FINAL 28/08/2011choose <strong>in</strong> the best <strong>in</strong>terest of their child, and one might argue that it is the responsibility of theparents to have their child participate <strong>in</strong> NBS <strong>in</strong> its own <strong>in</strong>terest.The need for an overall framework for the assessment of neonatal<strong>screen<strong>in</strong>g</strong>Rapid advances <strong>in</strong> genetics and genomics are outpac<strong>in</strong>g the ability to adequately <strong>in</strong>tegratenew discoveries <strong>in</strong>to health services and have led to a grow<strong>in</strong>g implementation gap betweenwhat is technologically possible, what exists <strong>in</strong> practice and what is acceptable and desirableor clearly justifiable. There is <strong>in</strong>creas<strong>in</strong>g pressure to <strong>in</strong>troduce or expand genetic <strong>screen<strong>in</strong>g</strong>programs, although evidence of the cl<strong>in</strong>ical validity and utility of <strong>screen<strong>in</strong>g</strong> tests is oftenlagg<strong>in</strong>g beh<strong>in</strong>d. 6 Many exist<strong>in</strong>g programs, particularly opportunistic <strong>screen<strong>in</strong>g</strong> programs, havebetter designed and funded laboratory levels as compared to the cl<strong>in</strong>ical and programmanagement strata. The reason for this is that proponents of new and expanded <strong>screen<strong>in</strong>g</strong>programs often focus on the availability of a valid and <strong>in</strong>expensive test, without consider<strong>in</strong>gthe additional costs and implications of the <strong>screen<strong>in</strong>g</strong>. As a result, programs which lacksufficient resources to develop important aspects such as education, counsell<strong>in</strong>g, treatment,follow-up and oversight may be unable to evaluate their <strong>in</strong>tended objectives. As well, unlessthere is fund<strong>in</strong>g available from the outset to ensure appropriate evaluation, many programsmay cont<strong>in</strong>ue unaltered, miss<strong>in</strong>g important opportunities for cont<strong>in</strong>uous qualityimprovement. 7Several countries have expanded their NBS programs recently, as <strong>document</strong>ed <strong>in</strong> the CurrentPractices (CP) of NBS <strong>document</strong> that is also a result of this EU project. Some programsscreen for only one or two conditions, other for several up to a few dozens (Chapter 8 CP).The Health Technology Assessment (HTA) of <strong>screen<strong>in</strong>g</strong> possibilities has often beenperformed <strong>in</strong> national contexts, for <strong>in</strong>stance <strong>in</strong> Denmark, F<strong>in</strong>land, the Netherlands, Spa<strong>in</strong> andthe UK. 8 HTA reports summarize evidence, ethics and economics and come to differentconclusions, partly due to <strong>in</strong>sufficient evidence and due to differences <strong>in</strong> the organisation of6 Andermann A, Blancquaert I, Beauchamp S, Costea I.
