James Shafer in <strong>the</strong>ir home gym andconcluded with meeting Einstein <strong>the</strong>rooster and feeding <strong>the</strong> chicks soon tobe big and strong enough for survival in<strong>the</strong>ir own house. But I will have to putEinstein and <strong>the</strong> chicks on hold for <strong>the</strong>moment.It is difficult to talk about Helen or Dr.Shafer separately as <strong>the</strong>y are very much ateam busy with a neurology practice andleadership in <strong>the</strong> medical community,operating Einstein’s Bros Bagels, enjoying<strong>the</strong>ir children and grandchildrenand fundraising activities. Helen wasa tennis player and Dr. Shafer was doingbodybuilding when <strong>the</strong>y met. Theysoon found a common fitness activity<strong>the</strong>y loved doing toge<strong>the</strong>r. In fact, <strong>the</strong>ycelebrated <strong>the</strong>ir 20th anniversary doing<strong>the</strong> Iron Man competition in LakePlacid in 2008; just one <strong>of</strong> many competitions.I had to ask about <strong>the</strong> triathlonand marathon photos and medals on<strong>the</strong> wall in <strong>the</strong>ir gym as <strong>the</strong>y are humbleabout <strong>the</strong>m. Their accomplishments intough competitions are evident and explainwhy <strong>the</strong>ir day starts at 4:30 am.Helen and Dr. Shafer shared with me<strong>the</strong>ir passion <strong>of</strong> fitness, nutrition andsense <strong>of</strong> well-being. While <strong>the</strong>ir commitmentis personal, <strong>the</strong>ir willingness toshare and inspire o<strong>the</strong>rs is not.Healthy eating, being active and realisticgoal setting are fundamental to <strong>the</strong>Shafer’s successes and what <strong>the</strong>y desireto teach o<strong>the</strong>rs. Dr. Shafer emphasizesto eat healthy, frequently, have smallerportions, and to eat a variety <strong>of</strong> foods.As owners <strong>of</strong> Einstein Bros Bagels, <strong>the</strong>yeven make it easy for <strong>the</strong>ir customers toeat healthy. Just check out <strong>the</strong> “SmartChoices” on <strong>the</strong>ir menu. My favoriteis <strong>the</strong> “Asparagus, Mushroom & Swiss”on whole wheat with honey bagel thin.They also recommend setting realisticgoals and to making a commitment toyourself. Starting with realistic regularlyscheduled exercise builds in <strong>the</strong> habit <strong>of</strong>making exercise a normal part <strong>of</strong> one’sday. Do more - just even for five minutes.Dr. Shafer also recommends increasingyour physical activity by just five minutesa day and gradually doing more. Addingsmall increments <strong>of</strong> time to your dailyexercise regimen over a period <strong>of</strong> timewill generate great results. It is apparentthat being healthy and active is a bondfor <strong>the</strong>m and worth emulating.My interview concluded with helpingDr. Shafer feed <strong>the</strong> chicks <strong>the</strong>y arenurturing for fresh eggs and to also giveEinstein, <strong>the</strong> rooster, company. The mostrecent incident that continues to receivea lot <strong>of</strong> attention was New EnglandCompounding Center (NECC),a pharmacy that has been linked tohundreds <strong>of</strong> cases <strong>of</strong> fungal meningitisand over 50 deaths. While this case isvery tragic, it is important to “set <strong>the</strong>record straight” and define what a traditionalcompounding pharmacy doesand how it differentiates from what hashappened in this case.Compounding (Compounding pharmacy)(also pharmaceutical compounding and compoundingpharmacy) is <strong>the</strong> mixture <strong>of</strong> chemicals prepared by apharmacist based on a prescription by a prescriber foran individual patient’s needs.standards for sterile compounding beingmuch more strict. In <strong>the</strong> NECCcase, it also appears to be a failure <strong>of</strong><strong>the</strong> state board <strong>of</strong> pharmacy in Massachusettsto proper investigate complaintsthat it had received. This hascertainly generated many changes at<strong>the</strong> state and federal levels to make regulationsmore uniform and to ensuresafety to <strong>the</strong> patient.Compounding is actually <strong>the</strong> “roots”<strong>of</strong> pharmacy and is <strong>the</strong> way medicationswere prepared prior to pharmaceuticalmanufacturing and mass production.However, today <strong>the</strong>re are still manyCompounding is <strong>the</strong> mixture <strong>of</strong>chemicals prepared by a pharmacistbased on a prescription by a prescriberfor an individual patient’s needs. This“triad” <strong>of</strong> <strong>the</strong> prescriber-pharmacist-patientrelationship is KEY to compounding.If a compounded medication isprepared and it is NOT specific to anindividual patient or prescription, particularlyif it is being prepared in large“batches”, it could be considered manufacturing.Although we are waitingfinal investigation results in <strong>the</strong> NECCcase, it certainly appears that this pharmacywas manufacturing and in violating<strong>of</strong> <strong>the</strong> triad relationship.Pharmacies that compound are regulatedby <strong>the</strong>ir state boards <strong>of</strong> pharmacyand also by federal guidelines USP 795for non-sterile compounding and USP797 for sterile compounding, with <strong>the</strong>instances where compounding makessense and is even life saving! Some examplesinclude: 1) changing medicationsto a dosage form that is