INTRODUCING THE MY FATHER'S HOUSE-ANNEX AS <strong>CCM</strong>’S NEWEST COMPONENT IN OUR<br />
TRANSITIONAL HOUSING CONTINUUM. The program is built on <strong>CCM</strong>'s comprehensive Path to Stability<br />
model and is structured to include expectant and newly parenting mothers through a partnership with<br />
the SUN Clinic at the Cabarrus Health Alliance (CHA) and through funding from the McKenzie Opioid<br />
Settlement Grant.<br />
The “Annex” represents a collaborative between<br />
Memorial Baptist Church of Kannapolis, Cabarrus<br />
Health Alliance, Atrium Health - Cabarrus and<br />
Cannon Charitable Interests. The facility will<br />
provide housing and services for five resident<br />
families and will complete the replacement of<br />
capacity lost in 2022 with the closing of My<br />
Father's House.<br />
Through this partnership, CHA will work<br />
collaboratively with <strong>CCM</strong> to support these<br />
women and children. This partnership will<br />
encourage continued recovery through ongoing<br />
participation in the SUN Clinic program<br />
which includes behavioral health counseling,<br />
medication management, peer support specialist<br />
services and a host of wrap around services<br />
provided by SUN Project community partners.<br />
