CHECKLIST
What do you need to do to register for the course? READ CAREFULLY
Certification of a Photocopied Document
For us to accept a photocopy of a document, you must take the original document (passport, birth certificate, diploma or
grading list) to a recognised official (eg Ministry of Foreign Affairs, Netherlands Embassy or diplomatic representative in
the country where the certificate was issued, a lawyer/notary public, senior civil servant or Embassy official) and get
them to make a photocopy and put their official stamp/seal on it, and state that the document is a true copy of the
original. If there is text on both sides of the document then both sides should be copied. Ensure that the copy is
readable and can be photocopied by us.
The Certifier should confirm that the copy document is of “a true original” and should sign the copy document (printing
his/her name clearly in capitals underneath). The Certifier must also clearly indicate his/her position or capacity as a
Certifier (i.e. an indication that he/she is a lawyer etc) and stamp the certified copy with his/her business stamp or
provide certification on business stationery.
WE DO NOT ACCEPT HOME MADE PHOTOCOPIES WITHOUT CERTIFICATION!
Send:
Application form (Complete this on computer, print out and send signed original)
Motivation letter (Complete this on computer, print out and send signed original. Put your name and address
on this.)
Documents listed below:
1 A certified photocopy of your valid passport. Your passport has to be valid for at least 15 months after arrival
in the Netherlands.
2 Birth Certificate For students from the Dominican Republic, Ghana, India, Nigeria, Pakistan – your birth
certificate must be both legalised and ‘verified’. This means that the Netherlands embassy in the country in
question must examine the document and declare it valid.
3 Education You have to prove, through means of diplomas and the list of marks, that you have had sufficient
prior education to be able to begin the course. We request that you send certified copies of your diploma and
subject/grade lists and not originals as we cannot be held responsible for their safe receipt or return.
If you are still studying ask the school to print out your current grades and subjects to date on their headed
paper and stamp and sign this. We can then get preliminary checks done ahead of your getting your final
diploma. You must have your final diploma before 1 st September. If not contact AMFI Student Administration.
Certification in this instance may be carried out by the Director of your school. Instructions for the
certification/Certifier are in the “Certification” paragraph at the top of this list.
The only originals we accept are translations of documents (on the official headed paper of the translator – see
below) or transcripts from American schools (we also need your diploma if you apply with an American
education).
Translations
Diploma’s in any language other than English, French, German, or Dutch should be translated into English by a
sworn translator. Certified photocopies of the original documents and the original translations should be
supplied together.
4 English Level Please send your tests results from IELTS, TOEFL or the Cambridge Certificate. You are only
exempt from this if you have completed your full secondary education in English. If you have not done a test
