Section VHealth & Emergency PoliciesEmergency Information FormAn Emergency Information Form must be completed prior to issuing the student schedules. The school needs current informationon a student’s health conditions and parent/guardian contact information in case of an emergency. To effectively protect thewelfare of students, our school nurse keeps the requested information on file.Emergency & medical treatmentIn case of an injury or illness, the school will make every effort to notify parents or other emergency contacts as designated by theparent/guardian. Depending upon the severity, the paramedics may be called to assist in assessment and on‐site treatment.Parents may meet their student at the school or the hospital if such treatment is deemed advisable. School personnel trained inCPR and first aid, the school nurse, and administrators will make this decision, if necessary. If your student has a chronic or acutehealth condition that might affect them at school, please contact the school nurse so we can work with you.ImmunizationOur goal is that every student in the Nampa School District will be properly immunized for the health of the student, school, andthe community. In accordance with Idaho Code 39‐4801, parents are required to submit a copy of their child’s immunization statusto the school office before enrollment is complete. Exemptions for religious, personal, or medical reasons can be made; forms areavailable from the school nurse. If an outbreak of a contagious disease occurs, students not immunized will be excluded fromschool attendance.InsuranceEven with the greatest precautions and the closest supervision, accidents can and do happen at school. The school district does notprovide medical insurance to automatically pay for medical expenses when students are injured at school. This is the responsibilityof the parent/guardian. Inexpensive student medical insurance is available for individual purchase. Brochures outlining thecoverage and premiums are available at the school office.MedicationAny student taking medication needs to have a written release on file with the school administration office indicating the following:• Name of medication.• Doctor’s name.• Reason for taking and length of time.• Parent’s signature.All medication must be supervised and dispensed by the school nurse or trained designee. Parents/students, please contact theschool nurse to make arrangements. Asthma multi‐dose inhalers may be carried by the student, with the written permission ofstudent’s physician, parent, and with the knowledge of the school nurse.Section VIAthletics & ActivitiesActivity Transportation & Travel RequirementsThe following rules apply to extra-curricular trips:1. The NHS Administration must approve all school-related trips;2. Any out-of-state or overnight trips must also be approved by the Nampa School District Board of Trustees;3. Students must arrange for transportation to events within Nampa with the exceptions of those participating in track andfootball. Where school transportation is not provided, school officials and/or coaches will not transport students nor helparrange for rides.4. Transportation to events and competitions outside Nampa is by bus or other Administration approvedtransportation. Each bus must have at least one teacher or approved parent/adult;5. Students must ride on school transportation both ways. Exceptions will be made on a case-to-case basis when aparent or guardian makes a written request to transport the child home.Athletic Physicals & InsuranceAll freshman and junior students wishing to participate in athletics must have a physical examination. This is an Idaho High SchoolActivities Association regulation. Sophomores and seniors must obtain a sports physical if they did not receive one as a freshman orjunior.- 20 -
Student insurance forms are available in the NHS Administration Office or from the athletic director’s office. The insurance contract isbetween the insurance company and the insured student. Some type of insurance is mandatory for athletes.Expectations for Extra-Curricular ParticipationParticipants will be expected to follow the standards listed below:1. Be courteous to opponents, fans, and cheerleaders.2. Be positive and cooperative.3. Respect and abide by the official's decision.4. Exercise self-control at all times; never boo an official, coach, cheerleader, or player. Violation of the Conduct ofStudents may result in further disciplinary action from the coach/advisor/administrator.5. Learn to win with character and lose with dignity.6. Follow the appropriate dress standards established by the school, school district, coaches, and advisors.7. If suspended out-of-school, the student may not participate in any activity during the time of suspension.8. Follow the School Board Attendance Policy. On days of an extra-curricular event, the participating student must bepresent in two or more classes to be eligible to participate that day. The administrator or designee must clear anytravel and/or extenuating circumstances requiring an exception in advance. On the school day following any activity,event, or contest, participants (coaches, advisors, and students) are expected to be in attendance at school.9. Display appreciation for a good performance or play regardless of the team.Extra-Curricular/Co-Curricular ProgramThe extra-curricular program at Nampa High School consists of a host of challenging activities designed to appeal to diverse interestsand talents. From the artist to the athlete, the student may seek to participate in activities where he/she can further develophim/herself academically, socially, emotionally, and physically. NHS and the other district high schools offer a wide range of cocurricularactivities. To make the most of your high school experience, get involved in one or more of the following:Football Baseball Band Student Government Family Career Community Leaders of AmericaCross Country Softball Jazz Band Honor Society Business Professionals of America (BPA)Volleyball Tennis Orchestra Yearbook (Sage) Bilingual Educ. The club sponsor must be present at all club activities and clubmeetings. Concession privileges for operation on campus must be obtained from the athletic director.Fund-Raising ActivitiesMost of the activities, clubs, organizations, and classes conduct fund-raising activities during the school year. These must be approvedin advance by the administration, and all funds collected are dispersed to the Associated Student Body Fund according to establishedfinancial guidelines. Students collecting money for school organizations must turn in the money to the faculty sponsor on a daily basis.Interscholastic Activity EligibilityAll students must meet the following NHS minimum standards as well as the IHSAA standards in order to be eligible to participate inafter school activities:1. Students must be enrolled full-time (minimum of 4 classes for seniors), and must have passed 3 of 4, 4 of 5, 5 of 6, 5 of 7, 6 of 8classes. This policy applies to all students at NHS in grades 9-12. All activities governed by the Idaho High School ActivitiesAssociation are covered by this policy. Students falling below the minimum standard will become academically ineligible to try outfor their chosen activity. (See the Eligibility Grading Policy below.)2. All participants and their parent/guardian must have completed, signed and returned, the Nampa HighSchool Participation Form.A. Notwithstanding any other district policy violation, any civil or criminal law infraction or conduct by a student participantthat is determined by the team head coach and building athletic/activities director to be detrimental to the activity program,school or school district will result in advising by the team head coach and the building athletic/activities director withpossible suspension or expulsion from activities.B. For athletic activities, no participant may quit one sport and turn out for another after the season has begun without mutualconsent of both team head coaches and the building athletic/activities director.- 21 -
