is the secondleading cause of cancer death (justafter lung cancer) in the UnitedStates, claiming an estimated57,000 American lives last year.Still, the public is largely unawarethat effective screening measuresfor the early detection of colorectalcancer are available andrecommended. Remarkably, fewerthan 50% of those who shouldbe screened for colorectal cancerhave been.Currently, a screening colonoscopy– a procedure that visualizes thefull colon – is recommended at age50, and every 10 years thereafter.However, those who have a familyor personal history of colon canceror a personal history of colonpolyps are at higher risk and mayrequire more frequent screeningsbeginning at an earlier age.Screening can detect precancerousabnormalities, such as polyps,which, when removed, reduces thelikelihood of cancer formation.Screening can detect colon cancer atearlier stages – before it spreads tolymph nodes or distant organs suchas the liver or lungs. Early detectionincreases the likelihood of curefollowing therapy, and offers abetter chance of long-term survival.Fortunately, the last few years havebrought more hope to patients withadvanced colorectal cancer. Newdrug combinations have more thandoubled the average survival ofsuch patients. Approval ofchemotherapies that target bloodsupply to the tumor (bevacizumab,Avastin ® ), as well as agents thatblock tumor growth and reversedrug resistance (Cetuximab,Erbitux ® ), may further improveoverall outcomes. Thus, it is nolonger unusual to extend the survivalof patients with advanced diseaseto several years.Research at <strong>Roswell</strong> <strong>Park</strong> (RPCI) andother cancer centers worldwide willhelp identify even more effectivetreatments. Clinical trials at RPCIwith oral chemotherapy-basedcombinations are being tailored forboth older and younger populationswith advanced colorectal cancer.Other studies have been designed toprovide more convenient and effectivetreatments for patients with locallyadvanced rectal cancer who arereceiving radiation therapy. Theseregimens may provide superioroutcomes with minimum discomfort.Furthermore, the integration ofsurgical procedures, such as theresection of isolated tumor spreadto the liver or lung, has resulted inadditional survival benefits andpossible cures for some patients.However, despite these significantimprovements, advanced colorectalcancer continues to be fatal withinfive years of diagnosis in more then90% of patients. An Update on Colorectal<strong>Cancer</strong> Screening Testsby Philip NyhuisWhen properly completed by individuals at home, the fecal occult bloodtest (FOBT) has long been regarded as a useful non-invasive screening forcolorectal cancer. But recent studies indicate that the test may have someserious shortcomings as a reliable indicator of colorectal health.A study published in The New England Journal of Medicine reports that anew option in noninvasive screening, the fecal DNA test, is four times moresensitive than the FOBT test in detecting colorectal cancers and more thantwice as sensitive in detecting precancerous growths. Although the DNA testhas not yet been approved by the FDA for widespread use in colorectalcancer screening, previous studies have shown it to be effective at detectingabnormalities in people at high risk for the disease. Because it does notrequire putting fecal samples on a card, the DNA test may encourage morepeople to be tested for colorectal cancer. This is consistent with the findingsof another study, which also found that the FOBT test may miss up to 95%of potential cancers.“The fecal occult blood test has never been that reliable in terms of itssensitivity and its ability to diagnose,” says Ashwani Rajput, MD, AttendingSurgeon in <strong>Roswell</strong> <strong>Park</strong>’s Department of Surgery. “The reason is thattumors and polyps don’t always bleed; they bleed intermittently so you maynot catch the time of bleeding. DNAtesting is more effective becauseyou’re actually looking at shed cellsin the colon. A tumorous polypdoesn’t always bleed, but it will shedcells with abnormal DNA that you’llbe able to pick up.”Gastrointestinal specialists, however,are quick to point out that neitherthe FOBT nor the DNA test comeclose to approaching the detectionaccuracy of a colonoscopy.