1 Surgical Anatomy of Upper Cervical Spine11Fig. 1.14 Anterior of axisafter splitting the tongue andposterior pharyngeal wallanterior tubercleof atlasmusculus longuscollitendonMandible (splited)Tongue (splited)epiglottisthyroid cartilageThere are various extended transoral approaches:Transmandibular circumglossal approach:Median mandibulotomy without glossotomy:The mandible is split without cutting through the skinof lower lip in this approach.This approach only requires stealth incision of themucosa between the lower lip and mandible beforesawing open the mandible, and the superior margin ofC3 is exposed after stealth peeling of the periosteum.Median mandibulotomy without glossotomy is suitablefor patients with ventral lesions in upper cervicalspine accompanied with difficulty opening the mouth.This approach leaves no scars after the operationand thus has cosmetical significance.The mandible is split, and the lateral side of the tongueis split through the sublingual muscles without cuttingthe tongue body.Although this approach results in little trauma, theexposure is smaller than the former approach.Transmandibular circumglossal approach is suitablefor patients with ventral lesions in the upper andlower cervical spine and lesions that occur on one side.The hypoglossal nerve should be isolated and protectedif applying the paramedian mandibulotomyapproach.
