In the first case opposite we learn about<br />
blocked fire suppressant pipes, which were<br />
fortunately discovered before they could<br />
contribute to a disaster. This is a clear example<br />
of how a <strong>CHIRP</strong> report can be used to educate<br />
other mariners so they can take preventative<br />
action on their own vessels.<br />
In another case we learn how modern engines<br />
can delay the application of astern power, and<br />
how emergencies can be exacerbated by poor<br />
design. Bad design also features in a report<br />
about pilot boarding arrangements and the<br />
position of lights. Many of these problems<br />
would be avoided if owners arranged for ex -<br />
perienced mariners to be involved during plan<br />
approval and at every stage of construction so<br />
that new vessels are delivered fit for purpose.<br />
In our opinion it is high time that the people<br />
who are expected to sail on ships and make<br />
them work efficiently should be given more say<br />
in their design and construction – it costs<br />
money, but the cost is insignificant compared<br />
to the long-term benefits.<br />
Pipeline pressure surges on oil tankers are<br />
covered and readers are reminded that, if in<br />
doubt, it is always possible to reduce flow rates.<br />
There are also useful lessons about electrical<br />
faults, avoiding electrical overload, and the need<br />
to preserve all your fingers when carrying out<br />
maintenance tasks.<br />
The importance of constant vigilance is<br />
highlighted again in reports about faulty CO 2<br />
systems and defective valves, and there are<br />
several references to the fact that a robust<br />
safety culture on board is vital.<br />
There are lessons here for everyone and there<br />
is much which can be learned, so we urge you<br />
