RECORDS, DOCUMENTATION AND STATISTICSHealth <strong>and</strong> <strong>safety</strong> activities are documented <strong>and</strong> kept on file for due diligence purposes, <strong>and</strong> to assistwith loss prevention.Unit RequirementsThe types of records that Units maintain are:• Worker orientation records• Records of worker <strong>and</strong> supervisor training with the date, attendance, <strong>and</strong> general content of the<strong>program</strong>• Records of meetings <strong>and</strong> crew talks where <strong>health</strong> <strong>and</strong> <strong>safety</strong> issues were discussed• Inspection reports <strong>and</strong> records of actions taken to solve problems• Accident investigations reports• Supervisors’ notes <strong>and</strong> logs of <strong>health</strong> <strong>and</strong> <strong>safety</strong> contacts with workers• Records showing the use of progressive discipline to enforce <strong>health</strong> <strong>and</strong> <strong>safety</strong> rules• Subcontractor pre-qualification documents• Local Health <strong>and</strong> Safety Committee minutes, showing what steps have been taken to address<strong>health</strong> <strong>and</strong> <strong>safety</strong> issues• Equipment log books <strong>and</strong> maintenance records• Health <strong>and</strong> <strong>safety</strong> forms <strong>and</strong> checklists• Medical certificates, hearing tests, <strong>and</strong> first aid records• Sampling <strong>and</strong> monitoring records of exposures to harmful substances• Statistics on the frequency <strong>and</strong> severity of accidents.Injury/illness records are used in developing statistics to identify:• Types of injuries• Total claim loss for each injury• Total time loss for each injuryTrend analysis is produced from all data accumulated under the Health <strong>and</strong> Safety Program to identifypatterns, which may lead to the identification of systematic problems not perceived when looking atisolated incidents.Records <strong>and</strong> statistics are reviewed regularly at senior management <strong>and</strong> Health <strong>and</strong> Safety Committeemeetings. This process is crucial to identifying trends <strong>and</strong> hazards not previously identified, <strong>and</strong> outdatedor ineffective control measures.University Reports• All accidents <strong>and</strong> WorkSafeBC claims must be reported to Health, Safety <strong>and</strong> Environment on ast<strong>and</strong>ard accident report within 24 hours.30
