Xerox copy of Birth Certificate / S.S.L.C. Certificateii. Xerox copy of Community Certificateiii. Xerox copies of HSC/UG/PG/M.Phil. Mark Statements and Convocation Certificatesiv. Xerox copies of approval of qualification / appointment of teachers by the University of Madras.v. Xerox copies of Certificates of Research Experiencesvi. Part-Time candidates should enclose the following certificate along with application.a. A service certificate from the Head of the Institution where the candidate is employedindicating the date of appointment ( i.e., whether permanent or temporary or leave vacancyetc.) should be furnished If there is any break in service ( if it is not a continuous service),the details should be furnished.b. A certificate from the Director of Technical Education / District Educational Officer statingthat the polytechnics / Higher Secondary / High School where the teacher is employed hasbeen recognized by the government.INSTRUCTIONSI. Serial Nos 1-18 should be filled by the applicant. Serial Nos.19-24 should be filled by the Supervisor.Serial No.25 should be filled by the Head of the Research department/ Principal/ Director. Serial No.26should be filled by the employer.II. The filled in applications should be sent to the concerned Heads of the Departments of the University /Principals of the Colleges / Directors of recognized Research Institutions on or before 2 nd January, 1 stApril, 1 st July and 1 st October of every year.ONLY LIMITED HOSTEL FACILITIES ARE AVAILABLE IN THE UNIVERSITY HOSTELS. HENCE, THEREIS NO GUARATEE FOR HOSTEL FOR ACCOMMODATION TO FRESH CANDIDATES.
