Introducción<br />
El metotrexato (MTX) nació en 1948. Por aquél entonces,<br />
Sydney Farber, un conocido pediatra del Children´s<br />
Hospit<strong>al</strong> de Boston investigaba el papel del ácido fólico<br />
en el cáncer. Su búsqueda de antimetabolitos del folato<br />
dio como resultado la elaboración del MTX, cuyo<br />
uso como agente antineoplásico fue aprobado por la<br />
FDA en 1953.<br />
Esenci<strong>al</strong>mente, el MTX inhibe de forma competitiva<br />
la tetrahidrofolato reductasa (THFR) que es un<br />
enzima implicado en la síntesis del ácido fólico. El<br />
fármaco, que es mil veces más afín por dicho enzima<br />
que el ácido fólico, frena la síntesis de éste último,<br />
el cu<strong>al</strong> a su vez es necesario para la síntesis de<br />
novo del nucleótido timidina y por ende del DNA.<br />
La droga también está implicada en el freno de la<br />
timidilato sintetasa y de determinadas reacciones<br />
de transmetilación que así mismo conllevan un serio<br />
menoscabo en la síntesis de DNA y RNA, y por<br />
tanto en la proliferación celular. Sabemos que el<br />
MTX actúa en la fase S del ciclo celular en especi<strong>al</strong><br />
de aquellas células caracterizadas por una rápida<br />
proliferación.<br />
Así pues, el MTX se configuraba como una potente<br />
droga antineoplásica en sus inicios por lo que a dosis<br />
<strong>al</strong>tas se ha venido empleando en leucemias, osteosarcoma,<br />
tumores trofoblásticos y en determinados<br />
tumores de mama. Con el transcurso del tiempo<br />
y en base a su reconocido efecto antiinflamatorio,<br />
se ensayaron dosis más bajas en enfermedades in-<br />
