ALS PRACTITIONER <strong>PROTOCOLS</strong>Information on professional stationery4. (1) A practitioner shall only print or have printed on letterheads, account formsand electronic stationery information pertaining to such practitioner’s –(a) name;(b) profession;(c) registered category;(d) speciality or subspeciality or field of professional practice (if any);(e) registered qualifications or other academic qualifications or honorarydegrees in abbreviated form;(f) registration number;(g) addresses (including email address);(h) telephone and fax numbers;(i) practice or consultation hours;(j) practice code number; and(k) dispensing license number (if any).(2) A group of practitioners practising as a juristic person which is exemptedfrom registration in terms of section 54A of the Act or a group of practitionerspracticing in partnership, shall only print or have printed on letterheads,account forms and electronic stationery information pertaining tosuch juristic person or partnership practitioners’ -(a)(b)(c)(d)(e)(f)(g)(h)(i)(j)(k)(l)name;profession;registered category;speciality or subspeciality or field of professional practice (if any);registered qualification(s) or other academic qualification(s) orhonorary degrees in abbreviated form;registration number;addresses (including email address);telephone and fax numbers;business hours;practice code number;exemption from registration in terms of section 54A of the Act; anddispensing license number (if any).(3) A practitioner shall not use prescription forms or envelopes on which thename or address of a pharmacist is printed.10 SEPTEMBER 2006
