On one hand the demand for less invasive face lift and alternative procedures is a strong driving force behind the<br />

cosmetic surgery market.<br />

2000 M. Sulamanidze opened the door for the thread lift technology with his APTOS-threads. A lot of modifications<br />

inclusive Anchor sutures and a lot of possible combinations came to the market. The surgical anatomy of the aging<br />

face is the basic for all thread procedures.<br />

(03) FACIAL RHYTIDECTOMY – THE DERMATOLOGIC FACELIFT<br />

Lifting procedures are designed to reverse gravitational aging and ideally should act in a vertical upward vector.<br />

A minimal incision facelift in combination with liposuction of the neck and jowls can achieve substantial improve-<br />

ment in gravitational aging of the face and neck. In this course we will discuss the „dermatologic approach“ to<br />

rhytidectomy. This approach is characterized by the use of tumescent local anesthesia, which minimizes the need<br />

for sedation.Tumescent anesthesia, because of vasoconstriction, minimizes post-operative bruising and swelling,<br />

thus speeding recovery.<br />

We will discuss anatomy of the face and neck relevant to liposuction and rhytidectomy, including SMAS and deep<br />

fasciae, motor and sensory nerves, and vasculature. The technique for administering tumescent local anesthesia<br />

and for performing liposuction of the neck and jowls and minimal incision rhytidectomy will be discussed. Avo-<br />

idance of surgical complications will be considered, as will management of complications that do occur.<br />
