Conclusions
An initial list of high-alert medications for patients with chronic diseases
has been develop. This list should serve to facilitate the development of
effective safe practices designed to avoid the most frequent errors with
these medications and to identify those patients who may be at a higher risk
of suffering serious harm when medication errors occur and for whom the
implementation of these practices should be considered a priority.
