SWORN AFFIDAVIT

To the members of the Texas Board of Veterinary Examiners:
This affidavit is to acknowledge that I have received a Cease and Desist Order from the Board as
authorized by the Veterinary Licensing Act, Chapter 801-Veterinarians, Texas Occupations Code. I
have read and understand the provisions of the Veterinary Licensing Act, particularly sections
801.002,801.004 and 801.25lofthe Texas Occupations Code, that relate to the unlawful practice of
veterinary medicine. I understand that the practice of veterinary medicine, as defined in section
801.002 (5), is "the diagnosis, treatment, correction, change,manipulation, relief, or prevention of
animal disease, deformity, defect, injury, or other physical condition, including the prescription or
administration of a drug, biologic, anesthetic, apparatus, or other therapeutic or diagnostic substance or
technique". I also understand that administering a prescription drug to an animal by anyone other
than the animal's owner, an employee of the owner, or a designated caretaker of the animal is the
practice of veterinary medicine under the Veterinary Licensing Act. I further understand an animal's
owner, an employee of the owner, or a designated caretaker may administer prescription drugs to an
animal unless the ownership, employment, or designation is established with the intent to evade the
limitations set by Chapter 801 of the Texas Occupations Code. I affirm that I am not a licensed
veterinarian in the State of Texas.
