CURRICULUM VITAELOIS W. CHOI-KAIN M.D. P.3Lyons-Ruth, I am coordinating a study of attachment behaviors in subjects withand without BPD and their mothers. This study uses rigorously diagnosedsamples from Dr. Gunderson’s ongoing family study that include BPD andnormal control subjects and their mothers. Mother-daughter dyads participate inan experimental protocol that involves a video-taped 10 minute conflictdiscussion between mothers and daughters. We are investigating the relationshipbetween markers of attachment behaviors in the video-taped conflict discussionwith BPD. Also, we are studying the functioning of the hypothalamic-pituitaryadrenalaxis in response to this interpersonal stressor by obtaining baselinesalivary cortisol samples at the start of the lab visit, a reactive sample 20 minutesafter the start of the conflict discussion, and a recovery sample 40 minutes afterthe start of the conflict discussion. Our hypotheses are that 1) BPD will beassociated with markers of disorganized attachment and 2) disorganizedattachment will be associated with dysregulated stress responses in the HPA axischaracterized by an absence of expected decreases in salivary cortisol in therecovery samples.2007-2009 Fund for Psychoanalytic ResearchThis $40,000 grant funds a pilot study aimed to assess the relationship betweenattachment status, neurocognition, and social cognition in Borderline PersonalityDisorder(BPD). BPD has been distinguished by three realms of disturbance:affect regulation, behavioral control, and disturbed relationships. The symptomsrelated to disturbed relationships in BPD provide the greatest diagnostic value, demonstrate significant familiality and heritability, and may represent a majorunderlying phenotype of the disorder. In the realm of disturbed relationships,BPD has been both theoretically and empirically associated with preoccupied andfearful insecure attachment types. While previously published research hasestablished increased rates of attachment insecurity in BPD samples, themechanism by which attachment insecurity develops into the syndrome of BPDis not well understood. One theoretical model of BPD proposes that earlyattachment insecurity interferes with the development of critical social cognitiveskills such as mentalization, defined as the capacity to understanding one’s ownand other people’s mental states. The neuroscientific literature on BPD suggeststhat difference in brain structure and function underlie key clinical features ofBPD, which raises the question of whether deficits in neuropsychologicalfunction are determinants of social cognitive skills, and are thereby implicated inthe interpersonal disturbances in BPD. This application proposes a pilot studyaimed to assess the relationship between attachment status and 1) BPD and itsphenotypes, 2) neuropsychological skills, and 3) social cognitive skills. Ourmethods will involve administration of the Adult Attachment Interview (AAI), abrief but comprehensive neuropsychological battery, and assessments of socialcognitive skills relevant to mentalization to 60 rigorously diagnosed and wellcharacterizedfemale subject (30 subjects with BPD and 30 without BPD)ascertained from John Gunderson and Mary Zanarini’s ongoing family study ofpersonality traits (NIMH R01 MH070377-01A2). These pilot data will besummarized and prepared for both publication and application to federal andprivate funding sources for larger scale research.3
