Location:<br />

The post is primarily based at the Royal <strong>London</strong> Hospital and the Centre for Gastroenterology,<br />

Barts and the <strong>London</strong> (Adult & Paediatric), Queen Mary’s School of Medicine & Dentistry.<br />

Academic Gastroenterology<br />

Laboratory research is conducted in the Centre for Gastroenterology within the new Medical School<br />

building at the Whitechapel Site, the Institute of Cell and Molecular Science. (For details please<br />

see: http://www.icms.qmul.ac.uk/centres/gastroenterology/index.html). Candidates will be able to<br />

select their research supervisor within the Centre/Medical School, which includes<br />

Neurogastroenterology (Professor Qasim Aziz), GI genetics (Professor David van Heel),<br />

Hepatology (Professor Graham Foster, Dr Patrick Kennedy), Growth factors and repair (Professor<br />

Ray Playford), Epithelial cell molecular biology (Professor Ian Sanderson) Immunology (Professor


Tom MacDonald or Professor Ping Wang), Tropical Gastroenterology/infectious disease (Dr Paul<br />

Kelly or Dr Vincent McDonald) Clinical IBD research (Professor David Rampton and Dr James<br />

Lindsay) and intestinal cell biology (Professor Sir Nicholas Wright). It is expected that the field<br />

selected will develop from the expertise the candidate has developed as a PhD.<br />

Clinical Department of Gastroenterology<br />

This is based at the Royal <strong>London</strong> Hospital with additional out patients at St Bartholomew’s<br />

Hospital and some emergency endoscopy at the <strong>London</strong> Chest Hospital and at St Bartholomew’s.<br />

The Digestive Disorders Clinical Group integrates luminal and pancreaticobiliary<br />

gastroenterology, hepatology, IBD and nutrition in three firms as detailed below. An integrated<br />

multi-disciplinary service is run within the department and in conjunction with other clinical<br />

groups, most importantly surgery, pathology and radiology. There are also close links with the<br />

clinical department of paediatric gastroenterology allowing opportunities for extra experience and<br />

training (e.g. in adolescent medicine). There is a high volume of clinical activity within the unit<br />

which sees 8000 out-patients (2500 new), performs ~7000 endoscopic procedures and admits<br />

~500 patients annually. The Clinical service is directed by Dr Richard Marley, Consultant<br />

Gastroenterologist.<br />

The Department offers a wide range of training opportunities with special interests in:<br />

� Clinical Nutrition covers inpatient parenteral and enteral nutrition, and has the third largest UK<br />

home parenteral nutrition service.<br />

� Diagnostic and Therapeutic Endoscopy: ERCP, sphincter of Oddi manometry, colonoscopy,<br />

endoscopic mucosal resection, APC, stenting, endoscopic ultrasound, and wireless capsule<br />

endoscopy. PDT and Nd-YAG laser work should come on-stream this year.<br />

� Gastrointestinal cancer – Upper GI, Lower GI and HPB.<br />

� Inflammatory bowel disease<br />

� Non-transplant hepatology, particularly viral hepatitis<br />

� Physiology: GI motility, 24 hour pH studies, ano-rectal physiology<br />

� Neurogastroenterology and functional bowel disorders<br />

Endoscopy training is provided within a modern 5-roomed unit (opened 1999) with integrated<br />

screening which takes part in the National Endoscopy Programme. We expect to start screening<br />

patients in the National Bowel Cancer Screening Programme in 2007. There is also a GI<br />

investigation/physiology laboratory in the Endoscopy Unit.<br />

All clinical staff are actively involved in the teaching of both under- and post-graduates and<br />

ongoing research, audit and Clinical Governance.<br />

Teams also care for general medical admissions. There are currently 3 SHOs and 4 Pre-registration<br />

House Officers (F1 grade), as well as 2 specialist nurses in nutrition, 2 in Gastroenterology and<br />

Hepatology.


Six Specialist Registrars are attached to the unit (including this post and an ACF). Two senior posts<br />

provide opportunities for specialist interest training and also spend some time at Barts. Three junior<br />

posts rotate every 4 months between the 3 teams. The sixth post is a sixth year post exclusively in<br />

Hepatology, and rotates between two <strong>London</strong> Hepatology Units during the year.<br />

Clinical Governance<br />

There is a monthly Clinical Governance meeting (afternoon of the first Thursday of each month).<br />

SpRs are expected to take a full part, and will be expected to complete and present at least one audit<br />

to the meeting during their tenure.<br />

General Internal Medicine<br />

All the SpRs (except the Hepatologist) take part in the medical ‘take’ rota (currently 1:8, either day<br />

or night, however this is subject to change). Admissions average 25 per 24 hour period (range 15-<br />

35). There is a ward-based system of triage to specialties the day after admission.<br />

The firm structure the firm structure and general medical provision will change, in order to have<br />

consultant triage. The exact format is being finalised, but it is likely that the SpR equivalents will<br />

spend a short period of time attached exclusively to an ‘acute medical care firm’, with the majority<br />

of the year being spent exclusively in gastroenterology.<br />

Main purpose of the post<br />

The post is designed to provide:<br />

• Comprehensive training in academic gastroenterology including research methods.<br />

• The opportunity to complete clinical training in gastroenterology and general medicine to the<br />

completion of the CCST (50% of time will be spent in the clinical service and 50% in academic<br />

postdoctoral training)<br />

• The opportunity to assist in, and contribute to, the development and delivery of the<br />

undergraduate programme in medicine and gastroenterology and postgraduate programme in<br />

gastroenterology including the MSc in Gastroenterology.<br />

• A generic Monday afternoon training programme in academic medicine with clinical lecturers<br />

and academic clinical fellows in other areas of medicine, surgery and dentistry<br />

Main duties of the post<br />

Research<br />

The post provides opportunity for postdoctoral research in both clinical and laboratory aspects of<br />

Gastroenterology in collaboration with the candidate’s chosen supervisor and other investigators in<br />

the Centre of Gastroenterology.<br />

Teaching<br />

The post holder also plays a major role in the organisation and running of the teaching and<br />

assessment within the centre. This department teaches medicine to students from St Bartholomew’s<br />

& The Royal <strong>London</strong> School of Medicine & Dentistry.<br />

The post holder will also contribute to postgraduate teaching within the speciality of<br />

gastroenterology. (including the MSc in Gastroenterology).<br />

Clinical duties<br />

The post holder will act as an SpR in Gastroenterology for 50% of the time. The time between<br />

academic activities and clinical service will be divided into long blocks in association with a<br />

matched Clinical Lecturer.<br />

Clinical training<br />

Applications must have the MRCP (UK) or equivalent and have completed two higher specialist<br />

training (Calman years 1 & 2) or equivalent in gastroenterology, in addition to having submitted a<br />

doctorate. It is expected that the candidate will remain in this position until completion of CCST.<br />

be sympathetically considered.
