26 _ Preparing to Go HomePreparing To Go HomeDischarge InformationStart thinking about plans for your discharge early in your <strong>Hospital</strong> stay. A social worker can help youand your family arrange an appropriate discharge plan. Each patient has different needs, and everyplan is carefully made with those needs in mind.When your doctor tells you that you are ready to go home, your nurses will discuss ongoing care withyou. If any prescriptions are required, they will be given to you the night before you go home. Youwill also receive a written discharge plan that describes the arrangements for any future care thatyour doctor may order after discharge. You may not be discharged until the services required in yourwritten discharge plan are secured or determined by the <strong>Hospital</strong> to be reasonably available. You alsohave the right to appeal your discharge plan. An envelope for your discharge information is providedin the front pocket of this Guide so you will be able to easily keep track of the material.As you make arrangements to leave the <strong>Hospital</strong>, please note that discharge time is before 10 am.