“Each patient should talk to theirdoctor about all available screeningoptions and combinations,” saysRajput. “Some of these options areDNA screening, the fecal occultblood test, sigmoidoscopy,colonoscopy, and barium enema.But…and I can’t stress thisenough… the colonoscopy remainsthe screening gold standard becauseit allows physicians to actuallysee with their own eyes what’sgoing on.” ♦ANOTHER FIRST FOR RPCIThe Commission on <strong>Cancer</strong> (CoC) of the American College of Surgeonshas granted approval with special commendation to the cancer programat <strong>Roswell</strong> <strong>Park</strong> <strong>Cancer</strong> <strong>Institute</strong> (RPCI).“<strong>Roswell</strong> was one of the first organizations to be accredited by theAmerican College of Surgeons in the 1930s. Following that tradition,RPCI is now one of the first cancer centers to attain the designation ofaccreditation with commendation,” said Dana Jenkins, Assistant VicePresident of Organizational Performance Improvement, RPCI.CoC approval is given only to those facilities that have voluntarily committed to provide the best in cancer careand to undergo a rigorous evaluation process and review of its performance. To maintain approval, facilities mustundergo an on-site review every three years.Receiving care at a CoC-approved cancer program ensures that a patient will have access to: quality care close tohome; comprehensive care offering a range of state-of-the art services and equipment; a multispecialty team approachto coordinate the best treatment options available; information about cancer clinical trials, education and support;lifelong follow-up through a cancer registry that collects data on type and stage of cancers and treatment results; andongoing monitoring and improvement of care.5

Her physician blamedthe attacks on the lack of estrogenand had her resume taking thesupplements. When McClanahanasked repeatedly about the safety,she was assured there was only atiny risk of breast cancer.McClanahan literally fell off theexamination table when she receivedher diagnosis. She was the first inher family to develop breast cancer.McClanahan consulted three doctorsregarding cancer treatment andchose the one who suggested alumpectomy followed by severalmonths of chemotherapy andradiation treatments instead of thedouble and single mastectomy twoof them had proposed. She continuesto be checked every six months. Doctors and lawyershave not always stood on the sameside of the professional fence orwithin the same circles of trust.But that may be yesterday’s news.According to today’s headlines, savvydoctors and sympathetic lawyersare putting their heads together tofind creative, streamlined ways topromote and maintain the overallwell-being of the residents in theircommunities.Case in point: <strong>Roswell</strong> <strong>Park</strong> doctorshave teamed up with NeighborhoodLegal Services (NLS) to forge anotherunique medical-legal initiative – apilot project that targets cancerpatients. NLS provides free civil legalservices to low-income and disabledpeople in Western New York throughmore than 30 federal, state, countyand city funding sources.RPCI physician Dr. KerryRodabaugh, Gynecologic Oncology,had an early interest in the rightsof cancer patients. Prior to joiningRPCI’s faculty in 2001, she anda group of social workers at theUniversity of Missouri-Columbiaexamined the issues related tocustody planning for children ofsingle parents with terminal cancer.The researchers found that almostone-half of the children were notinformed in advance of the custodyarrangements, causing them unneededstress and anxiety. Most disturbingwas that one-half of the singleparents died without having a custodyplan in place.Continuing this important andvirtually untapped area of researchat RPCI, Rodabaugh approached thelegal staff at NLS, and together, theysubmitted a grant to the New YorkState Department of Health to helpsupport the end-of-life legal needs ofcancer patients. SOS!PATIENT IN DISTRESSNew NCCNGuidelinesDevelopedto Easethe Stressof <strong>Cancer</strong>PatientsIf you’ve been diagnosed withcancer, there’s little doubt thatyou’re experiencing the non-medicalside effects of the disease, mostnotably distress.The National Comprehensive <strong>Cancer</strong>Network (NCCN), in collaborationwith the American <strong>Cancer</strong> Society(ACS), has recently released itsDistress Treatment Guidelines forPatients, the latest in a series ofresources designed to enhancepatients’ lives and support patientdoctorcommunication.The Distress Treatment Guidelinesfor Patients helps patients and theirfamily members understand andrespond to the psychological andsocial issues they may be facing.Distress is defined here as a widerange of feelings, from powerlessness,sadness and fear to depression,anxiety and panic.