ShoreTel Trade-In Program<br />

Customer Requirements<br />

This Addendum refers to ShoreTel’s Customer Trade-In Program (the “Program”) including associated program<br />

discounts and credits. Under the terms and conditions of the Program, Customer has the responsibility for the<br />

decommissioning and destruction of any ShoreTel premises equipment relevant for the trade-in; including<br />

switches, servers and phones that are replaced by the ShoreTel Connect CLOUD service as the customer<br />

migrates from a ShoreTel premise-based telephony system to a ShoreTel Connect CLOUD-hosted telephony<br />

system.<br />

Customer responsibilities under the Program are detailed below. Acknowledgement and execution of this<br />

Addendum constitutes agreement and acceptance of these responsibilities.<br />

Step 1: Decommission Trade-in Product<br />

All ShoreTel equipment associated with the premise-based system and migration to Connect CLOUD must<br />

be decommissioned and removed from operation.<br />

ShoreTel will decommission ShoreTel feature licenses associated with the trade-in.<br />

Customer is responsible to itemise and arrange destruction of trade-in gear using an ISO-14001 certified<br />

e-Waste company, and must provide ShoreTel with confirmation documentation.<br />

After Customer delivers the trade-in equipment to their chosen ISO-14001 certified e-Waste disposal<br />

provider, Customer must provide ShoreTel an appropriate Certificate of Destruction that includes the e-<br />

Waste provider’s company name, address, website and contact information.<br />

All destroyed trade-in equipment and quantities must be listed on the Certificate of Destruction, including<br />

serial numbers for servers and switches. Serial numbers are not required for phones.<br />

The Certificate of Destruction must be sent via email to TradeInCustomerRewards@shoretel.com no<br />

later than 60 days after the ShoreTel Connect CLOUD services are provisioned to Customer.<br />

Step 2: Notice of Decommission<br />

A list of ShoreTel premises licenses to be decommissioned must be emailed to the following<br />

address: TradeInCustomerRewards@shoretel.com. The list of licenses can be found in the<br />

ShoreTel Director on the Licenses page.<br />

ShoreTel will decommission ShoreTel feature licenses associated with the Program.<br />

For Customers migrating their equipment in phases, the Certificate of Destruction and the list of<br />

decommissioned licenses must be sent for each part of the network being decommissioned. The<br />

hardware and licenses must represent a reasonable network construct for the number of onpremises<br />

seats credited under the Program<br />

Step 3: ShoreTel Reconciles Customer Assets<br />

ShoreTel will remove assets from the ShoreTel Customer account record based on the<br />

Certificate of Destruction and list of decommissioned licenses received by ShoreTel.