easier to take(i.e. tablet to liquid), 2) preparing medicationswithout certain preservatives orcolors for patients who are allergic, 3)creating a strength <strong>of</strong> a medication thatis not commercially available and manyo<strong>the</strong>r examples. Compounded prescriptionsare <strong>of</strong>ten prescribed for hormonereplacement, hospice, pediatrics, andeven veterinary uses.I hope this summary <strong>of</strong> compoundinghelps shed a new light on how beneficialthis integral part <strong>of</strong> <strong>the</strong> pr<strong>of</strong>ession<strong>of</strong> pharmacy is and how it can benefitmany patients in need. GUARDIANSHIPBy Sarah Elmasian, MS, NCG, GCMGuardianship:A way to care for our most venerable residentsGuardianship is based upon <strong>the</strong> ideathat a society needs to take care <strong>of</strong> thosewho are venerable and unable to care for<strong>the</strong>mselves. Guardianship occurs whenan individual is declared legally incapacitatedby <strong>the</strong> court, has no advanceddirectives and needs assistance to keep<strong>the</strong>m safe and from being exploited.Guardianship is a legal procedure inwhich <strong>the</strong> court finds a person’s abilityto make decisions so impaired dueto mental or physical disabilities that<strong>the</strong>ir rights are granted to ano<strong>the</strong>r person.Most <strong>of</strong> <strong>the</strong> time, a family memberor friend will become <strong>the</strong>ir guardian.However, when <strong>the</strong>re is no family memberor friend to become <strong>the</strong> guardian, orfamilies disagree about <strong>the</strong>ir care or <strong>the</strong>management <strong>of</strong> <strong>the</strong>ir property, a pr<strong>of</strong>essionalguardian is appointed.The pr<strong>of</strong>essional guardian is a surrogatedecision maker in personal, healthcare, and financial matters for <strong>the</strong> incapacitatedWard <strong>of</strong> <strong>the</strong> court. The incapacitatedperson still retains <strong>the</strong> rightto dignity, privacy, and quality care, buto<strong>the</strong>r rights are given to <strong>the</strong> guardianto perform with court oversight andapproval. In many cases, <strong>the</strong> guardianmust petition and seek a court order toact on behalf <strong>of</strong> <strong>the</strong> ward. The Guardianis charged with making sure that<strong>the</strong> Ward is safe and to act in <strong>the</strong>ir bestinterest while respecting <strong>the</strong>ir wishes asmuch as possible. They can apply forbenefits on behalf <strong>of</strong> <strong>the</strong> ward and monitorhealth care and placement based ona court approved plan <strong>of</strong> care.If you know <strong>of</strong> a person you believeis unsafe or may be exploited, you cancall a registered guardian to assess <strong>the</strong>situation. If <strong>the</strong> person is unsafe, <strong>the</strong>guardian will contact an attorney to filea petition for Emergency TemporaryGuardianship with <strong>the</strong> court to immediatelycare for <strong>the</strong> person. Geraldine Searlcelebrated her 105th birthday with a special Irish blessing and toast <strong>of</strong> champagnefrom Fa<strong>the</strong>r John Morrissey <strong>of</strong> St. Sebastian Catholic Church, along with her family,friends and staff.Born in Galesburg, Illinois in 1910, Geraldine and her twin sister Josephine wona local department store-sponsored “Most Beautiful Twins” contest. More than 200babies were entered into a variety <strong>of</strong> categories and now 105 years later Geraldinetells all that she won a beauty contest over 100 years ago.Jennifer Searl Palleschi, Geraldine’s granddaughter is also <strong>the</strong> Administrator <strong>of</strong>Grace Rehabilitation Center <strong>of</strong> Vero Beach. Jennifer tells me that everybody callsGeraldine “Grandma” while associates say that she keeps everyone in good spirits, isoutgoing and is very active. PULSE POINTSTonja Tory, R.N., Takes On New Roleas Director <strong>of</strong> Applications and Informaticsin Management Information SystemsSince coming to Indian River MedicalCenter almost 20 years ago as a staffnurse, Tonja Tory, R.N., described her timehere so far as a ‘journey.’ And it has encompassedmany roles—charge nurse, casemanager, nurse manager, clinical nurseliaison, and most recently, manager <strong>of</strong>Clinical Informatics. She has now takenon <strong>the</strong> role <strong>of</strong> director <strong>of</strong> Applicationsand Informatics, with her journey nowincluding overseeing <strong>the</strong> development <strong>of</strong>Paragon and <strong>the</strong> much-anticipated CPOElaunch as well as being responsible forplanning and coordinating <strong>the</strong> processesrequired for <strong>the</strong> provision <strong>of</strong> user applicationsand systems necessary for clinicaland business operations.CPOE stands for Computerized ProviderOrder Entry. By May 2013, orders will beentered electronically by physicians using asystem that uses clinical guidelines, ordersets with links to peer-reviewed evidence,safety/quality alerts and special calculators,such as <strong>the</strong> insulin sliding scale, to automate<strong>the</strong> ordering process.Paragon culminates ultimately on Oct.1, 2013, when IRMC demonstrates thatit successfully qualifies for ‘MeaningfulUse’ (MU). The hospital must qualify forMU in order to receive partial reimbursementfrom <strong>the</strong> government.
