Strategic Directions:Achievements and Challenges - Accountability (continued)Accountability to Those in our CareA critical incident is an unintended event that causes seriousharm to a patient, resident or client in the health care system.<strong>Report</strong>ing systems were put in place in <strong>2007</strong>/<strong>2008</strong> to facilitatereporting of, and learning from, critical incidents.A 24-hour Critical Incident <strong>Report</strong>ing Line (788-8222) wasestablished by the <strong>Winnipeg</strong> <strong>Regional</strong> <strong>Health</strong> Authority in <strong>2007</strong> toenable patients, families and health care providers to participatein patient safety improvements. The telephone reporting linehelps the region gather information regarding critical incidentsand initiate steps and recommendations to prevent them fromhappening. The line was enhanced in <strong>2007</strong>/<strong>2008</strong> to include astaff/patient support function, and is now being operated out ofKlinic Community <strong>Health</strong> Centre. Critical incidents continue tobe carefully monitored and reported to Senior Management andthe Committee of the Board.The <strong>Winnipeg</strong> <strong>Regional</strong> <strong>Health</strong> Authority offers a number ofcritical incident training sessions for managers. This includes afull day workshop that assists participants on how to investigateor review a critical incident and formulate recommendations toreduce the risk of recurrence.The <strong>Regional</strong> Integrated Patient Safety Strategy continued itswork in <strong>2007</strong>/<strong>2008</strong> focusing on four areas:1. Promoting culture change2. Direct involvement of patients3. Learning from clinical practice4. Promoting change in care deliverymanagements aspects of the disclosure process, along withthe opportunity to hone the communication skills required withpatients and families.Access to InformationThe <strong>Winnipeg</strong> <strong>Regional</strong> <strong>Health</strong> Authority continues to meet itsresponsibility to provide information to members of the publicthrough accessible sources. This includes maintaining an openand transparent flow of information between the <strong>Winnipeg</strong><strong>Regional</strong> <strong>Health</strong> Authority and the public while considering allaspects of privacy and confidentiality of clients. The <strong>Winnipeg</strong><strong>Regional</strong> <strong>Health</strong> Authority Chief Privacy Officer responds torequests made via the Freedom of Information and Protection ofPrivacy Act (FIPPA). In <strong>2007</strong>/<strong>2008</strong>, of 916 requests, 552 werefrom media and 364 from political parties. In the future, theChief Privacy Officer’s office will be working to set up a processfor tracking and reporting various activities within the region tothe public in accordance with FIPPA.Requests for information can be submitted via our website atwww.wrha.mb.ca/contact/infoaccess_fippa.php. Critical Incident Learning Summaries - reportsof outcomes regarding review and follow up actions of criticalincidents - are distributed internally to regional health staff tofacilitate broader learning from critical incidents and preventrecurrence.Disclosure WorkshopsDisclosure workshops are now being offered to enhance healthcare workers’ ability to re-establish trust and rapport in theface of adverse outcomes. The workshops provide participantswith a better understanding of organizational, ethical and riskwww.wrha.mb.ca
