As the def<strong>in</strong>ition of benefit evolves, this raises questions of what issocially acceptable. The potentially broaden<strong>in</strong>g goals of NBS (from identify<strong>in</strong>g conditionswhere irreparable health damage can be avoided by early diagnosis to <strong>in</strong>clude reduction ofdiagnostic odyssey) should be subject to public debate.Neonatal <strong>screen<strong>in</strong>g</strong> programs and related <strong>in</strong>terventions should be def<strong>in</strong>ed <strong>in</strong> a way that theyare consistent with the overall health care strategies, capacities and the culture of a country.Screen<strong>in</strong>g goals and benefits should be assessed versus goals and benefitsestablished/accepted for other cl<strong>in</strong>ical and public health <strong>in</strong>terventions and strategies adopted<strong>in</strong> the country. The opportunity offered by neonatal <strong>screen<strong>in</strong>g</strong> should also be assessed tak<strong>in</strong>g<strong>in</strong>to consideration the possibility of <strong>screen<strong>in</strong>g</strong> at a later life stage, for those goals(reproductive choices?) and disorders where preventive <strong>in</strong>tervention is still timely, so thatparticipation <strong>in</strong> <strong>screen<strong>in</strong>g</strong> can be decided directly by the subject. Presymptomatic test<strong>in</strong>g <strong>in</strong>m<strong>in</strong>ors shall generally be limited to conditions where there is direct benefit, because earlytreatment can prevent irreparable damage.Relevant charters, declarations and pr<strong>in</strong>ciples recognised by EUmember statesBesides the Wilson and Jungner criteria 25 for neonatal <strong>screen<strong>in</strong>g</strong> (Chapter 4), which have beenfollowed with vary<strong>in</strong>g flexibility <strong>in</strong> many EU countries, other pr<strong>in</strong>ciples have been developedfor different purposes, which, however, have bear<strong>in</strong>g on the context of neonatal <strong>screen<strong>in</strong>g</strong> andon this <strong>document</strong>.Article 168 of the Treaty on <strong>Europe</strong>an Union 26 <strong>in</strong>dicates that a high level of health protectionis to be sought <strong>in</strong> the Union policies, with the <strong>Europe</strong>an Union action be<strong>in</strong>g subsidiary tonational policies.In 2006 the EU EPSCO Council also signed a declaration 27 recogniz<strong>in</strong>g that universality,access to good quality care, equity and solidarity are common values of the EU healthsystems, although differences <strong>in</strong> approaches and the need for f<strong>in</strong>ancial susta<strong>in</strong>ability limit theuniform operation of the health care <strong>in</strong> EU.
