Open Studio <strong>Liability</strong> <strong>Waiver</strong>Metalsmithing is not a dangerous occupation, but it does have both risks and dangers associated with it. Thesedangers and risks include but are not limited to:• sparks and/or dust in eyes• minor/major burns from hot steel or welding sparks• cuts, scratches, abrasions, etc.• staining and/or destruction of clothing from sparks, heat, acids, paints, etc.• muscle injuries• traumatic injury due to inherent dangers of working metal shopI the undersigned hereby acknowledge that I understand the dangers and risks inherent to metalsmithing andwaive all right to lawsuits or other legal or punitive action against <strong>FDJ</strong> <strong>On</strong> <strong>Time</strong>, LLC, its owners, employees orother associated persons. By signing I take full responsibility for my own actions and any accidental injury that Imay experience in the course of my relationship with <strong>FDJ</strong> <strong>On</strong> <strong>Time</strong>, LLC.I understand that I will not use or operate any tools or equipment that I am not fully competent and capable touse. This includes, but is not limited to, torches, flexible shaft machines, tumblers and hand tools. Experience inusing studio tools in a must. Code of ConductSafety is MANDATORY…and other things…1. Must wear safety glasses at all times.2. Wear comfortable, closed-toed shoes.3. No long sleeves or be able to roll them up.4. No jewelry, scarves or anything else that a tool can grab and pull.5. Long hair must be tied back.6. <strong>Waiver</strong> must be signed and presented before using studio time.7. Wear comfortable clothes that can get dirty.8. No children.9. No pets.10. No smoking.11. No food or drink in work areas.12. No cursing.13. No rowdy behavior.14. No harassing or intimidating.15. Payment must be made in full before using any studio time.16. <strong>On</strong>ly use studio tools and equipment that you are competent andexperienced in using. No instruction in the use of tools will beoffered.17. No consumable items will be provided for use. This includes but is notlimited to sawblades, solder, polishing compounds, abrasives, drillsand burs.18. Keep you work area clean for others. Any messes must be takencare of before leaving studio time.19. If any damage to studio tools and equipment is incurred, you, theuser are responsible for repair or replacement charges.