“It’s normal to be worried andstressed as a result of a cancerdiagnosis,” said <strong>Roswell</strong> <strong>Park</strong>’sChairman of Psychosocial OncologyMichael A. Zevon, PhD, whoparticipated on the NCCN PracticeGuidelines for Distress Managementpanel. “All patients cope in a uniquemanner. These guidelines help toboth evaluate how patients cope anddetermine if and when the care teamneeds to get involved.”The guidelines also include a self-testcalled a Distress ManagementScreening Measure (DMSM) thatexamines practical, physical, family,emotional and spiritual stressors.Members of RPCI’s PsychosocialDivision evaluated the DMSM ina recent study published in thejournal Psycho-Oncology. The teamfound that the self-assessmenttool efficiently and effectivelyidentifies the distress experiencedby cancer patients.“Our preliminary study indicatedthat the DMSM seems to work verywell,” said Zevon. “Additionalresearch is underway to support thisobservation, but we feel that thismethod does allow clinicians toquickly determine when theirpatients require additional support.”A sample of the DMSM is included in the Distress TreatmentGuidelines for Patients, which is available in booklet form by calling1-877-ASK-RPCI (1-877-275-7724)or online atwww.cancer.org/downloads/CRI/NCCN_Distress.pdf.8

Cayuga Medical Centeropens new $6.5 millionRadiation Medicine facility<strong>Cancer</strong> patients in Tompkins Countyand the Finger Lakes region nowhave access to state-of-the-artradiation therapy services, with theJanuary 17 opening of CayugaMedical Center Radiation Medicine.That’s welcome news for cancerpatients in the region, some of whomhave chosen to travel long distancesin recent years to receive radiationoncology services.The new $6.5 million facility,located adjacent to Cayuga MedicalCenter (CMC), will be operatedin conjunction with <strong>Roswell</strong> <strong>Park</strong><strong>Cancer</strong> <strong>Institute</strong> (RPCI), the onlyNational <strong>Cancer</strong> <strong>Institute</strong>-designatedcomprehensive cancer center inUpstate New York. THE RIDE FOR ROSWELLIT’S NOT ABOUTTHE BIKE!By Mitch FlynnAs volunteers and cyclists prepare for the 10thanniversary “Ride”, event founder Mitch Flynn reflectson the emotional power behind all that pedaling.My apologies to Lance Armstrong for recyclingthe title of his first book, but as the Ridefor <strong>Roswell</strong> approaches its 10th anniversary,I would have to say that it, too, is “not aboutthe bike.”Having said that, the thing that immediatelycomes to my mind about the Ride is a bike –specifically, a three-wheeler ridden by a nineyear-oldpatient named Katy DeGirolamo in1996, the Ride’s first year.What was remarkable about this particular bikewas as much about the rider as how it wasridden. Katy wore a red-white-and-blue sequinedoutfit. She was a star-spangled contrast to herdad, Jim, whose sweat-soaked t-shirt testified tothe exertion of kick-pushing her trike every 50feet for the better part of nine miles.At the post-Ride awards ceremony, Channel 4’sJacquie Walker presented Katy with a Sabresjersey and trophy as the event’s top fundraiser –and Jim with a well-deserved hug. He said, “Itwas probably in my life the proudest momentI’ll ever have.”Later that day, stacking chairs and collectinggarbage (no good deed goes unpunished), myco-chair John Farmelo and I had our own proudmoment: who’d have guessed we’d get 1,100riders and raise $105,000 our first year out?But then, who’d have guessed that nine yearslater, we’d be $2.5 million and 20,000 ridersto the good?Some of our volunteers might have sensed thatpotential. Sally Russell, Barb Keough, LisaWylie, Katie Johnson-Hoffman, and Dr. JohnHoughtaling are among those who have beenthere since the beginning, as has bike shopowner Tom Lonzi, who now has his own<strong>Roswell</strong> story to tell.Tom has supported the Ride’s 62.5-mile routeevery year since 1996, fixing flats and assistingthe mechanically-challenged. Last year, his wifeValerie found a mole on his back that lookedsuspicious, so he went to <strong>Roswell</strong> <strong>Park</strong> to haveit checked. If you believe that “what goesaround, comes around,” here’s one you’lllike: Dr. William Kraybill, who removed thecancerous lesion, had just bought a bike fromTom a few weeks before! (Tom is doing fine andwill be supporting the Ride again this year.)With four routes, eight rest stops, 150 teams,400 volunteers and 2,500 riders, the Ride for<strong>Roswell</strong> literally touches the near and far ofWestern New York. The amazing thing is howmany of our collaborators, riders, volunteers16


