This work has been achieved thanks to several grants from the Swiss National Science<br />

Foundation and in collaboration with the Seminar für Statistik, ETH-Zürich. Part of<br />

ROBETH has been included into the NAG library, Numerical Algorithms Group Ltd,<br />

Oxford, UK, (Mark 13 and above) and into the statistical system S-plus.<br />

Current research<br />

1 Robust methods for the analysis of hospital length and cost of stay<br />

This project develops methods to estimate the mean length of stay (LOS) and the mean<br />

cost of hospital stays as a function of available administrative and medical data, which<br />

often contain extremely expensive or unusually cheep cases.<br />

Hospital stays are usually grouped according to a given Patient Classification System<br />

(PCS), an important tool of hospital management. The classes are usually called Diagnosis<br />

Related Groups or DRGs. PCSs are used for epidemiological monitoring and for<br />

standardised comparison of hospital activity. The main applications are however hospital<br />

budgeting, funding and reimbursement. For this purpose, a mean cost of each class is<br />

usually estimated with the help of available information on a national basis. The cost<br />

means are then used as a basis for negotiation to fix prospective payment or retrospective<br />

reimbursement rates.<br />

The analysis of LOS and cost of stay requires appropriate statistical tool to take into<br />

account the following features of data: (a) LOS and cost distributions are asymmetric<br />

and heteroscedastic; (b) LOS and cost distributions contain outliers. A few outliers can<br />

completely distort both the means and the comparisons based on them. As a remedy,<br />

various kinds rules for removing outliers are currently used in practice; unfortunately,<br />

these rules lack of theoretical ground.<br />

Preliminary research (e.g., <strong>Marazzi</strong>, Paccaud, Ruffieux, and Beguin, 1998) showed that<br />

the conceptual framework of parametric robust statistics is a very attractive basis for the<br />

development of adequate statistical tool. Therefore, as a first step, new robust rules for<br />

trimming atypical values have been developed (<strong>Marazzi</strong> and Ruffieux 1999; Ruffieux, Paccaud,<br />

and <strong>Marazzi</strong> 2000 ). As a second step, tool for modeling LOS and cost distributions<br />

with the help of covariates, is being developed (Project 2).<br />

<strong>Marazzi</strong>, Gardiol and Duong (2007) propose reimbursement schemes based on PCSs that<br />

include adjustments for LOS and exceptional costs and are designed to minimize undesirable<br />

effects of economic incentives. In addition, in this paper, a novel statistical approach<br />

to compare the schemes and the underlying PCSs is proposed.<br />

2 Robust methods for positive asymmetrically distributed responses<br />

Project 1 motivates the development of general robust statistical methods to estimate<br />

the mean of a positive and asymmetrically distributed random variable as a function of<br />

a number of covariates, in the presence of outliers. The following steps have been made<br />

(papers can be downloaded from www.iumsp.ch).<br />

1. Development of M-estimates for the parameters of the Gammma, Weibull, and Lognormal<br />

distributions (<strong>Marazzi</strong> and Ruffieux, 1996). This kind of estimate is well known in<br />

theory but too complex for applications in hospital management.<br />

2. Development of “truncated means” of asymmetric distributions using parametric models<br />

(<strong>Marazzi</strong> and Ruffieux 1999). Although the statistical performance of this kind of estimate<br />

is not optimal, it is the method of choice for practitioners in hospital management.<br />

3. Bootstrap tests for robust means of asymmetric distributions (<strong>Marazzi</strong> 2002). Simple<br />

inference procedures for comparing LOS or cost means using samples of stays.<br />

4. Robust parametric means of asymmetric distributions: estimation and testing (<strong>Marazzi</strong><br />

and Barbati 2003). Extension of the methods developed in steps 2–3.<br />

5. Adaptively truncated maximum likelihood regression with asymmetric errors (<strong>Marazzi</strong><br />

and Yohai 2004). Extension of the methods developed in steps 2–4 to multiple regression.<br />

6. Development of robust regression methods for asymmetrically distributed responses using<br />

Box-Cox transformations (<strong>Marazzi</strong> and Yohai 2004; 2006; <strong>Marazzi</strong>, Villar, Yohai<br />

2009). This is a robust approach to nonlinear regression with asymmetric and heteroscedastic<br />

errors.<br />

7. Extension of robust regression for asymmetrically distributed responses to survival analysis.<br />

These methods are useful for censored data, e.g., incomplete follow up, incomplete<br />

hospital stays. e.g., transfer to a different hospital. (Locatelli, <strong>Marazzi</strong>, Yohai, 2010). A<br />

package for robust accelerate failure time regression (RobustAFT) has been made available<br />

on The Comprehensive R Archive Network (CRAN) of the statistical environment<br />

R (<strong>Marazzi</strong>, Muralti, 2011).<br />

8. Robust estimation of the generalized log-gamma regression model. (<strong>Marazzi</strong>, Yohai,<br />

Agostinelli, 2011). Work in progress.<br />

3 Maintenance and development of ROBETH<br />

The main purposes of this project are:<br />

- to include the new tool developed under Project 2 into the ROBETH software;<br />

- to maintain ROBETH and its interface to S-plus;<br />

- to implement the methods of ROBETH into the R-software (http://www.r-project.org/).<br />
