CHOLECYSTOSTOMIE PERCUTANEE TRANS-HEPATIQUE SPIRA RM. Percutaneous transhepatic cholecystostomy and delayed laparoscopic cholecystectomy in critically ill patients with acute calculus cholecystitis. Am J Surg 2002 • 55 patients sur une période de 5 ans. • 23 en choc septique ou sepsis sévère (42%). • CPTH réalisée avec succès chez 54 patients / 55 (98%). - 1 hématome hépatique (laparotomie) Complications - 9 déplacements de cathéters - 3 décès par MOF • Amélioration clinique rapide (59% à 24h – 96% à 72h). • Durée moyenne d’hospitalisation: 15,5 +/- 11,4 jours. • 44 patients de retour à domicile, cathéter en place. • 30 patients opérés à distance (2 laparotomies – 28 cœlioscopies dont 4 conversions) • CPTH sûre et efficace chez des patients fragiles.