and sponsors have a connection to <strong>Roswell</strong>. Our team namesconjure up diagnoses, prognoses, families and friends: Blast TheMass; Pedal Pushers; Rainbow Riders; Team Rudolph; Sean’sRockin’ Reds.A gang of nine-year-olds first came to the Ride in 1997, callingthemselves Michael’s Angels. Eight years later, they did anabbreviated ride so they could be on time for the high schoolgraduation their departed friend and team namesake would alsohave attended.In 2003, we started putting a 5-foot x 5-foot t-shirt at the Ridestart line, inviting riders to inscribe it over the course of the day.It bloomed with messages like a time-lapse movie. Sincethen, researchers worldwide, including those at<strong>Roswell</strong> <strong>Park</strong> <strong>Cancer</strong> <strong>Institute</strong> (RPCI), have beeninvestigating the health effects of radiationexposure on the population of the FSU, oftenunder difficult economic and social circumstances.The challenges faced and lessons learned byresearch groups conducting a large study of acuteleukemia (AL) among children in areas of the FSUwere reported in Environmental Health: A GlobalAccess Science Source. Arthur Michalek, PhD,Dean and Chair, Educational Affairs, andcolleagues, described how a number of significantchallenges – language, physical infrastructure,road conditions, geographic distances andlocating/interviewing participants – were effectivelyovercome through organization, communicationand quality assurance. “The challenges were offsetby the opportunities to glean information ofconfirmed the accuracy of 99% of the cases ofAL diagnosed in the immediate aftermath ofthe Chernobyl accident. “The period was one ofeconomic hardship,” said Dr. McCarthy. “However,the FSU hematologists and hematopathologistswere able to make accurate diagnoses under verydifficult social and economic conditions. Theseresults will be important for a leukemia casecontrolstudy that has been submitted forpublication by the ICRHER.”Martin C. Mahoney, MD, PhD, <strong>Cancer</strong> Preventionand Population Sciences, and colleagues reporteda dramatic increase in the incidence of thyroidcancer among residents of the Republic of Belarusin the International Journal of Epidemiology.“Increases of this magnitude among both men andwomen are remarkable over a relatively limitedtime period,” noted Dr. Mahoney, “and argue forcontinued surveillance in Belarus and otheraffected areas of the FSU.”18

BUILDING BLOCKS FOR A CUREGrants Support <strong>Roswell</strong> <strong>Park</strong> ResearchThe Thrasher Research Fund of Salt Lake City, UT, awarded $147,564 toJohn K. Cowell, PhD, Chairman, <strong>Cancer</strong> Genetics, to study the geneticchanges in pediatric brain tumors. Cowell hopes to develop a moreaccurate way of predicting patient prognoses and responses to therapy.Does Interleukin-6 (IL-6), a naturally-occurring protein, thwart theeffectiveness of traditional androgen-withdrawal therapy in certainprostate cancer patients? Allen Gao, MD, PhD, Medicine andPharmacology & Therapeutics, believes so. This research, which buildsupon an earlier study, is being supported by a $25,000 grant from theCharlotte Geyer Foundation, which also provided $50,000 for the originalIL-6 study.Lionel Coignet, PhD, <strong>Cancer</strong> Genetics, is using an $88,371 award from theElsa U. He was awarded$65,000 from the Flight Attendant Medical Research <strong>Institute</strong> of Miami,FL, to implement a long-term follow-up study.The Community Foundation for Greater Buffalo and the Mae StoneGoode Trust contributed $40,000 and $10,000, respectively, to <strong>Roswell</strong><strong>Park</strong> to purchase a $109,550 Philips Orthovoltage X-Ray Therapy System.The state-of-the-art radiation unit will be a shared core resource used bymore than 20 biomedical researchers. Additional dollars will come from<strong>Institute</strong> funds.Two researchers in the Department of Pharmacology & Therapeutics havereceived funding from the Ralph C. Wilson, Sr. and Ralph C. Wilson, Jr.Medical Research Foundation. Athena Lin, PhD, was awarded $100,000to study alternative genetic tumor growth suppression strategies for breastcancer. Adam R. Karpf, PhD, received a second-year grant of $100,000to explore the mechanisms that impact cell behavior and regulate tumorantigen gene expression.
