Our ServicesPrimary Care ServicesAs your primary care provider, we offer you medical services thatfacilitate the early detection, prevention and treatment of diseases.As your medical home, we will facilitate access to age-appropriateSecondary Care (Specialist), Tertiary Care (Hospital), and AncillaryServices (Physical Therapy). Insurance companies recognize theimportance of preventative care services to the long-term cost ofhealth care: this is why most plans won’t charge you any co-pay.The quality of our care is measured by the percentage of our patientpopulation that receive age appropriate preventative services. This includesthe healthiest patients. For our patients with chronic health issues, we areassessed on the safety of medications we recommend, your compliance withmedications, appropriate use of diagnostic tests, the degree of control ofdiseases such as diabetes, the frequency with which you use the emergencyroom, and your understanding of your overall care.All patients receive a comprehensive health assessment annually.It involves a review of the following: Past Medical History,Surgical History, Family History, Medication History, AllergyHistory, Social Risk Factors for poor health outcomes (smoking,alcohol use, illicit drug use, inability to afford medications, lack of socialsupport, adverse living conditions), Other Risk Factors (such as poornutrition, risk of falls, inability to take care of daily needs), MmemoryAssessment, Examination for abnormal signs of disease, BodyMass Index and Blood Pressure measurement.Chronic Care ServicesPatients with two or more complex medical diagnoses qualify forthe Chronic Care Management Program. These patients willreceive a written Plan of Care. They may receive phone callsbetween their appointments to assist them in overcoming carebarriers. They will have direct access to a Medical Assistant tohelp them access needed services. Patients will sign an agreementto receive chronic care services.
Things You Should KnowAppointmentsPatients are seen by appointment. Ifyou cannot make your appointment,please call the office and cancel it.There is a $25 No Show charge if youdo not call to cancel. Barringemergencies, we try our best to seeyou at your scheduled time. If youhave not been attended to within 15minutes of arrival, please alert thereceptionist.For your comfort, we have separateseating areas for our clients. Wellfamilies are seated in the play room.Well adults are seated in the largewaiting room. Sick adults andchildren are seated in our familyroom left of the reception desk.Visit TypesAs your primary care provider, wewill perform a comprehensivephysical/well visit annually. Atthis visit, we will identify riskfactors and barriers to your havinga healthy life. You will be providedwith a Plan of Care andrecommendations including cancerscreens and immunizations. Mostinsurances strongly recommendthis and will cover your visit.During chronic diseasemanagement or routine officevisits, we will review your chronicdiagnoses, such as asthma, refillmedications, and review testresults. The recommendedintervals for your appointmentsare based on best practicestandards to ensure your safety.A sick visit does not replace yourchronic disease managementvisit. An extended visit willcombine routine and physicalvisit features.MedicationFor your safety, prescriptionrequests will be handled onlyduring office hours. Narcoticsprescriptions will never be calledin outside of office hours.Medications are reviewed andrefills provided at your officevisit. Please bring medicationbottles to your appointment forverification. Call-backs for refillsafter an appointment will becharged $10.For your convenience, wedispense some medications inthe office.This saves you a trip tothe pharmacy when you leastwant to go.Diagnostic ServicesAs part of our commitment toprimary prevention and earlydetection of disease, we alsooffer cardiac screening whenappropriate and lung functiontesting.
