On November 24th an <strong>update</strong> will be posted to all <strong>SpEd</strong> <strong>Forms</strong> sites. This <strong>update</strong> will include all<br />

changes detailed below.<br />

Student Setup<br />

1. Added a “Relationship to child” link that when clicked will show a pop up with more information.<br />

2. The Date of Birth is now a required field on the Student Setup page.<br />

1. The date control used throughout the program will show years in the year drop down beginning<br />

21 years in the past up to 3 years in the future (custom years can still be entered manually if<br />

Student Transfers<br />

1. The “ISD#” is now shown for educators to help differentiate users with the same names on the<br />

Student Sharing page.<br />

IFSP<br />

1. The Signature / Primary disability and team membership page can now be printed separately. A<br />

print preview and print button have been added to the page.<br />

2. Added the “Autism Spectrum Rating Scales (ASRS)” Test.<br />

Individualized Education Program<br />

1. Added two new Services (#13) drop down options:<br />

a. Specialized Instruction: Oral Expression<br />

b. Physical/Health Disabilities Consultation<br />

2. On the Goals page, a new red warning message is shown stating “<strong>Inc</strong>lude information on how<br />

the child’s disability affects the child’s involvement and progress in the general education<br />

curriculum.”<br />

3. The Progress Report will now print the students’ current grade rather than the grade the<br />

student was in when the Service Plan was finalized.<br />

Evaluation Plan/Prior Written Notice<br />

1. On the Eval/Reevaluation Plan Prior Written Notice and the Prior Written Notice/Consent forms,<br />

the team member name entered in the “You are protected by procedural safeguards and can<br />

get…” section are now separate from the IEP Team Member name when saved. The IEP Team<br />

Members name will still populate the fields by default.<br />

Parental Consent/Objection<br />

1. The Date of the form is no longer editable and will always be the date of the Prior Written<br />

Notice.<br />

1. The date field for the Parent Signature has been removed from the form page. A line will still<br />

print on the print page for the Parent to write a date.<br />

Prior Written Notice Part C<br />

1. Updated the page per the MDE state model form. Added a check box and the associated<br />

wording: “If this box is checked, the district has determined that your child is not eligible for<br />

services under Part C of the Individuals with Disabilities Education Act (IDEA). As a parent, you<br />

have a right to dispute this eligibility determination through alternative dispute resolution<br />

options such as mediation, filing a state complaint or requesting a due process hearing. Basic<br />

information about these options is included in this notice. In depth information about these<br />

options can be found in the attached Procedural Safeguards Notice.”<br />

Procedural Safeguards Part C<br />

1. The English, Spanish, Hmong and Somali versions have been <strong>update</strong>d per MDEs state model<br />

forms.<br />

ECSE Referral for Screening/Evaluation<br />

1. This form can now be finalized.<br />

Communication Log<br />

1. The “Relationship” drop down can now have custom entries entered. The new drop down is<br />

“Communication Log Relationship” (#141).<br />

Teacher Reports<br />

1. The Attendance Report can now be filtered by Student ID, First Name and Last Name.<br />

Admin Reporting<br />

1. Modified the Teacher Workload Analysis report and the Services report extensively. There is<br />

now a “Service Plan” drop down to choose All, IEP, IFSP, IIIP, ISP and 504. The “Service type”<br />

drop down has been modified to include “<strong>SpEd</strong> and Related/Early Intervention Services” which<br />

will pull services for the IFSP as well (based on the “Plan type” selected).<br />

Admin Organization<br />

1. You can now print individual Evaluation Report Templates from the menu pages, you no longer<br />

need to access the specific section to print.<br />

1. The Positive Behavior Support Plan will now finalize and print as a PDF.<br />

MA Reports<br />

2. The Student Claims report can now be filtered by Provider, Insurance and Finalized/Not<br />

Finalized. You can now also edit the students “Bill to” date, “MA Eligibility” as well as the<br />

“Consent to Release Private Data” in the results. The “Signature on File” check box has been<br />

removed.<br />

3. The Reconciliation report will now show log lines when searching by provider even if a different<br />

provider created the log.<br />

4. Improved the navigation for the Student Claims report when viewing logs and the student setup.<br />

5. Added a “Not finalized” check box to the Create/edit Transmission reports search criteria.<br />

6. We have made some extensive changes to the Reconciliation and Total Amounts paid report to<br />

calculate the amount paid based on the paid date. After the next <strong>update</strong> totals will only be<br />

displayed if the claim has been <strong>update</strong>d with a date and amount paid. Also included in these<br />

changes were new rate adjustments.<br />

7. Added a “Billable” check box to the Create/Edit Transmission report that is checked by default.<br />

8. For the Remark Codes links on the Create/Edit Claims Transmission report, those links now point<br />

to the Washington Publishing Company for the Remark and Adjustment codes. The link will


ing you to the list of codes, not the individual reason. This way you can scan the list for the<br />

code received on your remittance advice. Because of the length of the list and the frequency of<br />

codes being dropped and added, we felt the full list would be the most efficient resolution.<br />

9. The Create eligibility transmission (270) report will now allow more than 99 claims to be sent.<br />

Please be aware that claims with over 500 students may have issues being imported so splitting<br />

the 270s into exports with 500 or less students is recommended.<br />

10. When editing a Service or Student from the results of the Create/Edit Claims Transmission<br />

report, previously you were taken back to the top of the report. You are not taken back to the<br />

position on the page you were previously at.<br />

MA <strong>Forms</strong><br />

1. Modified the Activity and Trip logs. When service providers enter activity logs, they will be<br />

warned to add time, number in a group and a description. The log will not be able to be finalized<br />

until the information has been entered. We have also added the validation to check for<br />

instructional days. This will be a warning for now just in case some schools are not entering their<br />

instructional days. We hope these additions will reduce the amount of time checking errors<br />

during the batch process.<br />

2. The Create/Edit Claims Transmission report will now show warnings if a student is 21 years of<br />

age or older.<br />

3. For PCA logs, in the “Service providers” section, we have added the ability to click on the Team<br />

member icon and choose a user from the list. Similar to how team members are chosen on<br />

other forms throughout <strong>SpEd</strong> <strong>Forms</strong>.<br />

4. Both the Activity and Trip logs have been <strong>update</strong>d so that now only the supervisor is able to sign<br />

the logs. (The logged in user needs to be the supervisor chosen in the drop down).<br />

5. The MA Release of Information form is now available on the Form Menu for IFSP Plan Types.<br />
