ShoreTel UK Business Terms and Conditions:<br />

1. This Order Form is confidential information.<br />

2. Provided ShoreTel’s Services are available for use, invoicing for the Service Monthly Commitment<br />

shall take place within sixty (60) days of the Effective Date hereof.<br />

3. Unless otherwise specified in the Order Form, the Services are offered for a minimum term of<br />

twelve (12) months. The Term shall renew in accordance with the ShoreTel Terms of Service<br />

incorporated herein (see link below).<br />

4. Customer may increase Services unlimitedly during the Service Term at the Service Monthly<br />

Commitment prices agreed herein. A one-time implementation fee of £38.00 will be charged per<br />

profile added.<br />

5. Customer is liable for any and all applicable taxes and surcharges.<br />

6. Installation Services bundles include Local Number Porting (LNP) for the first two (2) invoices for<br />

each Location. LNP for each additional invoice is £189.98 per Location.<br />

7. Calls outside England, Scotland, Wales and Northern Ireland, premium-rate telephone numbers (e.g.<br />

Directory assistance calls, 900 numbers, etc.) and toll free inbound calls are billed on a usage basis.<br />

8. Customer is aware that ShoreTel Connect CLOUD does not support inbound calls for certain number<br />

ranges in the UK, specifically numbers with the 084, 087 and 09 prefix. Connect CLOUD does<br />

support outbound calls to 084 and 087 prefixes. See our posted charges for details on the cost per<br />

minute for these calls.<br />

9. Unless otherwise specified, Customer receives only standard features.<br />

IMPORTANT INFORMATION AND ACKNOWLEDEGEMENT (EMERGENCY SERVICE)<br />

If your broadband connection fails, your voice service will also fail. Your service may cease to function if<br />

there is a power cut or failure. These failures may be caused by reasons outside of ShoreTel’s control.<br />

I understand that this service allows calls to the emergency services numbers 999 and 112. However, I<br />

also understand that calls will fail if there is a power cut or my broadband connection fails<br />

By signing this Order Form, Customer agreed to the ShoreTel Terms of Service found at<br />
