In our study, we aimed that investigated the relationship betweenskin toxicities and vitamin D levels in the patients with EGFR mutant and treated with erlotinib.Methods: EGFR mutant patients that admitted to Medical Oncology department of Atatürk Chest andChest Surgery Hospital between 2014 – 2018 are included in this study. The patient’s data were collectedretrospectively.Results: Totally, 78 patients that have EGFR mutation and treated with erlotinib are included in thisstudy. The median level of vitamin D at the diagnosis was 34.6 (5-88) ng/ml. The patients were stratifiedtwo group according to vitamin D level. The 42 of whom (53.8%) had vitamin D level < 20 ng/ml and36 patients (46.2%) had vitamin D level ≥ 20 ng/ml. Grade 2 -3 skin toxicities were significantly higherin the patients had vitamin D level < 20 ng/ml (p: 0.038).Discussion and conclusion: In our study, we found that the higher grade erlotinib related skin toxicitieswere more frequently in the patients that had low vitamin D level. Both of the erlotinib treatment andlow vitamin D level may have contributed to the inhibition of keratinocyte proliferation, differentiationand cell cycles.Keywords: EGFR mutation, Skin toxicities, Vitamin DÖZETGiriş ve amaç: Epidermal büyüme faktörü reseptörü(EGFR) mutasyonu akciğer kanseri kanserindekullanılan tirozin kinaz inhibitörlerinin ek sık yan etkisi cilt toksisitesidir. Çalışmamızda hastanemizdeEGFR mutasyonu pozitif saptanan ve erlotinib tedavisi alan hastalarda serum 25(OH) vitamin D3 düzeyiile tedavi sırasında görülen cilt toksisitesi ilişikisinin değerlendirilmesini amaçladık.Yöntem ve gereçler: Ankara Atatürk Göğüs Hastalıkları ve Göğüs Cerrahisi Eğitim ve AraştırmaHastanesi, Tıbbi onkoloji polikliniğine 2014 – 2018 yılları arasında başvurmuş hastalar çalışmaya dahiledilmiş ve verileri retrospektif olarak toplanmıştır.Bulgular: Toplam 78 EGFR mutasyonu saptanan ve erlotinib tedavisi alan hasta çalışmaya dahil edildi.Tanı anında ortalama serum 25 (OH) vitamin D3 düzeyi 34,6 (5-88) ng/ml olarak saptandı. Hastalar tanıanındaki serum 25 (OH) vitamin D3 düzeyi ne göre 2 alt gruba ayrıldı. Toplam 42 (%53,8) hastanınserum 25 (OH) vitamin D3 düzeyi 20 ng/ml in altında 36 (%46,2) hastanın serum 25 (OH) vitamin D3düzeyi 20 ng/ml in üstünde saptandı. Serum 25 (OH) vitamin D3 düzeyi 20 ng/ml in altında olan gruptagrade 2–3 cilt toksisitesi daha faza olarak saptandı (p: 0.038).Tartışma ve sonuç: Çalışmamızda tanı anında vitamin D3 eksikliği saptanan hastalarda erlotinibe bağlıyüksek gradlı cilt toksisitesi gelişme sıklığı daha fazla saptanmıştır. Hem EGFR inihibisyonu hemvitamin d düzeyindeki azalma epidermiste direk olarak keratinosit proliferasyon –differensiasyon vehücre yaşam döngüsünün inhibe olmasına bağlı cilt toksisitesinde artışa yol açmış olabilir.Anahtar Kelimeler: EGFR mutasyonu, Cilt toksisitesi, Vitamin D3First Received: 06.11.2020, Accepted: 30.11.2021doi: 10.5505/aot.2021.71224
