• Brazil has more than 246,<strong>000</strong> community health agents serv<strong>in</strong>g 120 million<br />

people (63 percent of <strong>the</strong> population). The health agents make home visits<br />

where <strong>the</strong>y promote healthy practices such as breastfeed<strong>in</strong>g, monitor <strong>the</strong><br />

growth of children and counsel on follow up, and provide simple treatments<br />

such as oral rehydration solution for diarrhea. These health workers are<br />

residents of <strong>the</strong> communities <strong>the</strong>y serve and are selected <strong>in</strong> a public process<br />

with strong community engagement. The health worker program has been<br />

<strong>in</strong> place nationally s<strong>in</strong>ce <strong>the</strong> early 1990s.123 S<strong>in</strong>ce that time <strong>the</strong>re has been<br />

over a 90 percent decl<strong>in</strong>e <strong>in</strong> diarrhea-related mortality,124 and stunt<strong>in</strong>g has<br />

been reduced from 19 to 7 percent.125<br />

• Pakistan began tra<strong>in</strong><strong>in</strong>g and deploy<strong>in</strong>g “Lady Health Workers” <strong>in</strong> 1994.<br />

There are now more than 90,<strong>000</strong> female health workers throughout <strong>the</strong><br />

country, serv<strong>in</strong>g 70 percent of <strong>the</strong> rural population.126 Lady Health Workers<br />

focus largely on essential maternal and newborn care. Their tra<strong>in</strong><strong>in</strong>g emphasizes<br />

maternal nutrition, iron and folate use, rest dur<strong>in</strong>g pregnancy and<br />

promotion of breastfeed<strong>in</strong>g. Each<br />

Lady Health Worker looks after<br />

a population of about 1,<strong>000</strong> <strong>in</strong>dividuals.<br />

At group meet<strong>in</strong>gs, she will<br />

discuss issues related to better health,<br />

hygiene, nutrition, sanitation and<br />

family plann<strong>in</strong>g, emphasiz<strong>in</strong>g <strong>the</strong>ir<br />

benefits towards improved quality<br />

of life. In household visits, she<br />

will treat iron deficiency anemia <strong>in</strong><br />

women and young children, and<br />

provide nutritional education with<br />

emphasis on breastfeed<strong>in</strong>g and complementary<br />

feed<strong>in</strong>g practices, and<br />

maternal nutrition, <strong>in</strong>clud<strong>in</strong>g ways<br />

to reduce micronutrient malnutrition.127<br />

Pakistan still does poorly on<br />

breastfeed<strong>in</strong>g <strong>in</strong>dicators, but trends<br />

are mov<strong>in</strong>g <strong>in</strong> <strong>the</strong> right direction.<br />

Exclusive breastfeed<strong>in</strong>g rates <strong>in</strong>creased<br />

from 23 percent <strong>in</strong> 1990/91 to 37 percent<br />

<strong>in</strong> 2006/07. Dur<strong>in</strong>g that same<br />

period, rates of early <strong>in</strong>itiation rose<br />

three-fold, from 9 to 29 percent.128<br />

Over roughly <strong>the</strong> same period (1990-<br />

2008), maternal mortality dropped by<br />

nearly half.129<br />

Greater <strong>in</strong>vestments are needed to<br />

recruit, tra<strong>in</strong> and supervise/support<br />

more frontl<strong>in</strong>e health workers to build<br />

on <strong>the</strong>se successes. WHO estimates<br />

<strong>the</strong>re is a shortage of at least 1 million<br />

frontl<strong>in</strong>e health workers <strong>in</strong> <strong>the</strong><br />

develop<strong>in</strong>g world.130 And many exist<strong>in</strong>g<br />

health workers could do more to<br />

fight malnutrition if <strong>the</strong>y had better<br />

tra<strong>in</strong><strong>in</strong>g, equipment and support.131<br />
