DISSEMINATION OF BEST PRACTICES TO REDUCE SHS EXPOSURE OF CHILDREN: SMOKE-FREE HOMES/PEDIATRIC TOBACCO CHAMPIONS Dana Best, MD, MPH The overall goal of the Dissemination of Best Practices project is the dissemination of Smoke Free Champions training interventions and other educational activities to pediatric clinicians and their administrative staff. This training intervention is disseminated at state/regional/national conferences and in clinical practice settings. The objectives of the training intervention are to 1) educate pediatric clinicians on the harms of tobacco use and SHS exposure; 2) disseminate best practices and tools in parental smoking cessation counseling and SHS reduction that can be implemented in the pediatric practice setting and; 3) change clinical practice to improve counseling of families to reduce and eliminate tobacco use and SHS exposure. Subsequent to a Dissemination of Best Practices Project Planning meeting in 2006, a pilot study of Smoke Free Champions Training was conducted at three practices in Virginia, and a Dissemination of Best Practices to Promote Smoke Free Homes Conference took place on July 27, 2007 in Richmond, VA. Fifteen AAP Chapter leaders participated in this session. The training and prework/collaborative learning was difficult to implement, leading to reassessment of strategies to engage pediatricians and AAP Chapters in commitments to implementation over time. Further training in conjunction with the AAP’s Future of Pediatrics conference was held in the winter of 2009. Several bestpractice resources for clinicians and researchers are now available on the Richmond Center and SmokeFree Homes Web sites (www.kidslivesmokefree.org), which provide comprehensive training for pediatric clinicians in brief, effective methods to reduce childrens SHS exposure through parental smoking cessation and harm reduction. Three Disseminating Best Practices/Smokefree Homes free online CME modules are also available on the Web. These were publicized through an AAP News article and a card stock advertisement in the February 2008 issue. A Disseminating Best Practices workshop in tobacco control was accepted for the 2008 American Psychological Association (APA) meeting in Hawaii in May 2008, and also for the AAP National Conference and Exhibition in October, 2008. Dr. Best served as the visiting lecturer for two of the 2007 Visiting Lecture awardees, and is working with Dr. Kristen Anderson to help her prepare curriculum materials for use in pediatric residency training. Dr. Anderson will be presenting on the impact of her project and how the visiting lecture helped establish their tobacco curriculum to the Association of Pediatric Program Directors (APPD) in May, 2008. A follow up survey was conducted of participants in the 2005 EPA/FAMRI supported SmokeFree Homes/Tobacco Champions conference was conducted. An abstract based on this was accepted for the 2008 PAS meetings. The availability of anti-smoking materials was also publicized in the AAP On-Call and other venues; materials now also include a new CDC Office of Smoking and Health English/Spanish-language toolkit to help promote in-home protection against SHS in Hispanic/Latino populations. RAPID QUANTITATIVE ASSESSMENT OF SECOND HAND TOBACCO SMOKE EXPOSURE FOR USE IN CLINICAL PEDIATRIC SETTINGS (MEASUREMENT) Dana Best, MD, MPH The overall goal of the project is to develop and pilot a rapid in-office test for SHS exposure of children aged 0-6 years for use in the pediatric practice setting. The test will be designed to provide immediate feedback to parents regarding their child’s SHS exposure. This project also includes collaborative activities between the Richmond Center and the FAMRI-Bland Land Center at UCSF. Current project activities include 1) producing a manuscript summarizing ways to measure SHS exposure of children, including a complete review of studies reporting such measures; 2) conducting a study to address the feasibility, P A G E 4 3
